Visceral leishmaniasis (VL) or kala azar is one of the main neglected tropical diseases, of major importance due to the lethal nature of this zoonosis caused by protozoa of genus Leishmania. The study aimed to analyze the time trend in VL cases in the state of Piauí, Brazil, from 2008 to 2018, within the total period from 1971 to 2018. Data were obtained from the Brazilian Information System for Notificable Diseases (SINAN) and the Brazilian Mortality Information System (SIM). For burden of disease analysis, data were collected from the Institute for Health Metrics and Evaluation through the Global Burden of Diseases, Injuries, and Risk Factors (GBD Compare). Prais-Winsten regression analysis was used. A total of 2,374 cases were recorded with probable infection in the state of Piauí and 2,492 cases in residents of the state. The historical series analyzed, from 1971 to 2018, reaffirms the cyclical nature of the disease, based on the identification of epidemic peaks in 1983-1984, 1993-1994, 2003-2004, and 2013-2014. No significant alterations were identified in the trends in incidence, mortality, and case-fatality coefficients in the state. However, there was a significant increase in the incidence coefficient in the Chapada das Mangabeiras Development Territory, with an annual percent change of 17.5%, and a decrease in the Vale do Sambito Development Territory (-18.3%) and Vale do Rio Guaribas Development Territory (-8,1%). VL has a high burden of disease, measured by disability-adjusted life years, and is among the leading neglected tropical diseases in the state of Piauí. The study concludes by recommending the improvement of surveillance and control measures for this disease.HTLV is a retrovirus that affects the human T-lymphocytes. Transmission is bloodborne, sexual, or mother-to-child. Most infected persons are asymptomatic carriers, but some may develop severe cases such as tropical spastic paraparesis, leukemia/lymphoma, and other manifestations. Brazil is considered the country with the highest absolute number of cases, with an estimated 800,000 infected individuals. This article aimed to investigate and analyze the process of inclusion of HTLV on the government agenda in the states of Bahia and Minas Gerais, and which impacted health sector actions and policies. Various factors contributed to the theme's inclusion on the health sector agenda in Bahia, the state of Brazil with the widest range of policies on HTLV, involving high prevalence of the infection, a favorable political context, and activism by infected individuals. In Minas Gerais, we observed difficulties in the implementation of this health sector policy; due to the lack of organized advocacy groups, the problem was not a priority in the state's Health Departments. In the current context, individuals with HTLV infection and other stakeholders still have a long road ahead with social mobilization for investments in the area, drafting and implementation of public policies, and guarantee of social rights.The study analyzes the structural characteristics of primary health care (PHC) in its contextual and organizational dimensions in remote rural municipalities (counties) in Northern Minas Gerais State, Brazil. This is a case study with a qualitative approach, using 21 semi-structured interviews with health system administrators and health care workers from the family health teams (EqSF), as well as secondary data. For the contextual dimension, the results show that socioeconomic factors in the remote rural municipalities condition the organization of PHC and leave the population vulnerable, especially in the rural areas of the remote municipalities. As for the organizational dimension, the principal characteristics are coexistence of formal and informal assignment of the services' users, two modalities of first-contact services, namely basic health units (UBS) and 24-hour health centers; prioritization of response to the spontaneous demand; strong action by the Family Health Support Centers in the development of activities in promotion and prevention, expanded scope of practices by community health workers; partial guarantee of transportation for persons in treatment; partial computerization of the UBS with the implementation of the electronic patient record (e-SUS), telecardiology; and the More Doctors Program. The study found that remote rural municipalities are not a uniform unit, since the municipal (county) seat and the rural areas are unequal in terms of living conditions and lack specific organization, policies, and financing to guarantee access to PHC. With all the limitations, the observations show initiatives with major difficulties in maintenance and sustainability and sometimes without necessarily corresponding to the use of space and social life that define rural health itineraries.In the struggle for survival, millions of people submit to deregulated and informal work. Taking advantage of this, app-based companies have attracted growing numbers of workers under the fallacy of entrepreneurship. The current study aimed to describe the biosocial profile of these workers, the work process, and forms of company control, besides analyzing the principal and emerging aspects in the respective participants. This is a scoping study in which data collection was oriented by the question, "What has been produced in the scientific literature on the profile and risks of app-based work?" from 2014 to 2020. Content analysis revealed thematic dimensions the growth of apps in Brazil and worker relations; the profile of app-based workers; and their