y interventions in children with unclear speech.Purpose To determine the effect of Voglibose add-on therapy on daily glycemic excursions (using FreeStyle® Libre Pro™, a Flash glucose monitoring system) in Indian patients with type 2 diabetes mellitus (T2DM) receiving a stable dose of metformin (Met) or metformin+sulfonylurea (Met+SU). Patients and Methods T2DM patients with glycosylated hemoglobin (HbA1c) ≥7.0% and at least two postprandial excursions ≥140 mg/dL (within 2 h of meal) during the screening phase (visit 1/day -14 ± 2) were enrolled in this prospective, multicenter interventional study. The patients were randomized at visit 2 (day 0 ± 2) to receive Voglibose 0.2 or 0.3 mg tablets (BID/TID) as add-on therapy to Met and Met+SU. All the patients were followed at day 14 ± 2 (visit 3), month 3 ± 14 days (visit 4), 14 weeks (i.e., visit 4 + 14 days) ±2 days (visit 5), and month 6 ± 14 days (visit 6). Continuous glucose monitoring was performed to study glycemic excursions at visits 2, 3, and 5. The study outcomes were change in average number of glycight time glucose, and average MAGE and HbA1c levels was reported from baseline to week 14 in the Met+Voglibose group and the Met+SU+Voglibose group, respectively. At 6 months, body weight, glucose levels, cholesterol, low-density lipoprotein-cholesterol, and HbA1c were significantly lower, especially in the Met+SU+Voglibose arm. Conclusion Voglibose was useful in reducing glycemic variability and improving glycemic control in Asian Indian adults with T2DM. (CTRI/2018/04/013074).Purpose The purpose of this study was to explore the role of psychological processes (i.e., internalized shame, self-efficacy, psychological inflexibility) regarding hearing loss in the well-being of adults who have hearing loss. Method This study used a cross-sectional survey design. Two hundred sixty-four surveys were submitted for analysis. Data were collected using a survey posted to online social media support, in audiology clinics across the country, and through national organizations supporting individuals who have hearing loss. Regressions were used to determine whether internalized shame, self-efficacy, or psychological inflexibility held any predictive value over subjects' well-being. Results Shame, self-efficacy, and psychological inflexibility significantly predicted different aspects of well-being. Conclusion Audiologists should consider the role of internalized thoughts and emotions regarding patients' hearing loss on how patients understand and manage their hearing health.Fetal growth restriction can affect health outcomes in postnatal life. This study tested the hypothesis that the response to an inflammatory pulmonary insult is altered in pediatric fetal growth restricted rats. Using a low-protein diet during gestation and postnatal life, growth restricted male and female rats, and healthy control rats, were exposed to an inflammatory insult via the intratracheal instillation of heat killed bacteria. After 6 hours, animal lungs were examined for lung inflammation and status of the surfactant system. The results showed that in response to an inflammatory insult, neutrophil infiltration was decreased in both male and female rats in the growth restricted animals as compared to controls. The amount of surfactant was increased in the growth restricted animals, as compared to control, regardless of the inflammatory insult. It is concluded that fetal growth restriction results in increased surfactant and altered neutrophil responses following pulmonary insult.Supramolecular chemistry provides an effective strategy for the molecular recognition of diverse molecules. Significant efforts to design synthetic hosts have enabled the successful binding of many types of guests; however, less is known about how host-guest environments influence binding. Herein, we present a comprehensive study in which we measure the host-guest binding of a bis(arylethynyl phenylurea) host with a chloride guest in eight solvents spanning ET(30) values ranging from nonpolar (40.7 kcal mol-1) to polar (47.4 kcal mol-1). Polar solvents show significantly weaker binding in comparison to nonpolar solvents, and the bulk solvent polarity parameter, ET(30), shows a linear free-energy relationship with respect to the free energy of binding in the host-guest complex. These studies provide a better understanding of how host-guest binding in flexible receptors is governed by their environments and highlight the importance of host reorganization contributions in