Objective Acupuncture is one of the most widely used treatments of complementary and alternative medicine (CAM) within the military's health system. The success of CAM integration is partially dependent on both providers' and patients' perceptions that acupuncture is health-promoting. The aim of this research was to identify turning points, or changes, across treatments that enhanced or inhibited physicians' and patients' perception of acupuncture as health-promoting. Materials and Methods Using a retrospective-interview approach, interviews were conducted with 15 family medicine physicians practicing medical acupuncture in a family medicine setting and with 17 patients (N = 32). Turning points were separated into 2 groups (health-promoting or health-inhibiting). Similarities and differences between perspectives were noted. Results Patients and physicians identified two changes that enhanced their perspective of acupuncture as health-promoting (1) observed health changes and (2) pain-medicine/narcotic reduction/elimination. Patients identified their ability to fulfill personal or professional roles, whereas physicians identified (1) training experiences and (2) enhanced relationships with patients. Health-inhibiting changes in perspective were identified as logistical constraints/barriers by both parties, although their perspectives differed to some degree. Turning points that were viewed as health-inhibiting treatment were identified as clinical challenges by physicians and as a lack of consistency in care by patients. Conclusions The insight from these findings can help identify areas where medical acupuncture can be improved to promote successful integration in conventional medicine settings, as well as how providers can tailor communication with patients about acupuncture.Background Acupuncture, in general, is being proclaimed as an alternative analgesic amid the opioid crisis, and along with this, within emergency departments (EDs) there is a specific interest in a form of ear acupuncture called Battlefield Acupuncture (BFA). It is expected that BFA will be used more widely in emergency medicine as it is easy to learn and can be applied both promptly and easily. Teaching programs will be required to sustain this increased demand and upskill emergency clinicians in this skill. Objectives This article provides experiences and recommendations, based on faculty experiences from teaching BFA in Australia to ED clinicians combined with formal participant feedback. Main Points BFA courses were adjusted to suit ED doctors and nurses, along with their unique case mix and associated challenging environment. The content of the BFA courses included evidence, pain indications, contraindications, application, safety, mechanism of action, and how to negotiate barriers of credentialing. Workshops used the latest and most effective teaching methods that encompassed problem-based learning, infotainment, simulation, "four stage skills teaching," and "teaching on the run." Conclusion It is hoped that the experiences gained, and lessons learned in educating this new frontier of BFA to emergency clinicians will assist others in teaching BFA and its related techniques as a viable analgesic alternative in emergency medicine.An ab initio study of the interaction of O2, the most abundant radical and oxidant species in the atmosphere, with a Cu5 cluster, a new generation atomic metal catalyst, is presented. The open-shell nature of the reactant species is properly accounted for by using the multireference perturbation theory, allowing the experimentally confirmed resistivity of Cu5 clusters toward oxidation to be investigated. Approximate reaction pathways for the transition from physisorption to chemisorption are calculated for the interaction of O2 with quasi-iso-energetic trapezoidal planar and trigonal bipyramidal structures. Within the multireference approach, the transition barrier for O2 activation can be interpreted as an avoided crossing between adiabatic states (neutral and ionic), which provides new insights into the charge-transfer process and gives better estimates for this hard to localize and therefore often neglected first intermediate state. For Cu5 arranged in a bipyramidal structure, the O-O bond cleavage is confirmed as the rate-determining step. However, for planar Cu5, the high energy barrier for O2 activation, related to a very pronounced avoided crossing when going from physisorption to chemisorption, determines the reactivity in this case.In this work, a dielectrophoretic impedance measurement (DEPIM) lab-on-chip device for bacteria trapping and detection of Escherichia coli, Vibrio cholerae, and Enterococcus is presented. Through the integration of SU-8 negative photoresist as a microchannel and the precise alignment of the SU-8 microchannel with the on-chip gold interdigitated microelectrodes, bacteria trapping efficiencies of up to 97.4%, 97.7%, and 37.7% were achieved for E. coli, V. cholerae, and Enterococcus, respectively. The DEPIM device enables a high detection sensitivity, which requires only a total number of 69 ± 33 E. Selleck Filanesib coli cells, 9 ± 2 Vibrio cholera cells, and 36 ± 13 Enterococcus cells to observe a discernible change in system impedance for detection. Nonetheless, the corrected limit of detection for Enterococcus is 95 ± 34 after taking into consideration the lower trapping efficiency. In addition, a theoretical model is developed to allow for the direct estimation of the number of bacteria through a linear relationship with the change in the reciprocal of the overall system absolute impedance.Intraosseous hibernoma is an uncommon brown fat tumor that with about 2-dozen case reports described in the literature. Hibernomas are more commonly found in soft tissues of the thigh, shoulder, and back. However, more cases of intraosseous hibernomas are coming to light as a result of work-up due to pain or as an incidental lesion finding. Herein, we present a case of a thoracic intraosseous hibernoma suspicious for an isolated ovarian cancer metastasis, successfully treated with radiofrequency ablation and kyphoplasty.
