Tafasitamab is a humanized monoclonal antibody that binds to the CD19 antigen, which is expressed in tumor cells from patients with diffuse large B-cell lymphoma (DLBCL). On June 24, 2021, a positive opinion for a conditional marketing authorization was issued by the European Medicines Agency (EMA)'s Committee for Medicinal Products for Human Use (CHMP) for tafasitamab, in combination with lenalidomide, for the treatment of adult patients with relapsed or refractory DLBCL who are ineligible for autologous stem cell transplantation. Tafasitamab was evaluated in the phase 2 single-arm, multicenter, open-label L-MIND clinical trial. The primary endpoint of this trial was objective response rate (ORR). The best ORR, achieved at any time during the study, was 56.8% (95% confidence interval 45.3%-67.8%), and the median duration of response was 34.6 months (95% confidence interval 26.1-not reached). The most frequently reported adverse events by system organ class were infections and infestations (72.8%; grade ≥3 29.6%), blood and lymphatic system disorders (65.4%; grade ≥3 56.8%), gastrointestinal disorders (64.2%; grade ≥3 2.5%), and general disorders and administration site conditions (58.0%; grade ≥3 8.6%). The aim of this article is to summarize the scientific review of the application which led to the positive opinion by the CHMP.Molecular tests have become an indispensable tool for the diagnosis and prognosis of hematological malignancies and are subject to accreditation according to the International Standard ISO 15189. National standardization of these techniques is essential to ensure that patients throughout France benefit from the same care. We report here on the experience of the GBMHM (Groupe des Biologistes Moléculaires des Hémopathies Malignes). By organizing External Evaluation of Quality (EEQ) programs and training meetings, the GBMHM has contributed to improvement and standardization of molecular tests in 64 French laboratories. A retrospective analysis of the quality-control results of 11 national campaigns spanning 10 years was performed for the 3 most frequently prescribed tests BCR-ABL1, JAK2 V617F, and lymphoid clonality. For each test, particular attention was placed on comparing methodologies and their evolution throughout the period. The establishment of the BCR-ABL1, JAK2 V617F, and lymphoid clonality EEQ programs and the associated training meetings have initiated a process of collective standardization concerning the methods of implementation (JAK2 V617F) and the interpretation and formulation of results (lymphoid clonality). In addition, it resulted in objective improvement in technical performance (BCR-ABL1). Our evaluation of the impact of these EEQ programs demonstrates that it is possible to obtain reproducible values across different laboratories in France by applying national recommendations. To our knowledge, this is the first publication that evaluates the impact of a national quality assurance program on improving molecular results in hematology.
To assess the effectiveness of telemedicine video visits in the management of hypertensive patients at home during the first year of the COVID-19 pandemic.

A quantitative analysis was performed of all home video visits coded with a diagnosis of essential hypertension during the first 12 months of the COVID-19 pandemic (March 2020 through February 2021). A total of 10,634 patients with 16,194 hypertension visits were present in our national telemedicine practice database during this time. Among this population, a total of 569 patients who had 1785 hypertension visits met the criteria of having 2 or more blood pressure (BP) readings, with the last BP reading occurring in the report period. We analyzed baseline characteristics and BP trends of these 569 patients during the study period. Voluntarily submitted patient satisfaction ratings, which were systematically requested at the end of each visit, were also analyzed.

The mean age of the patients in this study cohort of 569 patients was 43.9 years, and 48.indicated a high degree of patient satisfaction, with a mean satisfaction score of 4.94 (5-point scale).

