Private ADHD Titration
There is currently a national shortage of medication to treat ADHD symptoms. This is due to manufacturing problems and an increased global demand.
Our service can provide private ADHD testing as an alternative to waiting for NHS funding through Right to Choose. Our fees for assessments include the cost of one prescription.
Titration
A highly experienced psychiatrist consultant will conduct a comprehensive assessment of your ADHD. This includes a medical history and an assessment using the common ADHD rating scales and questionnaires.
After your examination You will be advised whether medication is appropriate for your situation. This will be discussed at your follow-up appointment (which could be in person, via video link or by phone). Your physician will explain to you the various options for medication and, if you choose to begin taking medication, we will offer to titrate the dose to optimise the benefits/side effects balance for you.
This process may take several weeks. It involves adjusting the dosage of your child's medication gradually until the appropriate balance is reached. It is not uncommon to to titrate up to three different medicines before you can find the one that is most suitable for your child's ADHD. Parents must be involved in this titration process, and keep an watchful eye on their child's behavior, so that they can inform the doctor whether the medication is helping or causing adverse effects.
If you do not decide to return to NHS treatment, your medication will be issued with a private prescription which you will need to pay for at the pharmacy (this is not included in your consultation fees). You must attend six-monthly appointments to check your medication. This includes the measurement of pulse and blood pressure. Annual reviews are also required.
Follow-up appointments
Once the assessment is completed, you can schedule follow-up appointments with your psychiatrist. Typically, the appointment will take place via a zoom video call but in certain circumstances it might be possible to have an audio only consultation (again at an extra cost).
We will then go over treatment options and choose a bespoke, individualised ADHD medication adjustment. This will include a discussion and review of your medication dosage and any side effects that you might be experiencing. The cost for these titration sessions is PS150 for face-to face sessions and PS60 for online or telephone video consultations. Costs for medication are extra and are paid directly to the pharmacy.
The psychiatrist will ask you questions regarding your family history as well as the effects of your ADHD symptoms on your life, and any mental health issues that you may be experiencing. This is because ADHD can be a genetic condition and it is important to look into any family history that might indicate whether you are at risk of developing the disorder.
It is important to be honest and truthful to the doctor during this evaluation, as the information they gather will be used to determine if your ADHD fits the diagnostic criteria. This is a crucial step, since the wrong diagnosis can result in an inappropriate prescription of medication.
If you decide to be treated for ADHD, we'll arrange an appointment for a follow-up visit with a psychiatrist who specializes in this area. If you'd like to begin treatment immediately it is possible to start treatment at the time of diagnosis. You can also take two weeks to consider the implications of the diagnosis and the options available to you.
If you are unsure of what you would prefer, we suggest that you speak to your GP before booking your private appointment as certain providers have different policies about whether they require the submission of a GP referral letter or not. Find out if your GP accepts a Shared Care Agreement, which means that you will be referred to them when you leave the clinic for prescription medication.
Medication
For many who wish to obtain an individual diagnosis of ADHD medication is a key part of their journey. The process of getting a prescription from an NHS psychiatrist can be difficult and lengthy. NHS waiting lists are notoriously long and GPs are often reluctant to refer patients to specialist services.
Inquiring your GP to refer through Right to Choose is one way to avoid this (see psychiatry UK and ADHD 360). You can pick a provider that offers an assessment that is private and the titration. This will cut down your waiting time significantly and is more convenient when you are working and have a busy schedule.
Once you have been recommended by this method, your chosen clinician will assess you and decide whether or not you need to be titrated on medication. You will need to fill out forms that differ by provider but the most frequent ones are self-reporting forms and medical histories going to your childhood and into adulthood. They will help your physician determine any issues that could suggest you should not take ADHD medication if prescribed.
If you are titrated on medication, it is likely to be a combination of stimulants and non-stimulant medications. The titration procedure can be long and may require multiple appointments before your dose is stable.
The frequency of your follow-up appointments will be contingent on the type and quantity of medication that you are taking. They could be scheduled monthly or quarterly. The appointment will involve your doctor making sure that you are on the right medication, noting any adverse effects you've experienced, and making any adjustments needed to ensure that you're on a safe dosage. You are required to attend an annual review as well as a routine health exam that includes pulse, blood pressure and weight measurements.
For some individuals it could be because they are happy with their medication and don't need a further discussion about their titration. If, however, they do need further discussions about their titration and treatment plans, it's possible to be passed back to the private clinic that conducted their initial assessment. In this scenario the titration will be included in the consultation fee. The medication will be administered via an individual prescription. This may cost more than prescriptions from the NHS because the pharmacy will charge for the medication.
