Numerous patients receive acute migraine care in the Emergency Department (ED) setting. A shift of this care to the outpatient Neurology Clinic and outpatient Infusion Center setting has the potential to optimize clinical management while decreasing resource utilization.

Clinicians and administrators collaborated on the operationalization of an Acute Headache Infusion Clinic run through the outpatient Neurology Clinic. Data was collected on all patients treated in the Acute Headache Infusion Clinic from 9/2018-12/2019. Duration of the outpatient visit, cost per visit, and pre- and post-treatment pain scores were collected. Comparison was made to similar care administered at our institution's Emergency Department.

Despite great progress in radiological diagnostic tools for neurodegenerative disorders, their diagnostic accuracy has been unsatisfactory. One of the pathological hallmarks of progressive supranuclear palsy (PSP) is atrophy of the subthalamic nucleus, which has not attracted much attention for imaging analysis.

The clinical data of patients with PSP, multiple system atrophy (MSA), Parkinson's disease (PD), and corticobasal syndrome (CBS) who underwent brain magnetic resonance imaging at our department between June 2019 and March 2020 were retrospectively reviewed. The volumes of the subthalamic nucleus and of the whole cerebrum were then analyzed and compared among the disorders.

Fourteen PSP-Richardson syndrome (RS), 14 MSA, 14 PD, and 8 CBS patients were assessed. The mean volume of the bilateral subthalamic nuclei was smaller in PSP patients (0.148±0.012cm
) than in MSA (0.183±0.026cm
; p<0.001), PD (0.209±0.031cm
; p<0.001), and CBS (0.180±0.056cm
; p<0.001) patients. The volume of the whole cerebrum was not significantly different among the disorders. Using an STN volume cut-off of 0.01925, the sensitivity and specificity for differential diagnosis between PSP and the other disorders were 0.846 and 0.972, respectively.

Central nervous system (CNS) infections can be caused by a variety of viruses, but in a significant number of patients no viral or other pathogen can be identified using routine diagnostic work-up. Interestingly, several case reports and series described Hepatitis E virus (HEV) as a potential pathogen. However, systematic studies have not been conducted so far.

We identified 243 patients from Southwestern Germany with acute CNS infections of unknown cause treated in our clinic between 2008 and 2018, of which serum and/or cerebrospinal fluid (CSF) samples were available. These patients were retrospectively tested for anti-HEV IgM and IgG antibodies. In addition, HEV PCR was performed in the majority of cases including IgM-negative patients with symptom onset <8days. 263 healthy individuals served as controls.

Evidence of an acute HEV infection was found in four patients (1.7%). Three had recent HEV infection defined as positive anti-HEV IgM and IgG antibodies, one had current HEV infection defined as (additional) detection of HEV RNA in serum. However, anti-HEV IgM and IgG seroprevalence did not differ significantly from controls, though these had considerably lower IgM levels. Interestingly, anti-HEV IgG seroprevalence was unexpectedly high (30.7%) and revealed an age-dependent increase to more than 50% in patients older than 60years.

This study supports previous findings that HEV could play a role in acute CNS infections. Therefore, we encourage testing for acute HEV infection if no other pathogen can be identified. However, further studies are necessary to prove a causal role.

However, there is no report about circRNAs in the gonads of model fish medaka (Oryzias latipes). In this study, 1157 and 1570 circRNAs were obtained in the ovary and testis by RNA-sequencing. The characteristics of circRNAs were explored in sequence length, exon composition, and chromosome position. 24 circRNAs were significantly up or down-regulated in the testis compared to the ovary, 9 of which were verified by qRT-PCR. Interestingly, circ452 was highly expressed in the testis while circ880 expression exhibited sexual dimorphism. In situ hybridization (ISH) revealed that circ452 and circ880 were expressed in meiotic germ cells, and circ880 was also abundant in spermatogonia. In addition, dual-luciferase reporter assay manifested that circ880 and Oldnd can combine with miR-375-3p.
