Patients infected with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) frequently present with a febrile illness that may progress to pneumonia and hypoxic respiratory failure. Aerosolized epoprostenol (aEPO) has been evaluated in patients with acute respiratory distress syndrome and refractory hypoxemia. A paucity of literature has assessed the impact of aEPO in patients with SARS-CoV-2 receiving oxygen support with high flow nasal cannula (HFNC). The objective of this study was to evaluate whether aEPO added to HFNC prevents intubation and/or prolong time to intubation compared to controls only treated with HFNC, guided by oxygen saturation goals.
This was a single-center, retrospective study of adult patients infected with coronavirus 2019 (COVID-19) and admitted to the medical intensive care unit. A total of 60 patients were included. Thirty patients were included in the treatment, and 30 in the control group, respectively. Among patients included in the treatment group, response to theration in the rate of mechanical ventilation. Nevertheless, the application of this strategy may prolong the time to invasive mechanical ventilation, without affecting other clinical outcomes.
Diabetes, chronic kidney disease (CKD) and cardiovascular disease (CVD) are cardiometabolic diseases that remain amongst the leading causes of morbidity and premature mortality. Here, we review the current understanding of how anti-inflammatory intervention via inhibition of the pro-inflammatory but pleiotropic cytokine interleukin (IL) 6 may benefit patients with these or related diseases or complications.
Based on a PubMed literature search, this review integrates and contextualizes evidence regarding the clinical utility of anti-IL-6 intervention in the treatment of cardiometabolic diseases, as well as of the associated condition nonalcoholic hepatosteatosis.
Evidence implicates the pro-inflammatory effects of IL-6 in the pathophysiology of diabetes, CKD and CVD. Thus, targeting the IL-6 pathway holds a therapeutic potential in these cardiometabolic disorders. However, because IL-6 has multiple homeostatic roles, antagonizing this cytokine may be associated with side effects, such as increased risk on NASH, but the clinical usefulness of targeting IL-6 in this hepatic disorder remains largely unexplored.1. An investigation was conducted on the effects of aflatoxin (AFL)-contaminated diets and feed withdrawal periods from 0 to 12 h in broiler chickens at 28 d of age. Both factors can potentially affect liver colour and can cause failure at veterinary inspection in the slaughterhouse.2. A total of 240, one-d-old female Cobb 500 broiler chickens were fed a common corn-soy pre-starters (d 1-7) and then either a non-contaminated control (CON) or feed with 1 ppm AFL (AFL) from d 8 to 28. The inoculum of AFL had 792 ppb of aflatoxin B1, 35 ppb of aflatoxin B2 and 219 ppb of aflatoxin G1. On d 28, all broilers were weighed and euthanised for necropsy following three different feed withdrawal time periods (0, 6 or 12 h), in a 2 × 3 factorial arrangement.3. Body weight gain, liver weight and liver fat content decreased as feed withdrawal lengthened, whereas FCR and gallbladder weight increased (P ≤ 0.05). AFL-fed birds had reduced body weight and proportion of liver fat and increased FCR, liver and gallbladder weights (P ≤ 0.05).4. Livers from fed broilers (0 h withdrawal) showed more lightness (L*) and yellowness (b*) than livers of broilers from 6 or 12 h withdrawal (P ≤ 0.05). The L* and redness (a*) values of livers from broilers fed diets COB were lower than those from AFL fed broilers (P ≤ 0.05).5. Prolonging pre-slaughter feed withdrawal decreased liver L*, whereas feeding AFL increased liver b*. These findings can be used to support veterinary assessment in slaughterhouses as shackled birds move on line through the inspection site. Since chicken liver is a valuable organ and an indicator of animal health, attention must be paid to these differences to ensure consumer safety.A common but preventable complication of diabetes is diabetic foot ulcer. If appropriate care is not provided such foot lesions progress to the most severe diabetic foot complication, like infection, gangrene, amputation and even death. Diabetic neuropathy results abnormal planter pressure points under the foot and triggers the tendency of foot ulcer. The aim of this paper is to present the development of a low cost, power efficient, soft, lightweight and simple in-shoe planter pressure measurement system. The system is capable to determine the average static pressure under ball and heel of the foot. The insole is comfortable due to the use of textile pressure sensor and its simple data acquisition method makes operation easy for the users. An experiment with 10 participants with and without diabetes was carried out to observe the outcome of the system. The practical implication of this study is to minimise the damage caused by foot ulcer by determining the pressure abnormality at earliest with a fully developed cost effective design. The system is capable to identify the difference in average planter pressure values in different groups of participants. To monitor the foot health proactively, the proposed system is found to be a useful device and can successfully scan the planter pressure under ball and heel of the foot.Joint inflammation results from soft-tissue injuries and cartilage