Barley grass polysaccharides (BGPs) are some of the major bioactive constituents of barley (Hordeum vulgare L.) grass (BG). They exhibit favorable biological activities and health benefits. In this study, seven BGPs were extracted from BG, which was harvested at three different growth stages (e.g., seedling, tillering, and stem elongation), by alkaline-extraction method. Their physicochemical properties, structural characteristics, and biological activities were investigated and compared. Results demonstrated that the extraction yields, chemical compositions, monosaccharide constituents, and molecular weights of the seven BGPs obtained at different growth stages varied obviously. These BGPs had similar preliminary structural characteristics but different microstructures and thermal properties. Furthermore, the BGPs (BGP-Z12 and BGP-Z21) obtained at the seedling stage possessed stronger in vitro antioxidant potentials, cholic acid binding activity, and immunological activity than other BGPs. Therefore, these results indicated that that the seedling stage of BG was the preferable harvest time for preparing highly bioactive BGPs.In India, cow-ghee has been used in traditional medicinal preparations to solubilize lipophilic drugs and enhance intestinal absorption. However, reports exploring the role of cow-ghee, naturally rich in saturated fatty acids, in carotenoid chemistry is nil. We attempted to understand the influence of fatty-acid composition of cow-ghee and edible oils on intestinal absorption of lutein in mice. The postprandial plasma lutein level in the mice administered with cow-ghee significantly (p less then 0.05) reached the maximum (Cmax-135.76 pmol/mL; AUC-592.80 pmol.h/mL) within 2 h (Tmax). Cow-ghee improved oral bioavailability of lutein by 2.02, 1.41 and 1.66 folds in comparison to control, olive oil and flaxseed oil respectively. Cow-ghee, composed of 69.28% saturated fatty-acids, has the potential to be a delivery vehicle for lutein as evidenced by higher postprandial triglyceride levels. This study is first of its kind which reports the influence of saturated fatty-acids on the oral bioavailability of lutein in an in-vivo system.Detection of low-volatile explosives in concentrations below 10-14 g/cm3 is a great challenge for portable ion mobility spectrometers (IMS) and field asymmetric IMS (FAIMS). We study the capabilities of FAIMS detector with ultraviolet laser ionization combined with organic additives (dopants) toluene and 1-methylnaphtalene to sense nitro-explosives trinitrotoluene (TNT) and low-volatile cyclonite (RDX) and nitropentaerythritol (PETN). Differential mobility coefficients were measured for target ion peaks of TNT, RDX and PETN. Presence of dopants in the sample results in multiple growth of ion yield at laser intensities lower than 2 × 107 W/cm2. Limits of detection with dopant-assisted laser ionization were determined 4.7 × 10-16 g/cm3 for RDX and 9.8 × 10-15 g/cm3 for PETN. Obtained results propose a way to further improve sensitivity of detectors along with improvement of portability of current laser-based FAIMS prototypes by using less powerful and smaller lasers.
Rates of substance use and mental health conditions vary across military service branches, yet branches also differ notably in terms of demographics and deployment experiences. This study examines whether branch differences in substance use and mental health outcomes persist after adjustment for a comprehensive set of demographic and deployment-related factors.
Data on 16,699 Armed Forces Active Duty service members were from the 2015 Department of Defense Health Related Behaviors Survey. Service branch-specific prevalences were estimated for self-reports of heavy episodic drinking (HED), possible alcohol use disorder (AUD), current smoking, e-cigarette use, smokeless tobacco use, prescription drug misuse, probable post-traumatic stress disorder (PTSD), probable depression, and probable anxiety. Using logistic regression, we assessed whether branch differences persisted after adjusting for an extensive array of demographic factors (among full sample) and deployment/combat factors (among ever-deployed subgroup).
HED, AUD, smoking, e-cigarette use, smokeless tobacco use, depression, and anxiety were highest in the Marine Corps; prescription drug misuse and PTSD were highest in the Army. HED, AUD, smoking, smokeless tobacco use, PTSD, depression, and anxiety were lowest in the Air Force; e-cigarette use and prescription drug misuse were lowest in the Coast Guard. Demographics and deployment/combat experiences differed across branches. After adjustment, service members in the Army, Marine Corps and Navy exhibited nearly 2-3 times the odds of multiple mental health conditions and substance use behaviors relative to the Air Force.