the free energy of binding. In addition, these studies highlight that preorganization may not be as important as previously thought for weak binding in which enthalpic contributions are smaller versus in polar solvents where solvent effects are magnified.Ecumicins are potent antituberculosis natural compounds produced by the rare actinomycete Nonomuraea sp. MJM5123. Here, we report an efficient genetic manipulation platform of this rare actinomycete. CRISPR/Cas9-based genome editing was achieved based on successful sporulation. Two genes in the ecumicin gene cluster were further investigated, ecuN and ecuE, which potentially encode a pretailoring cytochrome P450 hydroxylase and the core peptide synthase, respectively. Deletion of ecuN led to an enhanced ratio of the ecumicin compound EcuH16 relative to that of EcuH14, indicating that EcuN is indeed a P450 hydroxylase, and there is catalyzed hydroxylation at the C-3 position in unit12 phenylalanine to transform EcuH16 to the compound EcuH14. Furthermore, promoter engineering of ecuE by employing the strong promoter kasO*P was performed and optimized. We found that integrating the endogenous ribosome-binding site (RBS) of ecuE together with the RBS from kasO*P led to improved ecumicin production and resulted in a remarkably high EcuH16/EcuH14 ratio. Importantly, production of the more active component EcuH16 was considerably increased in the double RBSs engineered strain EPR1 compared to that in the wild-type strain, reaching 310 mg/L. At the same time, this production level was 2.3 times higher than that of the control strain EPA1 with only one RBS from kasO*P. In this work, we show that cell synchronization during the stages of the cell cycle, obtained from long-term cultivation under specific growth conditions, greatly enhances the efficiency of transformation and allows the identification of DNA repair mechanisms that occur preferentially at different stages of the cell cycle. We demonstrate that the transformation of synchronized cells at different times was differentially associated with nonhomologous end joining (NHEJ) and/or homologous recombination (HR), and makes it possible to knock-in specific foreign DNA at the genomic nuclear location desired by exploiting HR. The Cu(II) mononuclear complex, poised in a distorted trigonal bipyramidal structure, reduces oxygen at an onset potential of 0.50 V vs RHE. Kinetics study by hydrodynamic voltammetry and chronoamperometry suggests a stepwise mechanism for sequential reduction of O2 to H2O2 to H2O at a single-site Cu-catalyst. The foot-of-the-wave analysis records a turnover frequency of 5.65 × 102 s-1. At pH 7.0, complex 1 undergoes a quasi-reversible mixed metal-ligand-based reduction and triggers the reduction of dioxygen to water. Electrochemical studies in tandem with quantum chemical investigation, conducted at different redox states, portray the active participation of ligand in completing the process of proton-coupled electron transfer internally. The protonated carboxamido moiety acts as a proton relay, while the quinoline-based orbital supplies the necessary redox equivalent for the conversion of complex 1 to Cu(II)-hydroperoxo species. Thus, a suitable combination of redox non-innocence and proton shuttling functionality in the ligand makes it an effective electron-proton-transfer mediator and subsequently assists the process of oxygen reduction.The distribution and composition of polychlorinated biphenyls (PCBs) within soil-plant systems around a notorious e-waste recycling site were investigated. The average total PCB concentrations in rhizospheric soils (RSs) and nonrhizospheric soils (NRSs) were 2160 and 1270 pg g-1 dry weight (DW), respectively. PCBs were more enriched in RS than NRS for most vegetable species. PCB accumulation in plant tissues varied greatly among plant cultivars, ranging from 4020 to 14 500 pg g-1 DW in shoots and from 471 to 24 400 pg g-1 DW in roots. The compositions of PCBs in soil and plants showed that hexa- and hepta-chlorinated PCBs were preferentially accumulated in soils, while tri- and tetra-PCBs were abundant in plant tissues. These results indicated that low-chlorinated PCBs might be prone to accumulation and transfer within plants, which was confirmed by the relationship between the root concentration factor and octanol-water coefficient. The first eluting enantiomers of PCB 84 and PCB 95 were preferentially transferred between the soil and plants, while the stereoselectivity of PCB 136 varied among plant species.
