Patients with Idiopathic Pulmonary Fibrosis (IPF) and their caregivers experience stress, symptom burden, poor quality of life (QOL), and inadequate preparedness for end of life (EOL) care planning as the disease progresses. The hypothesis for this study was early introduction of palliative care in the course of IPF would improve knowledge and preparation for the EOL, patient reported outcomes, and advance care planning in IPF patients and their caregivers.

We sought to determine the feasibility, acceptability and efficacy of a nurse-led early palliative care intervention, entitled "A Program of SUPPORT" in patients with IPF and their caregivers.

This included a total of 3 resicalTrials.gov (NCT02929017).Purpose To compare clinical manifestations and disease outcomes in Epidemic Retinitis (ER) with positive or negative Weil Felix Test (WFT).Methods Retrospective, observational, comparative study. WFT positive or negative patients formed Group 1 and 2, respectively. Patients receiving oral doxycycline monotherapy formed subgroup A and B. Duration of resolution of macular edema and retinitis was compared.Results Novel finding of "ring retinitis" was observed equally in group 1 and 2. Complete resolution of macular edema took 41.3 days (range 30-60 days) and 43.68 days (range 20-105 days) (p = .668) and retinitis lesions resolved in 34.3 days (range 14-65 days) and 34 days (range 12-60 days) (p = .875) in group A and B, respectively. All (n = 14) eyes with retinitis within 1 disc diameter of fovea improved better than 20/80 except 1.Conclusion No significant difference with respect to clinical, imaging findings and the treatment outcome was observed in WFT positive or negative cases.Introduction Knee injury and Osteoarthritis Outcome Score (KOOS) is a widely used patient-reported outcome measure designed for patients with knee osteoarthritis.Purpose To evaluate responsiveness and determine minimal important change (MIC) for Persian-version of KOOS in patients with knee osteoarthritis following physiotherapy intervention.Method One hundred and forty-six consecutive patients with knee osteoarthritis, undergoing 4-week physiotherapy completed KOOS alongside Oxford Knee Score at baseline and at 4-week follow-up. Patients also rated their overall changes on an external anchor at follow-up. Responsiveness was examined in two ways by testing four hypotheses regarding pre-defined correlations between change scores of KOOS subscales and Oxford Knee Score and external anchor, and by calculating receiver operating characteristic (ROC) curve. MIC was determined by identifying an optimal cutoff on ROC curve.Results KOOS appeared to be responsive, as all expected hypotheses were accepted. Also, all subscales showed area under the curve (AUC) > 0.70. The MIC values reflecting improvement for Pain, Symptoms, Activities of Daily Living (ADL), Sports and Recreation (Sport/Rec), and Quality of Life (QoL) subscales were 15, 12.5, 15.5, 17.5, 12.5 points, respectively.Conclusions All subscales of KOOS have adequate responsiveness and are able to measure the change in patients with knee osteoarthritis following a 4-week physiotherapy intervention.The available findings on the association between iron status and risk of stroke remain controversial. We used multivariable logistic regression and restricted cubic spline models to explore the association between iron exposures and risk of stroke in the US National Health and Nutrition Examination Survey (NHANES 2007-2016, n = 24,627). A total of 941 (3.82%) stroke cases were identified in this study. In women, the ORs with 95% CIs of prevalence of stroke were 0.92 (0.65-1.28), 0.66 (0.44-0.98) and 0.72 (0.49-1.08) across quartiles 2-4 compared with quartile 1 of iron intake, respectively. An inverse and L-shaped association between iron intake and risk of stroke in women was observed, and the curve plateaued at 20 mg/day. However, neither serum iron concentrations nor iron intake were significantly associated with riskof stroke in men. To describe the feasibility and safety of revising vagal nerve stimulation surgery in terms of the indications, surgical techniques, and outcomes.

A retrospective study, a series of eight cases with VNS implants that needed revision surgery have been reviewed, four devices were completely removed and four were only revised. The revision surgery was performed after a range of 7months to 6years, due to different reasons. Initial surgeries and revisions were performed at the otolaryngology department in a major tertiary center.

We concluded that the previously implanted vagal nerve stimulation electrodes can be completely removed without any significant sequelae on the nerve. It may also be re-implanted safely at the previously used segment of the vagus nerve with a similar outcome in seizure control as the initial implantation.

The patient was objectively tested and diagnosed with functional anosmia at age 13 and reported they were experiencing a new sensation of smell at age 22. Our results show an electrophysiological signal for two unimodal odorants. The patient had a retronasal score in the hyposmic range and self-reported the ability to smell non-trigeminal odors, but reported being disturbed by the presence of the new sense and co-occurrence of phantosmia. We discuss our case in routes of neurogenesis and non-forming memory association with odors.Heavy metals (HMs) in farmland soils lead to adverse influences on ecosystem and human health. Despite that, data on quantitative risk from different sources are still scarce. In this study, 100 farmland soil samples in Jiuyuan District were collected and analyzed for selected HMs (As, Cd, Co, Cr, Cu, Mn, Ni, Pb, V and Zn) content characteristics and pollution statuses. The positive matrix factorization (PMF) model combined with the Nemerow integrated risk index (NIRI) and human health risk assessment (HHRA) was used to quantitatively identify the primary risk sources. The results indicated that the mean contents or median values (mg/kg) of 10 HMs were all higher than the background values. The contamination factor (CF) and pollution load index (PLI) revealed that the soil was severely polluted. Based on PMF, the main source of HM pollution was anthropogenic activities, accounting for 78.91%. Sewage irrigation represented the biggest input but was not associated with the highest risk. The results of PMF-based NIRI and PMF-based HHRA showed that the chemical fertilizers and pesticides were the largest and priority risk sources with contribution rates of 38.10% to ecological risk and 34.61 and 32.82% to non-carcinogenic and carcinogenic risk, respectively. In addition, non-carcinogenic risk of children was higher than that of adults, while the carcinogenic risk was the opposite. The integrated approaches were beneficial for priority risk quantification from different sources and can provide direct risk information and effective policy recommendations for management and control of key risk sources.
