CONCLUSION PD-MPe offers clinical advantages in terms of MP resection margin status, local recurrence, long-term mortality, and DFS. The lower MP positivity rate, achieved with PD-MPe, leads to better outcomes both in terms of OS and DFS.Fats, proteins and carbohydrates are the main energy supplies in human nutrition. The ratio of these three has often been discussed within the nutritional sciences over the years. Carbohydrates were important for our ancestors since many carbohydrate-rich foods were easily storable without cooling in comparison to protein-rich or fat-rich food, such as meat or fish. While humans consumed a mostly low-calorie nutrition and experienced seasonal changes in food availability and abundance for a long time, food supply changed in the last 100 years. We are now living in abundance, leading to a drastic increase in man-made and chronic diseases, such as diabetes mellitus. High, unregulated blood sugar levels in combination with the accumulation of advanced glycation end products seem to be major causes for the development of diabetes. Therefore, it is under discussion how healthy carbohydrates are and if they have to be avoided in nutrition.Agricultural intensification, with its associated habitat loss and fragmentation, is among the most important drivers of the ongoing pollination crisis. In this quasi-experimental study, conducted in intensively managed vineyards in southwestern Switzerland, we tested the separate and interdependent effects of habitat amount and fragmentation on the foraging activity and reproductive performance of bumblebee Bombus t. terrestris colonies. Based on a factorial design, we selected a series of spatially replicated study sites across a dual gradient of habitat amount (area of ground-vegetated vineyards) and fragmentation (density of ground-vegetated vineyard fields) in a landscape predominantly consisting of vineyards with bare grounds. The foraging activity of individual bumblebees was measured using the radio frequency identification (RFID) technology, and we assessed final colony size to measure reproductive performance. We found an interactive effect of habitat amount and fragmentation on colony size. More specifically, the degree of fragmentation had a negative effect on bumblebee colony size when the amount of habitat was low, while it had a weak positive effect on colony size in landscapes with high amounts of habitat. At the level of individual vineyard fields, ground vegetation cover exerted a positive effect on bumblebee colony size. Fragmentation, but not habitat amount, significantly influenced foraging activity, with more foraging trips in sites with lower degrees of fragmentation. Our results emphasise the importance of studying the separate and interdependent effects of habitat amount and fragmentation to understand their influence on pollinators, providing guidance for optimising the spatial configuration of agricultural landscapes from a biodiversity viewpoint.Humans receive information from their environment via the visual and auditory systems. This information protects us from dangers and guarantees vital actions, such as social interaction, locomotion, work processes and nutrition. The most important anatomical and functional features of these two sensory systems are compared and elucidated with respect to their interaction/functional complementarity. For this purpose, a selective literature search was carried out in the databases PubMed (also in the Europe PubMed Central), Psychline, Google Scholar, Cochrane Library and Web of Science. Additional information was obtained from relevant books and websites in the fields of (neuro)anatomy, (neuro)physiology, (neuro)ophthalmology and (neuro)otology. Search terms were Hörbahn, Sehbahn, visual system, auditory system, visual pathway, auditory pathway, receptors, spatial hearing, spatial cognition, auditory cognition and visual cognition.Aberrant microRNA (miRNA) expression is a central hallmark of hepatocellular carcinoma (HCC) and identification of the mechanisms underlying the miRNA actions should provide invaluable resource for revealing the molecular basis of different malignant behaviors in HCC. Previous high-throughput analysis has identified miR-767-5p as a unique miRNA signature of HCC, but the biological relevance and corresponding molecular basis of miR-767-5p in HCC is still in its infancy. The current study was, therefore, designed to elucidate whether changes in miR-767-5p expression levels affect HCC pathogenesis, and to further identify the putative targets. miR-767-5p expression was observed to be upregulated by ~ 3.7-fold in surgical HCC specimens as compared to that in adjacent normal hepatic tissues, and this up-regulation trend correlated well to disease progression and predicted a poor prognosis in HCC patients. Functionally, miR-767-5p-overexpressing cells had a significantly higher proliferative, migratory, and invasive potential, and exhibited an enhanced anchorage-dependent clonogenesis and a tumor formation potential in vivo. Lanifibranor purchase Mechanistically, PMP22, a core component of integral membrane glycoprotein of peripheral nervous system myelin, was further identified as a direct down-stream target of miR-767-5p in HCC cells. Conversely, stable ectopic expression of PMP22 abrogated the promoting effects of miR-767-5p on HCC aggressive phenotype. Collectively, the available data suggest that as a potent oncomiR, miR-767-5p actions along HCC progression are in part mediated by its function as a posttranscriptional repressor of PMP22 signaling.INTRODUCTION Reduction of the distal fibula into the fibular notch (FN) poses a problem that has not been fully resolved, yet. A number of methods have been developed for the assessment of the position of the fibula in the FN, but none of them is