To evaluate the prevalence of AMD among optometric telemedicine users in Spain and to identify risk factors.

Retrospective analysis of a nationwide database conducted on subjects attending to optometry centers, between January 2013 and December 2019. Fundus photographs were performed by optometrists, using non-mydriatic cameras, and evaluated by a group of 12 retina specialists.

Among the 119,877 subjects included, the overall prevalence of AMD was 7.6%. The prevalence of early, intermediate, and advanced AMD was 2.9%, 2.7%, and 2.0%, respectively. Of the 9129 AMD subjects, 1161 (12.7%) had geographic atrophy, and 1089 (11.9%) had neovascular AMD, either scar (4.5%) or exudative (7.4%). There was a significant association between AMD and age (per year older, adjusted odds ratio, OR 1.116; 95% CI 1.114 to 1.119, p<0.0001). Women had higher prevalence (adjusted OR 1.17; 95% CI 1.12 to 1.23, p<0.0001). Every diopter (spherical equivalent) of progress toward hyperopia was associated with a significant increase in early AMD prevalence (adjusted OR 1.02, 95 CI 1.01 to 1.04, p=0.0074). 6-Diazo-5-oxo-L-nor-Leucine Presence of diabetes was associated with a lower AMD prevalence (p<0.0001).

The prevalence of AMD (any eye and any severity) was 7.6%, with a prevalence of advanced AMD of 2.0%. Older age and women were significantly associated with a higher prevalence of AMD, whereas myopia and presence of diabetes were associated with significantly lower odds of any AMD.

We assessed the ability of deep learning (DL) models to distinguish between tear meniscus of lacrimal duct obstruction (LDO) patients and normal subjects using anterior segment optical coherence tomography (ASOCT) images.

The study included 117 ASOCT images (19 men and 98 women; mean age, 66.6 ± 13.6 years) from 101 LDO patients and 113 ASOCT images (29 men and 84 women; mean age, 38.3 ± 19.9 years) from 71 normal subjects. We trained to construct 9 single and 502 ensemble DL models with 9 different network structures, and calculated the area under the curve (AUC), sensitivity, and specificity to compare the distinguishing abilities of these single and ensemble DL models.

For the highest single DL model (DenseNet169), the AUC, sensitivity, and specificity for distinguishing LDO were 0.778, 64.6%, and 72.1%, respectively. For the highest ensemble DL model (VGG16, ResNet50, DenseNet121, DenseNet169, InceptionResNetV2, InceptionV3, and Xception), the AUC, sensitivity, and specificity for distinguishing LDO were 0.824, 84.8%, and 58.8%, respectively. The heat maps indicated that these DL models placed their focus on the tear meniscus region of the ASOCT images.

The combination of DL and ASOCT images could distinguish between tear meniscus of LDO patients and normal subjects with a high level of accuracy. These results suggest that DL might be useful for automatic screening of patients for LDO.

average time to re-treatment and change in visual field mean deviation (MD) over the follow-up period.

