The pandemic has affected all aspects of society, dramatically changing our day-to-day lives and habits. It has also changed clinical practice, including practices of clinical laboratories. After one year, it is time to rethink what has happened, and is still happening, in order to learn lessons for the future of laboratory medicine and its professionals. While examining this issue, I was inspired by Italo Calvino's famous work, "Six memos for the next millennium".But I rearranged the Author's six memos into "Visibility, quickness, exactitude, multiplicity, lightness, consistency".Blood pressure is a reliable indicator of many cardiac arrhythmias and rheological problems. This study proposes a clinical set-up using conventional monitoring systems to estimate systolic and diastolic blood pressures continuously based on two photoplethysmogram signals (PPG) taken from the earlobe and toe. Several amendments were applied to conventional clinical monitoring devices to construct our project plan. We used two monitors to acquire two PPGs, one ECG, and invasive blood pressure as the reference to evaluate the estimation accuracy. One of the most critical requirements was the synchronization of the acquired signals that was accomplished by using ECG as the time reference. Following data acquisition and preparation procedures, the performance of each PPG signal alone and together was investigated using deep convolutional neural networks. The proposed architecture was evaluated on 32 records acquired from 14 patients after cardiovascular surgery. The results showed a better performance for toe PPG in comparison with earlobe PPG. Moreover, they indicated the algorithm accuracy improves if both signals are applied together to the network. According to the British Hypertension Society standards, the results achieved grade A for both blood pressure measurements. The mean and standard deviation of estimation errors were +0.3 ± 4.9 and +0.1 ± 3.2 mmHg for systolic and diastolic BPs, respectively. Since the method is based on conventional monitoring equipment and provides a high estimation consistency, it can be considered as a possible alternative for inconvenient invasive BP monitoring in clinical environments.Lambertianic acid (LA) is a diterpene bioactive compound mainly purified from different species of Pinus. It is an optical isomer of another natural compound daniellic acid and was firstly purified from Pinus lambertiana. LA can be synthesized in laboratory from podocarpic acid. It has been reported to have potential health benefits in attenuating obesity, allergies and different cancers including breast, liver, lung and prostate cancer. It exhibits anticancer properties through inhibiting cancer cell proliferation and survival, and inducing apoptosis, targeting major signalling components including AKT, AMPK, NFkB, COX-2, STAT3, etc. Most of the studies with LA were done using in vitro models, thus warranting future investigations with animal models to evaluate its pharmacological effects such as antidiabetic, anti-inflammatory and neuroprotective effects as well as to explore the underlying molecular mechanisms and toxicological profile. This review describes the chemistry, source, purification and therapeutic potentials of LA and it can therefore be a suitable guideline for any future study with LA.
Few studies have investigated the influence of emphysema on clinical features of patients presenting with community-acquired pneumonia (CAP).

The aim of this study was to examine the clinical and microbiological features of patients with both CAP and emphysema.

This retrospective study included patients with CAP who underwent computed tomography (CT) scan at the time of presentation. EGFR tumor Patients were allocated into emphysema and control groups, and clinical variables were compared between the two groups. The emphysema group was further divided into three subgroups (mild, moderate, and severe) according to the extent of emphysema on CT scan. The clinical variables of each subgroup were compared with the control group.

Of 1676 patients, 431 patients (25.7%) were classified into the emphysema group. CAP patients with emphysema were more likely to have a high CURB-65 score and pneumonia severity index and a lower incidence of complicated parapneumonic effusion or empyema. The emphysema group exhibited longer hospital stay. In addition, 30-day mortality in the severe emphysema group was significantly higher compared with the control group. As etiological agents, Streptococcus pneumoniae, Pseudomonas aeruginosa, Enterobacteriaceae, and multidrug-resistant pathogens were significantly more common in the emphysema group compared with the control group.

Here, we describe an easy-to-follow immunofluorescence protocol to localize proteins in whole-mount tissues of maize (Zea mays) and Arabidopsis. We present the whole-mount immunofluorescence procedure using maize ear primordia and Arabidopsis inflorescence apices as examples, followed by tips and suggestions for each step. In addition, we provide a supporting protocol to describe the use of an ImageJ plug-in to analyze colocalization. Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) associated acute liver injury (ALI) has been linked to poor outcomes in adults. Here we compare characteristics in children with elevated ALT (E-ALT) in two distinct manifestations of the infection, multisystem inflammatory syndrome-children (MIS-C) and coronavirus disease 2019 (COVID-19).

This is a retrospective study of patients ≤21years of age with positive for SARS-CoV-2 PCR. E-ALT was defined as alanine aminotransferase (ALT)>40U/L. Bivariate analysis and multivariable logistic regression were obtained to describe differences in children with and without E-ALT in COVID-19 and MIS-C.

E-ALT was detected in 36% of the 291 patients; 31% with COVID-19, and 51% with MIS-C. E-ALT in COVID-19 was associated with obesity (P<.001), immunocompromised status (P=.04), and chronic liver disease (P=.01). In the regression models, E-ALT in COVID-19 was associated with higher c-reactive protein (OR 1.08, P=.01) after adjusting for common independent predictors. Children with E-ALT and MIS-C were more often boys (P=.001), Hispanic (P=.04), or Black (P<.001). In MIS-C, male gender (OR 5.3, P=.02) and Black race (OR 4.4, P=.04) were associated with increased odds of E-ALT. Children with E-ALT in both cohorts had significantly higher multiorgan dysfunction, longer hospitalization, and ICU stay. Children with MIS-C had 2.3-fold increased risk of E-ALT compared to COVID-19. No association was found between E-ALT and mortality.

Since the flex-activation is non-destructive to the main polymer chains, the material can be subjected to multiple compression cycles to achieve iterative increases in the activation percentage of mechanophores. We examined the expiratory airflow dispersion during common neonatal resuscitation AGMPs using infant simulators.

Expiratory airflow dispersion in term and preterm manikins was simulated (n = 288) using fine particle smoke at tidal volumes of 5 ml/kg. Using ImageJ, we quantified dispersion during common airway procedures including endotracheal tube (ETT) and T-piece ventilation.

Maximal expiratory dispersion distances for the unsupported airway and disconnected uncuffed ETT scenarios were 30.2 and 22.7 cm (term); 22.1 and 17.2 cm (preterm), respectively. Applying T-piece positive end expiratory pressure (PEEP) via an ETT (ETT
) generated no expiratory dispersion but increased tube leak during term simulation, while ventilation breaths (ETT
) caused significant expiratory dispersion and leak.
