We developed a machine learning algorithm, based on a deep Reinforcement Learning (RL), for continuous management of oxygen flow rate for critical ill patients under intensive care, which can identify the optimal personalized oxygen flow rate with strong potentials to reduce mortality rate relative to the current clinical practice. Basically, we modeled the oxygen flow trajectory of COVID-19 patients and their health outcomes as a Markov decision process. Based on individual patient characteristics and health status, a reinforcement learning based oxygen control policy is learned and real-time recommends the oxygen flow rate to reduce the mortality rate. We assessed the performance of proposed methods through cross validation by using a retrospective cohort of 1,372 critically ill patients with COVID-19 from New York University Langone Health ambulatory care with electronic health records from April 2020 to January 2021. The mean mortality rate under the RL algorithm is lower than standard of care by 2.57% (95% CI 2.08- 3.06) reduction (P less then 0.001) from 7.94% under the standard of care to 5.37 % under our algorithm and the averaged recommended oxygen flow rate is 1.28 L/min (95% CI 1.14-1.42) lower than the rate actually delivered to patients. Thus, the RL algorithm could potentially lead to better intensive care treatment that can reduce mortality rate, while saving the oxygen scarce resources. It can reduce the oxygen shortage issue and improve public health during the COVID-19 pandemic.
The coronavirus disease (COVID-19) morbidity is rising in Uganda. However, data are limited about people's knowledge, attitudes, and practices.

To determine knowledge about COVID-19, attitudes towards presidential directives and Ministry of Health (MoH) guidelines, and adherence to practicing public health preventive measures (KAP) in Uganda.

This cross-sectional survey was conducted between April 28 and May 19, 2020. Data were collected using online social media platforms, websites, and popular media outlets. We descriptively summarized data and categorized KAP scores as knowledgeable about COVID-19, positive attitude towards presidential directives and MoH guidelines, and adherent to public health preventive measures, respectively. We tested sex differences in KAP using tests of significance and established independently associated factors using modified Poisson regression analysis, reported using adjusted prevalence risk ratio (aPR) with 95% confidence interval (CI).

We conducted an exploratory, randomised, double-blind, placebo-controlled trial of hospitalised subjects with severe COVID-19 (pulmonary infiltrates and oxygen saturation ≤94% on room air) conducted at four tertiary care centres in India. Patients were randomised 11 to receive either 0.3 mL·day

intradermally or a matching placebo for three consecutive days. The primary outcome of the study was the distribution of clinical status assessed on a seven-point ordinal scale ranging from discharged (category 1) to death (category 7) on study days 14, 21, and 28. The co-primary outcome was a change in SOFA (sequential organ failure assessment) score on days 7 and 14 compared to the baseline. HO-3867 manufacturer In this study, 450 MTurk workers completed measures of sociodemographic characteristics, perceived risk for COVID-19, general perceived vulnerability to disease, intolerance of uncertainty, and psychological flexibility. These variables were used to predict COVID-19 preventive health behaviors (PPE use), psychological distress, and physical symptoms. The surveys were completed between April 9, 2020 and April 18, 2020 which is a period that corresponded to the first 2-3 weeks of lockdown for most participants. A demographically diverse sample of participants was recruited. A substantial number of participants reported a reduction employment status and 69% were in self-isolation. Participants reported a high degree of perceived vulnerability to COVID-19. The relationship between intolerance of uncertainty and psychological distress/physical symptoms was stronger among participants with lower levels of psychological flexibility. These findings indicate psychological flexibility can reduce distress associated with COVID-19. To further understand the disease, we compared its clinical characteristics, symptoms and outcomes by gender. In an analysis of public surveillance data of Taiwan from January 21 to April 18, 2020, a total of 398 patients were diagnosed with COVID-19 by the detection of severe acute respiratory syndrome coronavirus 2 in pharynx swabs. We divided the patients into two groups men and women. The associated data were collected, and multivariate comparisons of radiographic infiltration were conducted to analyse the gender-based differences. The mean incubation period was 5.4 ± 5 days, and the incubation period in men was 3.2 days longer than that in women (8 ± 8.1 vs. 4.8 ± 3, p = 0.05). The male patients with COVID-19 with infiltration in chest X-rays (CXR) were 12 years older than their female counterparts. The mortality rate in the male patients with COVID-19 was 6.4-fold higher than that in the female patients (3.2% vs. 0.5%, p less then 0.05). The patients with comorbidities of diabetes mellitus and hypertension were vulnerable to infiltration in CXR and the patients with COVID-19 who had infiltration in CXR easily ended up with intubation, intensive care unit admission and mortality. Moreover, female patients with COVID-19 who had fever, cough and dyspnoea were susceptible to infiltration in CXR. This approach begins by performing a morphological reconstruction operation that is useful to remove the effect of the external disturbances on the infected regions and to locate different regions of interest precisely. The optimal size of the structuring element is selected using the Edge Content-based contrast matrix approach. After performing the opening by using the morphological reconstruction operation, further noise is eliminated using the closing-based morphological reconstruction operation. The original pixel space is restored and the obtained image is divided into some non-overlapping smaller blocks and the mean intensity value for each block is computed that is used as the local threshold value for the binarization purpose. It is preferable to manually determine the range of the infected region. If a region is greater than the upper bound then that region will be considered as an exceptional region and processed separately. Three standard metrics MSE, PSNR, and SSIM are used to quantify the outcomes. Both quantitative and qualitative comparisons prove the efficiency and real-life adaptability of this approach. The proposed approach is evaluated with the help of 400 different images and on average, the proposed approach achieves MSE 307.1888625, PSNR 23.7246505, and SSIM 0.831718459. Moreover, the comparative study shows that the proposed approach outperforms some of the standard methods and obtained results are encouraging to support the battle against the COVID-19.
