The incidence of syphilis infections is on the rise, particularly among African American men and men who have sex with men, and it is reaching epidemic levels in these communities throughout the United States. Although syphilis is relatively inexpensive to treat and cure and is a predictor for HIV incidence among men and transgender women who have sex with men, rates of co-screening for syphilis are low in the emergency department setting, with a dearth of literature on this topic since the 1990s and early 2000s. In this case study, we describe an operational model for routine syphilis screening implemented in June 2017 at the University Hospitals Cleveland Medical Center in Cleveland, Ohio. We describe the advantages of screening using a reverse testing algorithm rather than the traditional method and the necessity of partnering with the Cleveland Department of Public Health for both diagnostic and follow-up logistics.The inhibitory control is a component of the executive functions that allows the individual to inhibit inadequate behaviors, resist distractions and select a relevant stimulus when executing activities. In the neuropsychology field, evidences of stimulation and improvement of the inhibitory control through school interventions is sought by using computerized software, such as digital games. These research studies constitute an important investigation area within the executive functions in ecological approaches. This paper presents a systematic mapping study on inhibitory control stimulation in elementary school children with the use of digital games. The investigation encompassed an automated database search with further backward snowballing procedure with the final selection for additional publications as research strategy. The automated search considered six databases SCOPUS, PubMed, IEEE Explore Digital Library, ACM Library, Springer Link, and Scielo. The initial database search found 641 works published between 2014 and 2019. After the exclusion and inclusion criteria were considered, three publications related to digital games or mobile applications were found and selected for analysis, which focused on inhibitory control or correlated processes stimulation in school-based interventions with elementary school children. Results indicated that investigations within the field are incipient, pointing to an emerging research area.
Childcare is an important setting for nutrition; nearly half of young children in the United States participate in licensed childcare, where they consume up to two-thirds of their daily dietary intake. We compared state regulations for childcare with best practices to support breastfeeding and healthy beverage provision.

We reviewed regulations for childcare centers (centers) and family childcare homes (homes) in effect May-July 2016 and rated all 50 states for inclusion (1 = not included, 2 = partially included, 3 = fully included) of 12 breastfeeding and beverage best practices. We calculated average ratings for 6 practices specific to infants aged 0-11 months, 6 practices specific to children aged 1-6 years, and all 12 practices, by state and across all states. We assessed significant differences between centers and homes for each best practice by using McNemar-Bowker tests for symmetry, and we assessed differences across states by using paired student
tests.

Solutions have focused on supply-reduction strategies. These approaches, however, have failed to significantly curtail opioid overdose and related death. Demand for opioid use arising from social networks and environment is an important contributing factor to the current opioid epidemic. Adoption of existing underused methods is needed to drive further progress. This Perspective proposes the social contagion model as a promising framework through which to operationalize evaluation of the influence of social networks and environment in the opioid epidemic and argues for its greater application. Comparing the current epidemic with previous opioid epidemics reiterates the utility of the social contagion model. This model acknowledges social network influence on individual behavior. It leverages tools from epidemiology, permits evaluation of interpersonal influence, facilitates consideration of disproportionate and collateral effects, and overcomes limitations of traditional models and geographic assumptions inherent to many approaches surrounding the current opioid epidemic. Analyzing the opioid epidemic within a social contagion framework will enhance evaluation methods and enable the design of interventions to reflect the actual demands of the current crisis. If the influence of social networks and environment is not considered, the devastating toll of the opioid epidemic could grow.Policy makers are increasingly using performance feedback that compares physicians to their peers as part of payment policy reforms. However, it is not known whether peer comparisons can improve broad outcomes, beyond changing specific individual behaviors such as reducing inappropriate prescribing of antibiotics. We conducted a cluster-randomized controlled trial with Blue Cross Blue Shield of Hawaii to examine the impact of providing peer comparisons feedback on the quality of care to primary care providers in the setting of a shift from fee-for-service to population-based payment. Over 74,000 patients and eighty-eight primary care providers across sixty-three sites were included over a period of nine months in 2016. Patients in the peer comparisons intervention group experienced a 3.1-percentage-point increase in quality scores compared to the control group-whose members received individual feedback only. This result underscores the effectiveness of peer comparisons as a way to improve health care quality, and it supports Medicare's decisions to provide comparative feedback as part of recently implemented primary care and specialty payment reform programs.
Anterior cruciate ligament (ACL) ruptures are common, but the ideal timing for ACL reconstruction after injury is unclear with regard to meniscal insult.

