ntive implications.Primary ciliary dyskinesia (PCD) causes chronic infections and progressive bronchiectasis that can lead to severe lung disease. Because there are no cures or regenerative therapy options for PCD, treatment of severe lung disease in PCD is focused on managing symptoms, including aggressive administration of antibiotics and diligent airway clearance. The Genetic Disorders of Mucociliary Clearance Consortium (GDMCC) does not recommend routine lobectomy, reserving its use for "rare cases of PCD with severe, localized bronchiectasis" and warns that a lobectomy should be treated with caution. However, if aggressive medical management fails, selective surgical removal of severely defective lung may result in maintenance or improvement of pulmonary function. Certainly, the decision to recommend lung resection in the face of chronic bronchiectasis from PCD requires an extensive discussion before it is considered as an alternative treatment. The purpose of this manuscript is to demonstrate that in selected cases of unilobar disease with bronchiectasis that are not responsive to other therapies (antibiotics and airway clearance), removal of localized necrotic areas of the lung along with prophylactic antibiotics can improve the quality of life of children with PCD associated bronchiectasis and improve growth and nutritional status, and pulmonary function.
To describe factors related to the infant, mother, and breastmilk composition that may be associated with excessive weight gain in a cohort of exclusively breastfed infants younger than 6 months of life with excessive weight gain, and to compare these findings with data from a group of normal-weight exclusively breastfed infants.
36 exclusively breastfed infants younger than 6 months of life seen at two health-care centers between July 2016 and 2017 were enrolled in the study. The clinical features of the infants, their mothers, and the macronutrient composition of the breast milk were evaluated. We classified infants according to weight gain velocity between birth and 6 months of life into an excessive weight gain (EWG) and an adequate weight gain (AWG) group.
Mean age at protocol entry was 3.8 months. Thirteen patients were classified as EWG and 23 patients as AWG. Cosleeping was more often observed in EWG than in AWG infants. Mothers in the EWG group were younger and more often had gained more than 18 kg during pregnancy than those in the AWG group. No significant differences were found in the macronutrient content of the breast milk between both groups.

On the territory of Ukraine, the earliest reports of cases of WNV circulation in humans and birds relate to the 70s of the XX century. In Poltava region WNF was first registered in 2011. Though the epidemiological and clinical patterns of WNF in Ukraine and Poltava region remain understudied, primarily due to the lack of alertness of practitioners to the problem of WNF and to broader access to laboratory diagnosis of the disease. The first clinical case of West Nile Viral encephalitis, registered on the territory of Poltava region, has been given and analyzed.
Heterotopic pregnancy is the simultaneous occurrence of intrauterine and ectopic pregnancy. This situation is very rare (130 000 pregnancies), while recently, with the development of assisted reproductive techniques, the incidence has increased to 1100 - 1500 pregnancies. The aim of the study is to present the situation of coexistence of intrauterine pregnancy and ruptured tubal pregnancy.
The case concerns a 32-year-old patient in the 12th week of the second pregnancy in whom the only risk factor was the state after Caesarean section and thus possible intraperitoneal adhesions. The ultrasound revealed normal intrauterine pregnancy and a very large amount of free fluid in the smaller pelvis. After immediate surgical intervention, a ruptured right tubal pregnancy was found. Right fallopian tube was removed. After the operation, the patient with the preserved intrauterine pregnancy was discharged from the ward. Further intrauterine pregnancy was normal. Delivery by Caesarean section.
Conclusions The described case indicates that the existence of intrauterine pregnancy does not exclude the existence of ectopic pregnancy and emphasizes the great importance of correctly and accurately carried out ultrasound examination in the first trimester of pregnancy along with appendicitis assessment. Early diagnosis of heterotopic pregnancy reduces the risk of complications.

Most fatalities occur in elderly people with comorbidities. However, SARS-CoV-2 pneumonias do also occur in younger patients with no comorbidities or risk factors at all. We report here on one of the "early" cases that occurred in Germany. A 57-year-old man was infected and developed pneumonia after a skiing vacation in Northern Italy. Other members of the travel group also fell ill, but only showed flu-like symptoms. Only a few if any infections originated from the affected person; at this point in time the infection situation in the region could still be grasped. Under supportive measures, the disease developed positively despite impressive radiological findings. The positive course is likely also due to the age of the person affected and the lack of any risk factors. The purpose of the article is to present legal norms regarding the physician's duties in the field of preventing and combating infectious diseases. In addition, doctors closer knowledge of new legal regulations related to the SARS-coronavirus CoV-2.The analysis shows that doctors play an important role in implementing legal instruments for the prevention of infectious diseases. The aim To study health status among the children in connection with a prolonged exposure of drinking water from decentralized systems.
Materials and methods The prevalence rates of diseases among children of rural areas were studied according to the statistical reports in Dnipropetrovsk region healthinformation center from 2008 to 2013 using statistical form № 12 (totally 220 reports). The distribution of rural areas in the region was based on the cluster analysis. The horizontal dendrogram showing similarity between the research areas in a multidimensional space. Cluster analysis includes 2 groups of rural districts in Dnipropetrovsk region.
Results It is established that with increase of pH#62; 7.5-7.6 in water of decentralized systems decreased the prevalence in the majority of nosologies diseases of the blood and hematopoietic organs (rS=0,331; p#60;0,001), musculoskeletal system (rS=-0,174; p#60;0,001), anemia (rS=-0,331; p#60;0,001), congenital anomalies (rS=-0,272; p=0,002) among children population.
Conclusions Among those children, who consumed drinking water from wells with content of some essential trace elements on the limits of MPC 71% did not suffer from chronic diseases of the circulatory system (Fe >0.144 mg/dm3); 59% did not suffer from the diseases of digestive system (Cu #62;0.089 mg/dm3), and 60% had no congenital abnormalities of circulatory system (Mn #62; 0.099 mg/dm3).

The aim To investigate the features of cytokine profile of intraocular fluid in patients undergoing cataract phacoemulsification and the effect of their level on the nature and frequency of early and late postoperative complications from the retina.
Materials and methods The work involved 82 patients (82 eyes) with cataract during traditional phacoemulsification on the basis of Ophthalmology department of Poltava Regional Clinical Hospital. The intraocular fluid for the study, while performing phacoemulsification of the nucleus, was collected with a single insulin syringe through the paracentesis of the cornea in the volume of 0.3-0.4 ml for the examination of TNF-α and bFGF levels.
Results It was found that the level of TNF-α intraocular fluid ranged from 1.11 pg / ml to 140.55 pg / ml; its average was 24.61 ± 5.43 pg / ml. When examining the level of bFGF in intraocular fluid, it ranged from 1.0 pg / ml to 12.54 pg / ml; its average was 3.33 ± 0.44 pg / ml.
Conclusions Comparing the levels of the effect of TNF-α and bFGF of the intraocular fluid of patients with cataracts and morphological changes of the retina after phacoemulsification revealed a direct dependence of the level of these factors on the development of macular edema.