damage. PRICE is the standard treatment approach for acute soft tissue injuries like ankle sprain. Electrical stimulation, application of orthotic braces, etc. is also effective for this. In a synergistic device all these components are combined and applied simultaneously. This device was developed for joint inflammation and tested for grade I & II acute ankle joint sprain. To test a synergistic - semirigid device, combining PRICE & electrical stimulation for acute ankle sprains of grades I & II for pain, range of motion and swelling is a case series was the objective. Device was developed using novel concept of synergistic applications of PRICE with electrical stimulation. The joint contour of ankle and specific biomechanical bony surface landmarks were considered. Ethical approval was taken from NTCC committee, AIPT. Recordings were taken from eight patients of acute ankle sprain with - in two days of injury, after getting ethical approval. Elevation to the ankle was provided by keeping the part over the pillow and data was recorded with the help of 1.VAS scale for pain; 2. Measuring tape; 3. Goniometry. t-test was used to find out the significant difference pre and post the application of device. There was a significant reduction in pain (p = 0.006), edema (p = 0.011), dorsi-flexion (p = 0.015), and plantar flexion (p =0.008). The synegistic device has been effective for acute ankle inflammation - grade I & II ankle sprains in 5 sessions; sufficient for the return of function.
Growing interest in non-animal-based models has led to the development of devices to expose cells to airborne substances. Cells/tissues grown at the air-liquid interface (ALI) are more representative of lung cells/tissues
compared to submerged cell cultures. Additionally, airborne exposures should allow for closer modeling of human lung toxicity. However, such exposures present technical challenges, including maintaining optimal cell health, and establishing consistent exposure monitoring and control. We aimed to establish a reliable system and procedures for cell exposures to gases at the ALI.
We tested and adapted a horizontal-flow ALI-exposure system to verify and optimize temperature, humidity/condensation, and control of atmosphere delivery. We measured temperature and relative humidity (RH) throughout the system, including at the outlet (surrogate measures) and at the well, and evaluated viability of lung epithelial A549 cells under control conditions. Exposure stability, dosimetry, and toxicity were tested using ozone.
Temperatures measured directly above wells vs. outflow differed; using above-well temperature enabled determination of near-well RH. Under optimized conditions, the viability of A549 cells exposed to clean air (2 h) in the ALI system was unchanged from incubator-grown cells. In-well ozone levels, determined through reaction with potassium indigotrisulfonate, confirmed dosing. Cells exposed to 200 ppb ozone at the ALI presented reduced viability, while submerged cells did not.
Our results emphasize the importance of monitoring near-well conditions rather than relying on surrogate measures. Rigorous assessment of ALI exposure conditions led to procedures for reproducible exposure of cells to gases.

Multidisciplinary teams are effective at providing treatment for people with LTNCs; however, access to such services by people with disabilities can be difficult and as a result, good quality care is not universal. One potential solution is telehealth. This review describes the potential of telehealth to support people with LTNCs, the challenges of designing and implementing these systems, and the key recommendations for those involved in telehealth to facilitate connected services that can benefit patients, carers and healthcare professionals. These recommendations include understanding the problems posed by LTNCs and the needs of the end-user through a person-centred approach. We discuss how to work collaboratively and use shared learning, and consider how to effectively evaluate the intervention at every stage of the development process.The positional statement of the Asian Cardiovascular and Thoracic Annals is presented.
With aging, the human atrium invariably develops amyloid composed of ANP (atrial natriuretic peptide) and BNP (B-type natriuretic peptide). Preamyloid oligomers are the primary cytotoxic species in amyloidosis, and they accumulate in the atrium during human hypertension and a murine hypertensive model of atrial fibrillation susceptibility. We tested the hypothesis that preamyloid oligomers derived from natriuretic peptides cause cytotoxic and electrophysiological effects in atrial cells that promote arrhythmia susceptibility and that oligomer formation is enhanced for a mutant form of ANP linked to familial atrial fibrillation.
Oligomerization was assessed by Western blot analysis. Bioenergic profiling was performed using the Seahorse platform. Mitochondrial dynamics were investigated with immunostaining and gene expression quantitated using quantitative reverse transcription polymerase chain reaction. Action potentials and ionic currents were recorded using patch-clamp methods and intracellular calcium me oligomers are novel mediators of atrial arrhythmia susceptibility. Moreover, the accelerated oligomerization by mutANP supports a role for these mediators in the pathophysiology of this mutation in atrial fibrillation.
