Self Report Question 1:

Due to the symptoms of my disabilities, I can only do anything (including everyday tasks, cleaning, studying, etc.) with probably less than one third the speed, energy, and ability of a “regular” person. For example, in the middle of writing a sentence, I would be constantly distracted by discomfort in parts of my body. For doing tasks that require more effort, I would need to take many breaks to stretch, readjust my posture, do pain management, etc.

To not have my symptoms worsen, I am currently going to physiotherapy and and massage therapy in Newmarket, Ontario. I also take daily prescribed Effexor (75x2 mg) and Tylenol (extra strength), over the counter pain management gels, and use knee and ankle braces, etc. when moving around the house. If I go out of the house, I must use a cane and wheelchair. My mother can sometimes help drive me to therapy appointments in Newmarket. When she isn’t available, I use a transportation service called Routes Connecting Communities (fifty dollars per 2 hours). There are not many physiotherapy/massage therapy near where I live that I feel are suitable for my condition.

The answers for rest of the bullets points in the question “How Your Disability Affects Your Life” remain more or less the same as how I answered them for the initial application.