Patient satisfaction with these visits was high.The Urban Living Lab (ULL) approach has the potential to create enabling environments for social learning and to be a successful arena for innovative local collaboration in knowledge co-creation and experimentation in the context of research and practice in sustainability transitions. Nevertheless, complex issues such as the urban Food-Water-Energy (FWE) Nexus present a challenge to the realization of such ULL, especially regarding their inclusiveness. We present ULL as a frame for a local knowledge co-creation and participation approach based on the project "Creating Interfaces - Building capacity for integrated governance at the Food-Water-Energy-nexus in cities on the water". This project aims at making FWE Nexus linkages better understandable to the stakeholders (citizens and associations, city government, science, businesses), and to facilitate cooperation and knowledge exchange among them. This paper focuses on and discusses inclusiveness as a key aspect and challenge of ULLs and on what literature and our experiences in this regard suggest for the advancement of the concept of ULL towards ULL 2.0. These findings often also relate to framing transdisciplinary research in a wider sense.With the ever-growing data and the need for developing powerful machine learning models, data owners increasingly depend on various untrusted platforms (e.g., public clouds, edges, and machine learning service providers) for scalable processing or collaborative learning. Thus, sensitive data and models are in danger of unauthorized access, misuse, and privacy compromises. A relatively new body of research confidentially trains machine learning models on protected data to address these concerns. In this survey, we summarize notable studies in this emerging area of research. With a unified framework, we highlight the critical challenges and innovations in outsourcing machine learning confidentially. Epigenetic inhibitor We focus on the cryptographic approaches for confidential machine learning (CML), primarily on model training, while also covering other directions such as perturbation-based approaches and CML in the hardware-assisted computing environment. The discussion will take a holistic way to consider a rich context of the related threat models, security assumptions, design principles, and associated trade-offs amongst data utility, cost, and confidentiality.We are presenting the case of a 6-year-old male patient with progeroid phenotype and severe developmental delay referred to Genetic clinic. Given the complex phenotype an extensive metabolic and genetic evaluation was performed including a whole exome sequencing analysis that showed genetic variants in TTR, RELN, MYH6, PHIP, and SYNE2 genes. Patients' mother and brother were analyzed for the genetic variants in MYH6, PHIP and RELN. Both had same variants on PHIP and RELN as our patient, with no apparent phenotypical consequences. Physical examination was remarkable for dysmorphism including plagiocephaly, low set and abnormally shaped ears, up slanted palpebral fissures, hypoplastic alae nasi, and a head circumference two standard deviations below the 3rd percentile (microcephaly). Other characteristics include wrinkled skin, a broad forehead, sparse eyelashes in lower eyelid, short palpebral fissures, upturned nares, thick lips, right occipital plagiocephaly, overfolded helix and prominent anti-helix, protuberant chest, scaphoid abdomen, digitalized thumbs, and kyphosis due to low muscle tone. The patient presented abnormal EEG with evidence of epileptic discharges. A temporal bone CT showed plagiocephaly with flattening of the right occipital bone. Brain MRI showed callosal agenesis with bilateral colpocephaly with temporal horn dilatation, parahippocampal atrophy, lissencephaly and midbrain hypoplasia. The combination of de novo gene variants mentioned above has never been reported nor correlated as the result of haploinsufficiency mechanisms. Thus, we propose haploinsufficiency and loss of heterozygosity as etiological reasons for this patient phenotype. Further proteomic studies are needed to allocate the extense of genetic influence within the clinical manifestations.We report a case of pleomorphic adenoma (PA) of the maxillary sinus, an unusual and rare location of this frequent benign neoplasm originating from salivary glands and accounting for 70% of all salivary gland tumors, presenting with compressive phenomena from the orbit and the challenges in its differential diagnosis and management. A 66-year-old male presented with progressively worsening exophthalmos over the past 4 months and symptoms of nasal obstruction on the left side for the past year. Clinical evaluation and imaging studies revealed complete occupation of the left maxillary sinus by a mass with soft tissue characteristics, extending to the homonymous nasal cavity and orbit. Functional endoscopic sinus surgery (FESS) was performed. The surgical plan included septoplasty and medial maxillectomy and complete resection of the tumor with clear margins. The histopathology report was consistent with a PA of the maxillary sinus. PA of the maxillary sinus is an extremely rare disease entity. Orbital involvement due to extension of an undiagnosed PA as described in this case is very uncommon. Thorough investigation is required in cases of unilateral nasal obstruction. Complete surgical excision and close long-term follow-up are required due to the probability of tumor recurrence and potential malignant transformation.Malignant solitary fibrous tumors (MSFT) are rare neoplasms, and typically exhibit an aggressive course. While complete surgical resection is the primary treatment modality, the role of adjuvant radiation treatment in larger tumors is not well-established. Despite limited reported cases which demonstrated extended disease-free periods with adjuvant radiation, its utilization is conflictingly both recommended or discouraged across the literature due to the absence of high-quality published data. This is a report to add to the slowly growing body of literature to support the use of adjuvant radiation in these tumors. Specifically, a case of a 64-year-old man who developed rash and mild back pain after a total hip arthroplasty. He was found to have a large paravertebral MSFT, and was treated with surgical resection followed by adjuvant radiation due to size and focally positive margins. He has continued to have no evidence of disease 21 months after treatment. This case of successful treatment and continued disease-free interval with resection and adjuvant radiation contributes valuable supporting data to the management of this rare disease entity. Furthermore, a review of available literature on MSFT treatment is conducted to illustrate the inconsistency in post-surgical management, and demonstrate the necessity of additional detailed reports from a radiation treatment perspective.