Baseline IOP (±SD) was 22.2 (± 4.9). link2 link2 Baseline MD (±SD) of standard automated perimetry was - 1.28 (± 2.36). Decrease in IOP was 6.5 (±3.6) mmHg at 1 year (n=108), 5.2 (±4.6) mmHg at 5 years (n=84) and 3.8 (±2.7) mmHg at 10 years (n=18). 6-Diazo-5-oxo-L-nor-Leucine Treatment success rate 98% at year 1, 89% at year 5 and 72% at year 10. Here, a sandwich-type electrochemical immunosensor for ultrasensitive detection of CYFRA21-1 is developed. The sensor based on a combination of gold nanoparticle (AuNPs) decorated Ti3C2Tx-MXene (Au-Ti3C2Tx) as the substrate enhancer, and toluidine blue (TB) modified AuNPs doped covalent organic framework (COF) polymer as the signal tag (TB-Au-COF). The Au-Ti3C2Tx is used to capture numerous primary antibodies and accelerate the electron transfer rate of the substrate, while the TB-Au-COF can be applied to provide a large number of signal units TB and secondary antibodies. These features of composites endow the proposed immunosensor with high sensitivity and current response to CYFRA21-1. Under optimum conditions, the immunosensor offers a wide current response for CYFRA21-1 from 0.5-1.0 × 104 pg·mL-1 with a detection limit of 0.1 pg·mL-1. Furthermore, the biosensing platform can be applied for CYFRA21-1 detection to analyze real serum samples, providing an effective and useful avenue for the applicability of Au-Ti3C2Tx and TB-Au-COF composite materials in biosensing field. The hypereosinophilic syndrome (HES) is a rare disorder characterized by hypereosinophilia and infiltration of various organs with eosinophils. Eosinophilic cystitis (EC), mimicking bladder cancer clinically but also in ultrasound and in radiographic imaging, is one potential manifestation of the HES occurring in adults as well as in children. This case report describes the course of disease in a 57-year-old male presenting with severe gait disorders and symptoms of a low compliance bladder caused by a large retropubic tumor. After extensive urine and serologic examination and histologic confirmation of EC the patient was subjected to medical treatment with cetirizine and prednisolone for 5 weeks. While gait disorders rapidly resolved, micturition normalized only 10 months after initiation of therapy. Based upon this course the authors recommend patience and reluctance concerning radical surgical intervention in EC. Key Points • Eosinophilic cystitis is a rare condition with app. 200 cases reported, so far. • Etiology of eosinophilic cystitis is obscure, but allergies and parasitic infections may trigger the disease. • Genetic alterations (e.g., BRAF mutations) may predispose for the disease • Corticosteroids and antihistamines are the backbone of therapy and may be complemented by antibiotics and non-steroidal anti-inflammatory drugs in case of concomitant (underlying) infections. • As recovery can occur even after a long time, radical surgery should be restricted to highly selected cases. We quantify plastic litter (PL, > 2 cm) and microplastics (MP,  less then  5 mm) from the sediments of a beach formed at a riverine depositional area, at the upper Amazon River basin, Ecuador. In the collection area (4400 m2), the PL density was 0.045 items m-2, where low-density polyethylene bags were the prevalent PL. The beach was classified as "very clean" (Clean Coast Index (CCI) of 1.3 items m-2). Regarding MP, in 55 sampling stations, average MP concentrations ranged from 0 to 2200 items kg-1 of dry sediment (0.5-2 mm), and 0-4200 items kg-1 of dry sediment (2-5 mm). Blue fibers were the prevalent MP. Our results represent the first report to show the ubiquitous presence of PL and MP for the area. The monitoring and management of plastic disposal in freshwater beaches are necessary, as here we report a small part of an undocumented issue.Reef-building corals are inhabited by functionally diverse microorganisms which play important roles in coral health and persistence in the Anthropocene. However, our understanding of the complex associations within coral holobionts is largely limited, particularly transgenerational exposure to environmental stress, like ocean acidification. Here we investigated the microbiome development of an ecologically important coral Pocillopora damicornis following transgenerational exposure to moderate and high pCO2 (partial pressure of CO2) levels, using amplicon sequencing and analysis. Our results showed that the Symbiodiniaceae community structures in adult and juvenile had similar patterns, all of which were dominated by Durusdinium spp., previously known as clade D. Conversely, prokaryotic communities varied between adults and juveniles, possibly driven by the effect of host development. Surprisingly, there were no significant changes in both Symbiodiniaceae and prokaryotic communities with different pCO2 treatments, which was independent of the life history stage. This study shows that ocean acidification has no significant effect on P. damicornis microbiome, and warrants further research to test whether transgenerational acclimation exists in coral holobiont to projected future climate change.Resting-state functional connectivity (RSFC) has widely been used to examine reorganization of functional brain networks during normal aging. link3 The extraction of generalizable age trends, however, is hampered by differences in methodological approaches, study designs and sample characteristics. Distinct age ranges of study samples thereby represent an important aspect between studies especially due to the increase in inter-individual variability over the lifespan. The current review focuses on comparing age-related differences in RSFC in the course of the whole adult lifespan versus later decades of life. link2 We summarize and compare studies assessing age-related differences in within- and between-network RSFC of major resting-state brain networks. Differential effects of the factor age on resting-state networks can be identified when comparing studies focusing on younger versus older adults with studies investigating effects within the older adult population. These differential effects pertain to higher order and primary processing resting-state networks to a varying extent. Especially during later decades of life, other factors beyond age might come into play to understand the high inter-individual variability in RSFC.Although mind wandering during reading is known to be affected by text difficulty, the nature of this relationship is not yet fully understood. To examine this issue, we conducted an experiment in which participants read non-fiction texts that varied along five levels of difficulty under naturalistic conditions. Difficulty levels were determined based on Flesch-Kincaid Grade Levels and verified with Coh-Metrix indices. Mind wandering was measured with thought probes. We predicted that text difficulty and mind wandering have a U-shaped (i.e., quadratic) relationship. link3 link3 Contrary to our expectations, but in line with some prior studies, mind wandering linearly increased with text difficulty. Additionally, text interest moderated the effect of text difficulty on mind wandering. Finally, mind wandering was associated with worse performance on a comprehension test. Together, our findings extend previous work by showing that (a) a linear relationship between difficulty and mind wandering exists during common page-by-page reading of pre-existing texts and that (b) this relationship holds across a broad range of difficulty levels.
