Using a combined quantitative and qualitative approach, we explored COVID-19 related anxiety in paediatric patients with severe obesity in the Netherlands using semi-structured telephone interviews and the Paediatric Quality of Life Inventory (PedsQL) questionnaire, which had also been completed by the study population at baseline in the year prior to the COVID-19 outbreak. In total, 75 families participated in the semi-structured telephone interviews during the lockdown, April 2020. Characteristics of included patients were median age 10.5 years (interquartile range = 7.6-15.2); 52% female; mean BMI standard deviation score 3.8 (SD = 1.0). COVID-19 related anxiety was reported for 24/75 (32%) children. The mean decrease in PedsQL score between baseline visit and COVID-19 outbreak did not differ between children for whom anxiety was reported vs those for whom it was not (mean change -10.3 ± 36.5 vs -3.3 ± 24.4, P = .54). Self-imposed strict quarantine measures were taken by 19/75 (25%) families. During follow-up, several families reported that the previous contact alleviated their anxiety. In conclusion, healthcare professionals should address possible COVID-19 related anxiety in children with severe obesity. Addressing COVID-19 related anxiety could mitigate its potential negative effects.A large-scale study was carried out to determine the prevalence of antibodies against Pestivirus species in wild ruminants and describe their spatial variation in mainland Spain. Serum samples of 1,874 wild ruminants from different regions of this country were collected between the years 2000 and 2017. A total of 6.6% (123/1,874) animals showed antibodies against Pestivirus by both blocking ELISA (bELISA) and virus neutralization tests (VNT). The prevalence of antibodies against pestiviruses was different both among species and regions. Seroprevalence by species was 30.0% (75/250) in Southern chamois (Rupicapra pyrenaica), 7.0% (25/357) in fallow deer (Dama dama), 2.5% (10/401) in red deer (Cervus elaphus), 2.4% (8/330) in Iberian wild goat (Capra pyrenaica), 1.1% (4/369) in roe deer (Capreolus capreolus) and 0.8% (1/130) in mouflon (Ovis aries musimon), not detecting seropositivity (0/37) in Barbary sheep (Ammotragus lervia). The results confirm that exposure to pestiviruses was detected throughout mainland Spain, with significantly higher seroprevalence in Northern regions associated with the presence of Southern chamois. This indicates an endemic circulation of pestiviruses in Southern chamois and a limited circulation of these viruses in the remaining wild ruminant species during the last two decades, thus suggesting that non-chamois species are not true Pestivirus reservoirs in Spain. Nonetheless, the high spatial spread of these viruses points out that new epidemic outbreaks in naïve wild ruminant populations or transmission to livestock may occur, evidencing the usefulness of monitoring pestiviruses in wild ruminants, especially at the wildlife-livestock interface.Complexity in MLC-based radiosurgery treatment delivery can be characterized by the efficiency of monitor unit (MU) utilization and the average MLC leaf separation distance for a treatment plan. A reduction in plan complexity may be desirable if plan quality is not impacted. In this study, a number of strategies are explored to determine how plan quality is affected by efforts to reduce plan complexity. Ten radiosurgery cases of varying complexity are retrospectively planned using six optimization strategies an unconstrained volumetric modulated arc therapy (VMAT) technique, a MU-constrained VMAT technique, three techniques using various strengths of the aperture shape controller (ASC), and a hybrid technique consisting of a final-stage VMAT optimization applied to a dynamic conformal arc leaf sequence (ODCA). The plans are compared in terms of MU efficiency, MLC leaf-separation, conformity index (CI), gradient index (GI), and QA measurement results. The five VMAT techniques exhibited only minor differences in CI and GI values, though the ASC and MU-constrained techniques did require 6-20% fewer MU and had mean field apertures 5-19% larger. On average, the ODCA technique had CI values 3.5% lower and GI values 1.0-2.5% higher than the VMAT techniques, but also had a mean field aperture 24-47% larger and required 16-32% fewer MU. The QA measurement results showed a 0.61% variation in mean per-field 2%/1 mm gamma passing rates across all techniques (range 96.81%-97.42%), with no observed correlation between passing rate and technique. For simple targets, the ODCA technique achieved CI results that were equivalent to the unconstrained VMAT technique with an average 30% reduction in required MU, an average 50% increase in mean leaf separation distance, and brain V12Gy values within 0.38 cc of the VMAT technique for targets up to approximately 2 cm diameter. For MLC-based single-target radiosurgery, plan complexity can often be significantly reduced without an equivalent reduction in plan quality.
The current results of in vitro reproduction techniques in pigs, such as in vitro fertilization (IVF) and embryo development, show high performance with both epididymal and ejaculated spermatozoa. It has been reported that exposure of spermatozoa to reproductive fluids, such as SP or periovulatory oviductal fluid (pOF), modulates sperm functionality both in vivo and in vitro. But whether or not this modulating effect of pOF depends on the origin of the spermatozoa being epididymal or ejaculated, is still unknown.

