To explore diverse provider perspectives on strategies for addressing patient medication cost barriers; patient medication cost information gaps; current medication cost-related informatics tools; and design features for future tool development.

We conducted 38 semistructured interviews with providers (physicians, nurses, pharmacists, social workers, and administrators) in a Midwestern health system in the United States. We used 3 rounds of qualitative coding to identify themes.

Providers lacked access to information about patients' ability to pay for medications; true costs of full medication regimens; and cost impacts of patient insurance changes. Some providers said that while existing cost-related tools were helpful, they contained unclear insurance information and several questioned the information's quality. Cost-related information was not available to everyone who needed it and was not always available when needed. Fragmentation of information across sources made cost-alleviation information difficult to access. Providers desired future tools to compare medication costs more directly; provide quick references on costs to facilitate clinical conversations; streamline medication resource referrals; and provide centrally accessible visual summaries of patient affordability challenges.

These findings can inform the next generation of informatics tools for minimizing patients' out-of-pocket costs. Future tools should support the work of a wider range of providers and situations and use cases than current tools do. Such tools would have the potential to improve prescribing decisions and better link patients to resources.

Results identified opportunities to fill multidisciplinary providers' information gaps and ways in which new tools could better support medication affordability for patients.Branched-chain amino acids (BCAA) are three essential amino acids (AA) for lactating sows; however, the effects of dietary Leu, Val, and Ile on sow and litter performance within the literature are equivocal. The BCAA are structurally similar and share the first steps of their catabolism pathway where Leu, Val, and Ile are transaminated through BCAA aminotransferase and irreversibly decarboxylated by the branched-chain α-ketoacid dehydrogenase complex. Although these steps are shared among BCAA, Leu is recognized as the primary stimulator due to Leu's greater affinity towards the enzymes compared to Val and Ile. Since the late 1990s, sows are producing larger and heavier litters and generally consume diets with greater concentrations of Leu and crystalline AA, which may create imbalances among dietary BCAA. Research conducted with growing-finishing pigs confirms that high concentrations of Leu can impair BCAA utilization and growth performance. However, the effects of BCAA on lactating sow and litter performan performance. Although increasing dietary Val positively influences fat and protein composition of colostrum and milk, litter growth during lactation is not always positively affected. Given the competition among BCAA for utilization within mammary tissue, research evaluating the Leu and Ile requirement of modern lactating sows is warranted to fully understand the influence and interactions of BCAA on reproductive and litter growth performance.The objective of the experiment was to compare the effect of dietary inclusion of a prebiotic fermentation product of Lactobacillus acidophilus (LaP, RumaCell; 5 mL animal-1 d-1) or monensin on performance of replacement beef heifers. Heifers received a total mixed ration containing either LaP (n = 77) or monensin (MON; Rumensin; 200 mg animal-1 d-1; n = 79). Heifers were fed for 71 d in a GrowSafe unit, so individual feed intake could be measured. Heifers were weighed every 2 wk and feed efficiency calculated by residual feed intake (RFI). At the end of the RFI trial, heifers remained on their diets for an additional 27 d and were estrus synchronized using the 14-d CIDR + PG protocol and bred by artificial insemination (AI) followed by natural service. Prior to estrous synchronization, reproductive tract scores (RTS; 1 = infantile to 5 = cycling/presence of corpus luteum) were measured. Continuous variables were analyzed using generalized mixed models, whereas categorical data were analyzed by logistic regression. Body weights, average daily gain, feed intake, and RFI value were similar (P > 0.30) among MON- and LaP-supplemented heifers. Across treatments, heifers gained 0.9 ± 0.1 kg/d while consuming 9.3 ± 0.5 kg of diets daily. Reproductive development as indicated by RTS was similar (P > 0.28) between treatments. However, estrus response increased (P  0.33) for MON and LaP heifers. In conclusion, addition of LaP to replacement heifer diets may result in growth and reproductive performance similar to an ionophore, if dietary energy is adequate for normal heifer growth.Bulls often experience various levels of nutrient availability throughout the year. Nutritional management is a critical factor on overall ejaculate composition and the ability to get females pregnant. We hypothesized that differing nutritional levels and body condition score (BCS) affect reproductive fertility parameters in bulls. Mature Angus bulls (n = 11) were individually housed and randomly assigned to one of two dietary regimens 1) over-fed (n = 5) or 2) restricted (n = 6). Bulls were fed the same ration at different volumes to achieve desired effects resulting in