ideal. A majority of authors assess the FN 1 cm above the tibiotalar joint space, without specifying the reason for the choice of this distance. None of the previous studies has addressed at what level the FN is the deepest. Our findings show that it is 4-5 mm above the ankle joint space and verification of this hypothesis has been the aim of this study. MATERIALS AND METHODS Dry adult tibial bone specimens from the Pachner's collection of the Institute of Anatomy of 1st Faculty of Medicine, Charles University, Prague were used in the study. Height of the FN at its widest point, 3 mm and 10 mm above the articular surface of the distal tibia were measured in each specimen, as well as the depth of the FN at the deepest point, 3 mm and 10 mm above the articular surface of the distal tibia and the distance between the highest point of this surface and the deepest point of the notch. RESULTS The mean length of the tibia was 350 mm; the mean height of the FN was 42.5 mm; the mean width of the FN at its widest point was 23.6 mm, at 3 mm above the tibiotalar joint space 22 mm, 10 mm above this articular surface of distal tibia (tibial plafond) 18.9 mm. The mean depth of the notch at 3 mm above the tibial plafond was 3.8 mm, at 10 mm above this surface 4.1 mm. The maximum mean depth of the notch was 4.5 mm, the distance from this point to the highest point of the tibial plafond was 5.3 mm. CONCLUSION The deepest point of the FN lies 5 mm above the articular surface of the tibial plafond, with the mean value of the depth being 4.5 mm. This region is, therefore, ideal for assessment of the position of the distal fibula in the FN.PURPOSE The variability of the recurrent branch (RB) of the median nerve lends itself to an increased risk of injury when performing the minimally invasive approach for carpal tunnel release without its direct visualization. This risk is less so when it is released via the more invasive open approach as the RB can be easily identified, but the drawback is that of longer postoperative patient recovery time. Therefore, performing these releases via the less invasive approach should be more favorable for patients providing it could be done safely. Hence with there being a positive link between the hypertrophy of the thenar musculature and the course of RB according to previous studies. METHODS We dissected 28 hands of 14 donated bodies fixed using Thiel's method to try to demonstrate these findings of the associations among the RB, palmar creases and other superficial anatomical landmarks. Fisher's exact test was conducted to verify the relationship between those structures statistically. RESULTS Statistically significant links were found between the type of the RB and the type of the palmar creases (p value = 0.0094) and between the RB type and the palmaris longus muscle presence (p value = 0.028). CONCLUSION It was inferred that palmar creases and other superficial anatomical landmarks listed in the text could not be used to predict the variability of the RB and the choice of mini-invasive approach should not be based on their course.BACKGROUND There is evidence that the treatment of overt hyperthyroidism with thyroid hormones is able to reduce mortality as well as cardiovascular and musculoskeletal morbidity. It remains unclear whether these data can be extrapolated to the mildest form of hypothyroidism, subclinical hypothyroidism. Furthermore, it is uncertain whether and to what extent the threshold for therapeutic intervention needs to be modified in the elderly, in whom hypothalamo-pituitary regulation is increasingly insensitive to the negative feedback by thyroid hormones and the patients' response to thyroid hormones changes. OBJECTIVE The aim of this review is to evaluate the current evidence on the treatment of hypothyroidism in old age with regard to the initiation of therapy and the therapeutic goals. RESULTS AND CONCLUSIONS According to new original data and meta-analyses, therapy with thyroid hormones does not alter morbidity and mortality in patients with subclinical hypothyroidism with thyroid stimulating hormone (TSH) below the range of 7-10 mU/l. These data support the TSH threshold of 10 mU/l recommended in guidelines, particularly in elderly patients over the age of 65 years, in whom TSH serum levels increase with age. In contrast to the recommendations, the prescription of thyroxine more than doubled in a large study from Denmark and TSH levels decreased from 10 mU/l to under 7 mU/l between 2001 and 2015. As (the primarily unspecific) symptoms and quality of life are not altered by thyroxine replacement in studies on subclinical hypothyroidism and elderly patients are more susceptible to side effects, thyroid hormone substitution should generally not be started at TSH levels less then 10 mU/l.Peri- and postmenopausal disorders can have a significant impact on quality of life. Hormone replacement therapy (HRT) might be necessary in order to decrease women's symptoms. The German S3 guideline "Peri- and Postmenopause-Diagnostics and Therapy" (2020) provides recommendations that include the most recent evidence as well as the Women's Health Initiative (WHI) study results from 2002 and 2004. These results led to reduced prescription patterns due to a high risk of cardiovascular diseases as well as an increased risk for breast cancer if HRT had been administered. Both ongoing analyses of subgroups and other studies extenuated the WHI data, since the increased risks were neither generalizable to the typical postmenopausal patient (regarding age and risk profile) nor to the medication being used today. This article summarizes all aspects of HRT in peri- and postmenopausal women (indications, contraindications, practical approaches, risks, prevention) and provides recommendations with respect to the most recent S3 guideline.