The effects of incubating spermatozoa from the epididymis and ejaculate with pOF in capacitating conditions were investigated by analyzing sperm motility, phosphorylation of protein kinase A substrates and proteins in tyrosine (pPKAs and pTyr, respectively), the interaction of the spermatozoa with the oocyte in IVF and intracytoplasmic sperm injection (ICSI), and, finally, the spermatozoa chromatin condensation status.

The pOF modified events related to capacitation in epididymal spermatozoa by decreasing motility, pPKAs and pTyr. In the interaction with the oocyte after sperm capacitation, pOF regulated the epididymal and ejaculated spermatozoa differently. While pOF decreased the number of spermatozoa bound to the zona pellucida (Spz/ZP) and increased oocyte activation after ICSI with epididymal spermatozoa, with the ejaculated spermatozoa, it decreased the mean number penetrating each oocyte (Spz/O). Additionally, pOF significantly increased the nuclear decondensation of the epididymal spermatozoa after the fertilization of the oocyte.

Evidence syntheses provide the basis for evidence-based decision making in healthcare. To judge the certainty of findings for the specific decision context evidence syntheses should consider context suitability (ie, generalizability, external validity, applicability or transferability). Our objective was to determine the status quo and to provide a comprehensive overview of existing methodological recommendations of Health Technology Assessment (HTA) and Systematic Review (SR) producing organizations in assessing context suitability of evidence on effectiveness of health care interventions. Additionally, we analyzed similarities and differences between the recommendations.

In this Integrative Review we performed a structured search for methods documents from evidence synthesis producing organizations that include recommendations on appraising context suitability in effectiveness assessments. Two reviewers independently selected documents according to predefined eligibility criteria. Data were extracted in standardized and piloted tables by one reviewer and verified by a second reviewer. We performed a thematic analysis to identify and summarize the main themes and categories regarding recommended context suitability assessments.

We included 14 methods documents of 12 organizations in our synthesis. Assessment approaches are very heterogeneous both regarding the general concepts (eg, integration in the evidence synthesis preparation process) and the content of assessments (eg, assessment criteria).

Relationships between brain atrophy patterns of typical aging and Alzheimer's disease (AD), white matter disease, cognition, and AD neuropathology were investigated via machine learning in a large harmonized magnetic resonance imaging database (11 studies; 10,216 subjects).

High WMH volume was associated with brain aging and cognitive decline occurring in an ≈10-year period in CN subjects. WMHs were associated with doubling the likelihood of amyloid beta (Aβ) positivity after age 65. Brain aging, AD-like atrophy, and WMHs were better predictors of cognition than chronological age in MCI/AD.

These "hydrofoam" reactions increased CFPS yields by up to 100%. Contrary to traditional protein synthesis using living organisms, where foam bubbles cause cell-lysis and production losses, hydrofoam CFPS reactions are "cell-free" and better tolerate foaming.
