The Cochrane Skin-Core Outcome Set Initiative (CS-COUSIN) and the Core Outcome Measures in Food Allergy (COMFA) Initiative Joint Meeting was held virtually on September 23 through 28, 2021. This meeting brought together core outcome set (COS) developers within the domains of dermatology and food allergy, respectively. The meeting assessed the state of COS methodology, provided a forward look, and facilitated the networking of members of individual COS groups. The meeting hosted several expert-led key addresses and focus sessions, in which work was critically discussed, new developments were shared, and groundwork was laid for future projects. The conference emphasized the importance of ongoing COS work to improve patient outcomes and limit research waste.While current studies have supported oral minoxidil as a novel, adjunctive therapy in non-scarring forms of alopecia, there continues to be limited data on oral minoxidil for these conditions. To assess oral minoxidil use in the treatment of androgenetic alopecia and telogen effluvium, a multi-center, retrospective analysis was conducted in 105 adult patients treated for androgenetic alopecia and/or telogen effluvium with oral minoxidil (dose range 0.625-2.5 mg) once daily for ≥ 52 weeks, case matched by age (± 5 years) and gender with 105 controls with androgenetic alopecia and/or telogen effluvium who were not treated with oral minoxidil. 80 women (76.2%) with a mean age of 57.5 ± 13.56 (range 24-80) and 25 men (23.8%) with a mean age of 40.4 ± 13.79 (range 19-63) were included. Efficacy was evaluated based on provider assessment of clinical response and clinical photographic evaluation using a 3-point scale (worsening, stabilization, and improvement). 52.4% of patients demonstrated clinical improvement and 42.9% demonstrated stabilization. There was a significant difference in clinical response between the patient and control group, p  less then  0.001. Retrospective study design. These results suggest that oral minoxidil can be an effective treatment in androgenetic alopecia and telogen effluvium.
Total knee arthroplasty (TKA) and unicompartment knee arthroplasty (UKA) are among the most important treatment options for end-stage knee osteoarthritis. Previous papers have noted the importance of knowing the type of medial tibial wear in deciding to manage varus knee osteoarthritis patients with TKA vs UKA. But few studies have delineated the pre-operative variables predicting the type of tibia wear.

This study assessed individuals with varus knee osteoarthritis planned for knee arthroplasty. After recording the demographic variables, hip-knee-ankle joint alignment views were taken from all patients. Finally, the type of tibial wear encountered during the surgery (posteromedial, non-posteromedial) was documented.

A total of 325 knees and 301 participants were evaluated in the study. Participants aged 67.12 (± SD 8.14) and the male to female ratio was 0.20. Between either non-posteromedial/posteromedial wear or insufficient/sufficient ACL cases, there was a statistically significant difference with regard to MPTA, LDFA, VA, and JCA (P value < 0.05). Sixty-three percent of knees had non-posteromedial wear in the tibia plateau and 37% had posteromedial wear. Posteromedial wear was associated with 95% chance of ACL tear. Non-posteromedial knee had nearly 50% chance of having ACL insufficiency. Among non-posteromedial cases, VA of 14.5 as cut-off value had 65% sensitivity, 90% specificity, 73% negative predictive value, 87% positive predictive value, and 78% accuracy in detecting ACL insufficiency.

Posteromedial tibial wear is associated with ACL insufficiency. However, regarding non-posteromedial cases, varus angle > 14.5 is highly predictive of ACL tear (87% positive predictive value).

A reduction in meat intake is recommended to meet health and environmental sustainability goals. This study aimed to evaluate the effectiveness of an online self-regulation intervention to reduce meat consumption.

One hundred and fifty one adult meat eaters were randomised 11 to a multi-component self-regulation intervention or an information-only control. The study lasted 9weeks (1-week self-monitoring; 4-week active intervention; and 4-week maintenance phase). The intervention included goal-setting, self-monitoring, action-planning, and health and environmental feedback. Meat intake was estimated through daily questionnaires in weeks 1, 5 and 9. The primary outcome was change in meat consumption from baseline to five weeks. Secondary outcomes included change from baseline to nine weeks and change in red and processed meat intake. We used linear regression models to assess the effectiveness of all the above outcomes.