eight individual treatments gain to an over-fed body condition score ([BCS]; GO), gain after nutrient restriction (GR), loss after an over-fed BCS (LO), loss from nutrient restriction (LR), maintenance at ideal adiposity (BCS = 6) after overfeeding (IMO), maintenance at ideal adiposity after nutrient restriction (IMR), maintenance at an over-fed BCS (BCS = 8; MO), and maintenance at a restricted BCS (BCS = 4; MR). Body weight (BW) and BCS wer 8.61) treatment, with no differences between the other treatments. Total defects increased during the LO (43.80 ± 9.55) treatment with similar increases in bulls during the GR (29.40 ± 9.55) and IMR (35.60 ± 9.55) treatments. In conclusion, male fertility was impacted when a deviation from a BCS of 6 occurred which could be detrimental to reproductive and beef production efficiency.Studies on the physiological states of wild-caught organisms are essential to uncovering the links between ecological and physiological processes. Bumble bee queens emerge from overwintering in the spring. At this time, queens develop their ovaries and search for a nest site in which to start a colony. Whether these two processes, ovary development and nest-searching, interact with or influence one another remains an unresolved question in behavioral physiology. We explored the hypothesis that ovary development and nest-searching might be mechanistically connected, by testing whether (1) ovary development precedes nest-searching behavior; (2) nest occupation precedes ovary development; or (3) ovary development and nest-searching occur independently, in bumble bee (Bombus vosnesenskii) queens. We collected queens either nest-searching (and thus prior to occupying a nest) or pollen-collecting (and thus provisioning an occupied nest) and measured their degree of ovary activation. We further screened these queens for parasites or other symbionts, to identify additional factors that may impact their reproductive success at this time. We found that queens searched for and occupied nests at all stages of ovary development, indicating that these processes occur independently in this system. HSP27inhibitorJ2 Nest-searching queens were more likely to have substantial mite loads than pollen-collecting queens, who had already located and occupied a nest. However, mite loads did not significantly predict ovary developmental status. Collectively, our work shows that nesting status and symbionts alone are insufficient to explain the variation in spring bumble bee queen ovary development. We propose that ovary development and nest-searching occur opportunistically, which may enable queens to begin laying eggs earlier in the season than if these processes occurred in discrete succession.The mechanical properties of intestinal tissues determine how a thin-walled structure exerts forces on food and absorbs the force of food as it enters and travels down the gut. These properties are critically important in durophagous and stomachless fish, which must resist the potential damage to foreign bodies (e.g., shells fragments) in their diet. We test the hypothesis that the mechanical properties of the alimentary tract will differ along its length. We predict that the proximal region of the gut should be the strongest and most extensible to handle the large influx of prey often associated with stomachless fish that lack a storage depot. We developed a custom inflation technique to measure the passive mechanical properties of the whole intestine of the stomachless shiner perch, Cymatogaster aggregata. We show that mechanical properties differ significantly along the length of the alimentary tract when inflated to structural failure, with 25-46% greater maximal stress, strain, extension ratio, and toughness at the proximal (25%) position. We also find that the alimentary tissues (excluding the heavily muscular rectum) are generally highly extensible and anisotropic, and do not differ in wall circumference or thickness along the alimentary tract. These findings contribute to our knowledge of the mechanical properties of fish intestinal tissues and guide future studies of factors influencing the evolution of fish alimentary systems.
Drug-induced myocarditis is a rare complication of certain cancer treatments, characterized by the development of myocardial inflammation shortly after initiation of treatment, potentially leading to heart failure and/or malignant arrhythmias. The development of eosinophilic myocarditis after administration of lenalidomide has been described and bortezomib has been associated with the development of cardiomyopathies and atherosclerosis.

A 69-year-old woman, recently diagnosed with multiple myeloma underwent local radiotherapy for a pathological fracture of the 4th lumbar vertebra and was treated with bortezomib-lenalidomide-dexamethasone. Within 19 days after therapy initiation, she presented with gastrointestinal symptoms, an erythematous pruritic rash, and general fatigue. Surprisingly, routine electrocardiogram (ECG) showed upwardly concave ST-elevation in I and aVL and ST-depressions in II, III, and aVF. Troponin levels were markedly elevated to 5470 ng/L. Complete blood count revealed eosinophilia. Based on further cardiac work-up, including echocardiography, coronary angiography, and cardiac magnetic resonance imaging (MRI) showing positive T2 imaging and patchy subepicardial late gadolinium enhancement, she was diagnosed with hypersensitivity myocarditis.
