With the majority of UK mothers now accessing social media for support, Breastfeeding Support Facebook (BSF) groups have increased exponentially. BSF groups vary in type (local or national/international) and in moderation-overseen by breastfeeding mothers and by midwives or trained lactation specialists. Some groups aimed at supporting mothers in a specific geographical area also have associated face-to-face groups, facilitated as either professional or peer support. Little is currently known about these specific local groups, their prevalence, impact or value to mothers. This paper examines mothers' experiences of using local BSF groups and why they value them as part of a larger study exploring the impact of midwife moderation on these groups. An online survey consisting of open and closed questions was completed by 2028 mothers. Findings identified that local BSF groups are widely used and highly valued for their connection with local face-to-face services and other mothers. They offer access to expertise and shared experience in a format mothers find convenient and timely, improving confidence and self-efficacy. Local BSF groups enable the formation of support networks and development of breastfeeding knowledge that mothers credit with increased well-being, motivation and breastfeeding duration. As such, they have the potential to add value to local face-to-face services and improve breastfeeding experiences and knowledge in communities. The findings have important implications to support the development of integrated online interventions to improve public health.This is a comprehensive and rigorous review of currently available data on the use of mesh in the pelvis in colorectal surgery. MR elastography can determine organ-related stiffness, which reflects the degree of fibrosis. Liver stiffness increases in cirrhosis, and stiffness increases further post-prandially due to increased portal blood in-flow. Non-selective beta-blockers (NSBB) reduce the portal venous inflow, but their effect on liver and spleen stiffness are disputed.

To assess whether MR elastography of the liver or spleen reflects the severity of cirrhosis, whether treatment with NSBB changes liver and spleen stiffness and whether changes in stiffness can predict the effect of NSBB on portal pressure.

Fifty-two patients with cirrhosis underwent liver vein catheterization and two-dimensional (2D) MR elastography on separate days. Thirty-six of the patients had a hepatic venous pressure gradient (HVPG) of ≥12mmHg and were tested prior to, and after, intravenous infusion of NSBB using HVPG measurement and MR elastography.

HVPG showed a strong, positive, linear relationship with liver stiffness (r
=0.92; P<.001) and spleen stiffness (r
=0.94; P<.001). The cut-off points for identifying patients with a HVPG≥12mmHg were 7.7kPa for liver stiffness (sensitivity 0.78, specificity 0.64) and 10.5kPa for spleen stiffness (sensitivity 0.8, specificity 0.79). Intravenous administration of NSBB significantly decreased spleen stiffness by 6.9% (CI 3.5-10.4, P<.001), but NSBB had no consistent effect on liver stiffness. However, changes in spleen stiffness were not related to the HVPG response (P=.75).

Breastfeeding outcomes have yet to be explored among women who give birth to macrosomic (birthweight ≥4000 g) infants, a cohort for whom the benefits of breastfeeding may be particularly valuable, offering protection against later-life morbidity associated with macrosomia. This longitudinal prospective cohort study aimed to identify whether women who give birth to macrosomic infants are at greater risk of breastfeeding non-initiation or exclusive breastfeeding (EBF) cessation. A total of 328 women in their third trimester were recruited from hospital and community settings and followed to 4 months post-partum. Women gave birth to 104 macrosomic and 224 non-macrosomic ( less then 4000 g) infants between 2018 and 2020. Longitudinal logistic regression models calculated odds ratios (ORs) and 95% confidence intervals (CIs) to assess likelihood of EBF at four timepoints post-partum (birth, 2 weeks, 8 weeks, and 4 months) between women who gave birth to macrosomic and non-macrosomic infants, adjusted for maternal risk (obesity and/or diabetes), ethnicity and mode of birth. Macrosomic infants were more likely to be exclusively breastfed at birth and 2 weeks post-partum than non-macrosomic infants with adjusted OR = 1.94 (95% CI 0.90, 4.18; p = 0.089) and 2.13 (95% CI 1.11, 4.06; p = 0.022), respectively. There were no statistically significant associations between macrosomia and EBF at 8 weeks or 4 months post-partum. Macrosomia may act as a protective factor against early formula-milk supplementation, increasing the likelihood of EBF in the early post-partum period, but rates of exclusive breastfeeding continued to decline over the first 4 months post-partum.
To evaluate the effect of charcoal-based dentifrices on the color change (CC) and surface wear (SW) of resin composites.

Five dentifrices, including three toothpastes Colgate total-12 (C12), Black is the new White (CPX), and Natural (NAT); and two powders Carvvo (CVV) and Whitemax (WMX), were evaluated. Composite blocks (5x5x2mm- Z350 3M/ESPE, shade A2) were subjected to 417 and 5004 brushing cycles. The CC (n=7, CIEDE2000; ΔE
) was evaluated using a spectrophotometer. SW (n=5) was assessed using profilometry. The dentifrice particles were characterized by scanning electron microscopy (SEM).

In 417 brushing cycles, resin composite exhibited higher color change upon being exposed to NAT, CVV and WMC than to C12 (p≤ 0.05). In 5004 brushing cycles, resin composite showed higher color change values when exposed to all dentifrices, except C12 (p≤ 0.05). The powders promoted higher SW on resin composite than C12 in both evaluation times (p≤ 0.05). In 417 cycles, WMX caused higher SW on composite than C12, CPX, and NAT (p≤ 0.05). No significant different SW was found between powders and charcoal-based toothpastes after 5004 brushing cycles (p > 0.05). SEM showed larger particles for powder than for toothpaste dentifrices.

Within the limitations of this study, both null hypotheses have been rejected. Resin composites exposed to charcoal-based dentifrices exhibited significantly (p ≤ 0.05) higher color change and surface wear than conventional toothpastes.

Gastrointestinal infections represent a risk factor for functional gastrointestinal and somatoform extraintestinal disorders. We investigated the prevalence and relative risk (RR) of gastrointestinal and somatoform symptoms 5months after SARS-CoV-2 infection compared with a control cohort.

One hundred and sixty-four SARS-CoV-2 infected patients and 183 controls responded to an online questionnaire about symptoms and signs during the acute phase of the infection and after 4.8±0.3months. Presence and severity of gastrointestinal symptoms, somatization, anxiety, and depression were recorded with standardized questionnaires. Stool form and presence of irritable bowel syndrome (IBS) were also recorded. Any association between exposure to infection and symptoms was evaluated by calculating crude and adjusted RR values and score differences with 95% confidence intervals (CI).

An incremental number of cases of acute transverse myelitis (ATM) in individuals with ongoing or recent coronavirus disease 2019 (COVID-19) have been reported.

A systematic review was performed of cases of ATM described in the context of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection by screening both articles published and in preprint.

Twenty cases were identified. There was a slight male predominance (60.0%) and the median age was 56years. Neurological symptoms first manifested after a mean of 10.3days from the first onset of classical, mostly respiratory symptoms of COVID-19. Overall, COVID-19severity was relatively mild. Polymerase chain reaction of cerebrospinal fluid for SARS-CoV-2 was negative in all 14 cases examined. Cerebrospinal fluid findings reflected an inflammatory process in most instances (77.8%). Aquaporin-4 and myelin oligodendrocyte protein antibodies in serum (tested in 10 and nine cases, respectively) were negative.