Across the whole sample, meat intake was 226g/day at baseline, 118g/day at five weeks, and 114g/dayat nine weeks. At five weeks, the intervention led to a 40g/day (95%CI -11.6,-67.5, P = 0.006) reduction in meat intake, including a 35g/day (95%CI -7.7, -61.7, P = 0.012) reduction in red and processed meat, relative to control. There were no significant differences in meat reduction after the four-week maintenance phase (-12g/day intervention vs control, 95% CI 19.1, -43.4, P = 0.443). Participants said the intervention was informative and eye-opening.

The intervention was popular among participants and helped achieve initial reductions in meat intake, but the longer-term reductions did not exceed control.

ClinicalTrials.gov NCT04961216, 14th July 2021, retrospectively registered.This single-arm phase 3 study was conducted to confirm the results of our phase 2 study of bendamustine (B)-rituximab (R) in patients with relapsed/refractory diffuse large B cell lymphoma (rrDLBCL). The primary endpoint was overall response rate (ORR). Autologous stem cell transplantation-ineligible rrDLBCL patients with ≤ 2 prior chemotherapy regimens received R 375 mg/m2 IV on day 1 and B 120 mg/m2/day IV on days 2 and 3 every 21 days up to 6 cycles. Thirty-eight patients with a median age of 74 years (range, 43-86) received BR. The ORR and complete response rates were 76.3% and 47.4%, respectively. With a median follow-up of 19.5 months including long-term follow-up, median progression-free survival was 11.9 months. Median OS was 29.2 months. Discontinuation of treatment due to Gr3-5 TEAE was observed among 13 of 38 patients (34.2%). One patient with cytomegalovirus enterocolitis died during follow-up. This BR regimen was confirmed to be effective and tolerable in studied patients. Given their heterogeneous appearance and increasing detection on cross-sectional imaging, it is essential that radiologists understand the variable presentation and distinctions PaNENs display compared to other pancreatic neoplasms. Additionally, some of these neoplasms may be hormonally functional, and it is imperative that radiologists be aware of the common clinical presentations of hormonally active PaNENs. Knowledge of PaNEN pathology and treatments may influence which imaging modality is optimal for each patient. Each imaging modality used for PaNENs has distinct advantages and disadvantages, particularly in different treatment settings. Thus, the focus of this manuscript is to provide an update for the radiologist on PaNEN pathology, imaging, and treatments.
Senescence-associated transcription factor ATAF2 regulates cytokinin signalling and in vitro shoot multiplication in banana plants. MusaATAF2-like protein is a stress-related NAC transcription factor of banana. It regulates senescence in rooted banana plants. During the early stages of plant development under in vitro conditions, the presence of 6-benzylaminopurine leads to vigorous shoot multiplication. The major contributor to plant shoot multiplication is auxin to cytokinin ratio and their signalling components. The LC-MS analysis of transgenic banana plants overexpressing MusaATAF2 indicated significantly higher cytokinin content and remarkably lower auxin content. Auxin transport has been reported to be inhibited by flavonoids. Their significantly higher abundance in the shoot tissues in transgenic lines suggested potential negative regulation of auxin signalling in transgenic plants. Enhanced shoot multiplication in transgenic lines was further corroborated by reduced transcript abundance of type-A Arants overexpressing MusaATAF2 indicated significantly higher cytokinin content and remarkably lower auxin content. Auxin transport has been reported to be inhibited by flavonoids. Their significantly higher abundance in the shoot tissues in transgenic lines suggested potential negative regulation of auxin signalling in transgenic plants. Altogether, the data concludes that MusaATAF2 induces cytokinin hypersensitivity in banana shoots by modulating/regulating the cytokinin signalling components and flavonoids content.The prevalence of Parkinson's disease (PD) is rising, rendering it one of the most common neurodegenerative diseases. Treatment and monitoring of patients require regular specialized in- and outpatient care. Patients with PD are more likely to have a complicated disease course if they become infected with severe acute respiratory syndrome coronavirus type 2 (SARS-CoV-2). Regular in-hospital appointments place these patients at risk of exposure to SARS-CoV-2 due to travel and contact with other patients and staff. However, guidelines for the management of outpatients with PD during times of increased risk of infection are currently lacking. These are urgently needed to conduct risk-benefit evaluations to recommend the best medical treatment. This article discusses best practice approaches based on the current literature, as suggested by the multidisciplinary Network of University Medicine (NUM) in Germany. These include measures such as mask-wearing, hand hygiene, social distancing measures, and appropriate testing strategies in outpatient settings, which can minimize the risk of exposure.