To determine whether there is a relationship between timing from ACL rupture to ACL reconstruction and development of meniscal tears within this period.

Systematic review and meta-analysis.

A systematic literature search was undertaken independently by 2 reviewers using the Cochrane method for systematic reviews in 5 online databases. The reviewers performed independent data extraction and assessment of risk of bias and study quality. The search included any comparative study, including randomized controlled trials (RCTs), prospective and retrospective cohort studies, and case-control studies of an adult population, that assessed the relationship between timing of ACL reconstruction surgery and rates of meniscal tears.

After screening, 12 studies (No. of participants = 3042) out of 3390 records were included for analysis 3 RCTs (n = 272), 2 prospecrate of medial meniscal tears within this prereconstruction time frame. Further high-quality prospective studies may help determine whether this is a causal effect. However, based on current evidence, in those patients for whom ACL reconstruction is indicated, ACL reconstruction within 3 months of injury may be recommended.

Its unique design with uncoupling of fixation and sealing and the lowest profile in the market has expanded the treatment in AAA with challenging neck anatomies and, especially, in the females presenting mostly with narrow iliac access vessels. Morever, a new design modification, the Ovation Alto was recently introduced in the Market, while certain off-label uses of the Ovation in treating either juxtarenal- or short-necked AAA have been proposed. Areas covered Aim of this article was to present the current articles with the long-term results of the Ovation endograft and discuss the new developments and modifications in its use. Expert commentary the Ovation stent-graft exhibits very satisfactory clinical long-term results in AAA treated within the instructions-for-use, expands significantly the treatment of AAA in females and overcomes reliably challenging anatomical issues of the infrarenal neck that would render these cases ineligible for treatment with any other endograft. Εxperience with the specific, unique technical features of Ovation as well as careful selection of patients and meticulous preoperative study of the AAA anatomy are key-elements to the successful performance of this endograft.

) causes chronic brain inflammatory process and may thus contribute to both depression and its most serious complication, suicidal behavior. In this study, we hypothesized that latent toxoplasmosis may underlie current depression and/or suicidal behavior.

Currently depressed individuals (
 = 384) and age, sex, and residence-matched healthy controls (HC) (
 = 400) were tested for latent toxoplasmosis (i.e., positive serum
IgG antibodies). Exclusions included positive IgM and negative IgG antibodies indicating acute
infection and history of cognitive problems. Depression severity and history of lifetime suicide attempts were assessed using Beck Depression Inventory (BDI) and Columbia Suicide Severity Rating Scale, respectively.

Participants with seropositive anti-
IgG antibody had a significantly higher odds of being depressed compared with seronegative participants (OR = 2.9, 95% CI 1.9-4.3;
 < 0.001). BDI score wassion and suicidal behavior. Screening for, and treatment of, underlying T. gondii infection may help improve depression and curb the increasing suicide rates. Depressed T. gondii seropositive individuals have increased likelihood to have history of suicidal behavior.In the current study, we investigated the potential of Cronobacter sakazakii- ethylenediaminetetraacetic acid (EDTA) assisted phytoremediation potential of Zea mays L. to remediate lead (Pb)-contaminated soils. The C. sakazakii exhibited various stress tolerance mechanisms via plant growth promoting (PGP) traits, intrinsic extracellular enzyme production and antibiotic resistance. A greenhouse experiment was conducted to examine the dual effects of plant growth promoting endophytic bacteria (PGPEB)-chelator synergy in maize plants under different Pb contaminated soil regimes. C. sakazaii-EDTA (5 mM EDTA kg-1) complex significantly (p  less then  0.05) enhanced plant growth and biomass (48.91%); chlorophyll a, b and carotenoid contents (27.26%, 25.02% and 42.09%); relative water content (61.33%); proline content (63.60%); root and shoot Pb accumulation capacity (52.31% and 44.71%) in Pb contaminated soils. This may suggest the efficacy of current approach in enhancing plant tolerance capability toward Pb-uptake and phytoremediation capacity.