Although left frontal pole (LFP) repetitive transcranial magnetic stimulation (rTMS) has been recently investigated for the treatment of different substance use disorders, there is no current evidence that it can effectively influence craving or clinical outcomes in smokers. A single session of 1Hz rTMS over LFP is proposed to explore short-term effects of this protocol in tobacco use disorder.
A pilot randomized trial compared 1Hz rTMS of the LFP (n=12) and primary motor cortex (n=12) in a high-craving, severe nicotine dependence population (9 females, 15 males). A cigarette cue-reactivity paradigm with smoking-related and neutral visual stimuli was used for primary outcome measures. Chronic craving, dependence severity, impulsivity and cognitive measures were also obtained.
Compared to baseline, LFP rTMS significantly reduced cue-reactivity to both smoking-related and neutral cue types, while no change occurred in the motor cortex group. Reactivity to affectively neutral pictures was significantly reduced in the LFP vs. motor cortex analysis. The discordance rate between HER2-negative primaries and HER2 IHC2+ metastases that are ISH-amplified is unknown. We hypothesize that the majority of such cases are non-amplified. ISH testing is time-consuming and resource-intensive, and there may be situations where it is unnecessary. A retrospective review of IHC2+ metastatic lesions assessed with ISH at our center from 2013 to 2021 was undertaken. 105 cases were identified after exclusion of cases missing HER2 results, with primaries of unconfirmed origin, and cases of synchronous primary and metastatic disease. IHC and ISH results were recorded with detailed slide review of discordant cases. 91/105 metastases had HER2-negative primaries (87%). A metastasis was significantly more likely to be HER2-negative when the primary was HER2-negative (93%) versus positive (43%) (p less then 0.0001). 54/91 primaries were IHC2+/ISH-non-amplified, and 50/54 (93%) corresponding metastases had identical results. Of the 37 HER2-negative primaries that were IHC0/1+, 35 (95%) corresponding metastases were ISH-non-amplified. Six metastases in cases with HER2-negative primaries were discordant. Characteristics of metastases suggesting ISH testing was warranted to assess for discordance included IHC heterogeneity, morphological discordance, increased staining of moderate intensity, and ER/PR discordance. One or more of these factors were present in all discordant metastases. Our results suggest selective ISH testing on HER2 IHC2+ breast cancer metastases in the context of HER2-negative primary disease may be appropriate when there is careful review of the IHC. Validation of our findings awaits further studies with larger sample sizes.
The use of the teleconsultation system has increased in recent years, which enabled healthcare providers and patients to share, communicate and transfer information in real-time via different types of telemedicine services and applications. However, there is a lack of studies that underpin factors that motivate patients to use teleconsultation systems.
This study aimed to identify factors that influence consumers' motivation to use teleconsultation systems by developing a theoretical framework and empirically testing the developed framework with a real-world teleconsultation system.
A cross-sectional study was used for this research to empirically validate the proposed model. The online survey consisted of 51 items, which were developed by the researchers. Partial least squares structural equation modelling (PLS-SEM) was used to analyze the measurement model and structural model.

Pneumonia is the top communicable cause of death worldwide. Accurate prognostication of patient severity with Community Acquired Pneumonia (CAP) allows better patient care and hospital management. The Pneumonia Severity Index (PSI) was developed in 1997 as a tool to guide clinical practice by stratifying the severity of patients with CAP. While the PSI has been evaluated against other clinical stratification tools, it has not been evaluated against multiple classic machine learning classifiers in various metrics over large sample size.
In this paper, we evaluated and compared the prediction performance of nine classic machine learning classifiers with PSI over 34,720 adult (age 18+) patient records collected from 749 hospitals from 2009 to 2018 in the United States on Receiver Operating Characteristic (ROC) Area Under the Curve (AUC) and Average Precision (Precision-Recall AUC).
Machine learning classifiers, such as Random Forest, provided a statistically highly(p<0.001) significant improvement (∼33% in PR AUC and ∼6% in ROC AUC) compared to PSI and required only 7 input values (compared to 20 parameters used in PSI).