work exposure and demands for working conditions during the COVID-19 pandemic. The professional dynamic of app-based workers is marked by subordination, domination, and exhaustion. The fact that their employment relationship is not acknowledged does not deprive them of rights, dignity, and quality of life in the workplace. Spontaneous organization without trade union intermediation in this category of workers, backing a common agenda of demands to be discussed with other representatives of society, denotes the high degree of exploitation and precariousness and could suggest the path for app companies to adopt the principles of dignified, decent work.This study aims to investigate whether the intersectional identities defined by race/skin color and gender are associated with smoking and excessive consumption of alcohol in a representative sample of Brazilian adults. This is a cross-sectional study with 48,234 participants in the Brazilian National Health Survey (PNS) - 2013. Crude and adjusted odds ratios (OR) and respective 95% confidence intervals (95%CI) were used to estimate the associations of intersectional categories of race/skin color and gender (white woman, brown woman, black woman, white man, brown man, black man) with smoking and excessive consumption of alcohol, based on the combination of weekly "days" and "servings". The prevalence of smoking varied from 10.6% for white women to 23.1% for black men, while the prevalence of elevated consumption of alcohol ranged from 3.3% to 14%, respectively. In comparison to white women, only white, brown, and black men presented greater chances of smoking, reaching the OR of 2.04 (95%CI 1.66-2.51) in black men. As to excessive consumption of alcohol, all intersectional categories showed greater chances of consumption than white women, with the greatest magnitude in black men (OR = 4.78; 95%CI 3.66-6.23). These associations maintained statistical significance after adjustments made for sociodemographic, behavioral, and health characteristics. Results demonstrated differences in smoking habit and excessive consumption of alcohol when the intersectional categories were compared to traditional analyses. These findings reinforce the significance of including intersectionality of race/skin color and gender in epidemiological studies.Evolving evidence shows that ultra-processed food consumption may increase exposure to chemicals used in food packaging and production, such as per- and poly-fluoroalkyl substances, phthalates, and bisphenols. Studies suggested that these contaminants may be transferred from mother to child through placenta, increasing concerns for both maternal and child health. This study aimed to investigate the association of maternal consumption of ultra-processed foods with newborn exposure to perfluoroalkyl substances (PFAS) in the PIPA Project (The Rio Birth Cohort Study on Environmental Exposure and Childhood Development). The pilot cohort study conducted with 131 pregnant women-child pairs in a public maternity school in Rio de Janeiro, Brazil, was assessed. Maternal dietary intake in the third trimester of pregnancy was evaluated using a qualitative food frequency questionnaire. Food items were classified as non-ultra-processed food and ultra-processed food using the NOVA system and regular consumption of ultra-processed foods was estimated. Newborns of pregnant women who weekly consumed three or more subgroups of ultra-processed food presented the highest level of PFAS (2.47ng/mL; 95%CI 1.22; 3.72), compared to non-consumption of ultra-processed food investigated (0 ultra-processed food = 1.86ng/mL; 95%CI 1.38; 2.50). Additionally, cluster analysis grouped ultra-processed food, fish, and PFAS levels. link2 In conclusion, we found increased levels of PFAS in newborns whose mothers were higher consumers of ultra-processed foods.The study investigated how the interaction between axes of marginalization based on race/color, gender, schooling, and interpersonal discrimination affect different dimensions of quality of life in adult individuals. This is a cross-sectional study with data from the second wave of the EpiFloripa Adult Study in Florianópolis, southern Brazil, in 2012. We estimated linear regression models for each domain and for overall quality of life, measured with WHOQOL-Bref. The KHB method was used to estimate the mediating role of perceived discrimination in the associations between the axes of marginalization and quality of life. The analysis showed that schooling and gender operate additivelly, but not intersectionally, on overall quality of life and on the physical domain, with a disadvantage for women and individuals with 11 years of schooling or less. link3 Schooling and race/color were predictors of the environmental domain, with lower mean values for blacks and individuals with 11 years of schooling or less. In the psychological domain, the intersection between gender and schooling resulted in a mean value 2.9 points higher for women with 12 or more years of schooling. Gender and race/color were predictors of quality of life in the social domain, reducing the mean value for black women by 11.3 points. Mediation analyses showed that 29.6% of the effect of the intersection between schooling and gender on the psychological domain and 4.3% of the effect of the intersection between race/color and gender on the social domain were mediated by interpersonal discrimination. These results confirmed the study hypotheses, pointing to the importance and contribution of an intersectional analysis for studying inequities in quality of life.
