Inorganic arsenic is a naturally occurring toxicant that poses a significant and persistent challenge to public health. The World Health Organization has identified many geographical regions where inorganic arsenic levels exceed safe limits in drinking water. Numerous epidemiological studies have associated exposure to inorganic arsenic with increased risk of adverse health outcomes. Randomized clinical trials have shown that nutritional supplementation can mitigate or reduce exacerbation of exposure-related effects. Although a growing body of evidence suggests that epigenetic status influences toxicity, the relationships among environmental exposure to arsenic, nutrition, and the epigenome are not well detailed. This review provides a comprehensive summary of findings from human, rodent, and in vitro studies highlighting these interactive relationships.
With increasing prevalence of cancer survivors in the United States, health-related quality of life (HRQOL) has become a major priority. We describe HRQOL in a nationally representative sample of cancer survivors and examine associations with key sociodemographic, clinical, and lifestyle characteristics.
Cancer survivors, defined as individuals ever diagnosed with cancer (N = 877), were identified from the 2016 Medical Expenditure Panel Survey-Experiences with Cancer Survivorship Supplement, a nationally representative survey. Physical and mental health domains of HRQOL were measured by the Global Physical Health (GPH) and Global Mental Health (GMH) subscales of the Patient-Reported Outcomes Measurement Information System Global-10. Multivariable linear regression was used to examine associations of sociodemographic, clinical, and lifestyle factors with GPH and GMH scores. All statistical tests were 2-sided.
Cancer survivors' mean GPH (49.28, SD = 8.79) and mean GMH (51.67, SD = 8.38) were similar to geactors for worse HRQOL. Multimorbidity management and healthy behavior promotion may play a key role in maximizing HRQOL for cancer survivors.Teaching Point The imaging clues to differentiate idiopathic tumoral calcinosis from other calcified soft tissue lesions include pseudotumoral appearance with mass effect, bone erosion with intra-osseous protrusion of calcification, lobulated morphology, and peri-articular location on the extensor side.Event memories consist of associations between their constituent elements, leading to their holistic retrieval via the process of pattern completion. This holistic retrieval can occur, under specific conditions, when each within-event association is encoded in a separate temporal context adults are able to integrate the information into a single coherent representation. In this study, we sought to replicate the holistic retrieval of simultaneously encoded event elements in children, and examine whether children can similarly integrate across separated encoding contexts. Children (aged 6-7 years; 9-10 years) and adults encoded two series of three-element "events" consisting of an animal, object, and location. In the simultaneous condition, they encountered all three event elements at once; in the separated condition, they encountered each pairwise association separately (animal-object, animal-location, object-location). After encoding, they were tested on the retrieval of each within-event association using a 4-alternative-forced-choice task. We inferred the presence of holistic retrieval using a measure of retrieval dependency-the statistical dependency between retrieval of within-event associations. Memory for the pairs improved across ages, but there were no developmental differences in retrieval dependency. In the simultaneous encoding condition, all three age groups showed retrieval dependency. However, counter to previous studies, retrieval dependency was not observed in any age group following separated encoding. The results from the simultaneous encoding condition support the idea that pattern completion processes are developed by early childhood. The absence of retrieval dependency in adults following separated encoding prevent conclusions regarding the developmental trajectory of mnemonic integration.
Antegradely conducting left lateral accessory pathways are a risk for supraventricular tachycardias and pre-excited atrial fibrillation. Rarely, an anomalous coronary sinus can cause difficulty in locating the pathway. The left circumflex coronary artery and obtuse marginal branches supply the posterolateral left ventricle. We describe a case report of a high-risk accessory pathway associated with an anomalous coronary sinus which, between successive electrophysiology studies, was obliterated by a felicitous acute coronary syndrome in the left circumflex territory.
A 49-year-old male with palpitations and manifest pre-excitation was referred for electrophysiology study. Initial study revealed a high-risk left lateral accessory pathway with antegrade effective refractory period of 240 ms and rapidly conducting pre-excited atrial fibrillation. find more The coronary sinus could not be cannulated to localize the pathway. Coronary angiography and cardiac computed tomography showed an anomalous coronary sinus emptying i.As the coronavirus disease 2019 (COVID-19) pandemic, which is caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), is spreading rapidly worldwide, it has emerged as a leading cause of mortality, resulting in >1 million deaths over the past 10 months. The pathophysiology of COVID-19 remains unclear, posing a great challenge to the medical management of patients. Recent studies have reported an unusually high prevalence of thromboembolic events in COVID-19 patients, although the mechanism remains elusive. Several studies have reported the presence of aPLs in COVID-19 patients. We have noticed similarities between COVID-19 and APS, which is an autoimmune prothrombotic disease that is often associated with an infective aetiology. Molecular mimicry and endothelial dysfunction could plausibly explain the mechanism of thrombogenesis in acquired APS. In this review, we discuss the clinicopathological similarities between COVID-19 and APS, and the potential role of therapeutic targets based on the anti-phospholipid model for COVID-19 disease.
