The increase in demand of building stones and construction-grade sand has resulted in aggressive hard rock quarrying in many parts of the world. The problems are to be evaluated in detail for the judicious use of resources on the one hand and ensuring health of the ecosystems on the other. The present study aims to evaluate the impact of hard rock quarrying on one of the important twin - river basins in SW India, the Netravati-Gurpur river basin, which hosts the fast developing urban centre, the Mangalore city. A total of 64 hard rock quarries are located in the basin which together extracts about 6.75 million t y-1 of rocks/rock products for different purposes. An assessment of the impacts of quarrying using the rapid impact assessment matrix (RIAM) reveals marginal, short-term positive impacts in economic-operational components but major negative impacts on all the other environmental components. The study indicates that the present intensity of quarrying is unsustainable with a sustainability index of - 0.25. Therefore, hard rock quarrying in the study area is to be strictly regulated for bringing down the river basin degradation to the barest minimum level and maximizing the ecosystem benefits to its full potential.
Otitis media with effusion (OME) associated with Samter's triad (ST) is a difficult entity to treat. The aim of study was an investigation of the middle ear and nasal production of inflammatory mediators (IM) in patients with ST and analysing differences between them and controls.

Prospective case-control study. Nineteen patients with OME (five had allergic rhinitis, four had nasopharyngeal lymphoid hyperplasia, five had no evident sino-nasopharyngeal disease and five had confirmed ST) and 15 healthy participants were included. The concentrations of IM interleukin-1 beta (IL-1β), interferon-alpha 2 (IFN-α2), interferon-gamma (IFN-γ), tumor necrosis factor-alpha (TNF-α), monocyte chemoattractant protein-1 (MCP-1), IL-6, IL-8, IL-10, IL-12p70, IL-17A, IL-18, IL-23 and IL-33 were measured in nasal and middle ear secretions.

There was a difference that was close to a level of statistical significance only for IL-1β levels in middle ear fluid (p = 0.052) between the ST subgroup and the other patients with OMease.
This preliminary report showed some differences in IM production between the patients with OME associated with ST and those without it. Our results suggest a uniformity of the production of nasal and middle ear IM and supported the concept of a united airway respiratory disease.In tropical climate countries, animal production faces a huge challenge, being the high solar irradiation levels an important factor that negatively influences the welfare and animal performance. The purpose of this research was to check the environment inside mobile shelters with different kinds of cover materials. This research has been developed at the State University of Goiás, Brazil, being used four different cover materials DFPC, double-faced plastic canvas; PVC, polyvinyl chloride; VFB, tiles made with vegetable fiber and bitumen; and VFBW, tiles made with vegetable fiber and bitumen being the outer face painted white. Covers were placed up mobile shelters with the following dimensions 1 m length × 1 m width × 1 m height and cover pitch of 30°. The environment was evaluated through thermal comfort indexes enthalpy (H), temperature and humidity index (THI), black globe and humidity index (BGHI), mean radiant temperature (MRT) and thermal heat load (THL), and the measurement of thermal environmental variables (temperature of external (EST) and internal (IST) surface of the cover, dry bulb temperature (Tdb), black globe temperature (Tbg), relative humidity (RH), and wind speedy (WS)). Data analysis was performed using PROC MIXED of SAS® by analysis of variance (ANOVA), followed by the Tukey test, at 5% of significance level. There was observed beneficial effect of the usage of shading, with a decrease in air temperature and thermal comfort indexes when compared with the unshaded environment. The materials presented different thermal behavior, with better results for PVC and VFBW, that presented lower internal (IST) and external surface temperature (EST), in addition to lower black globe temperature, thermal heat load, and black globe and humidity index. PVC and VFBW were the best types of covers evaluated, with significant decreased thermal comfort indexes.
Retained products of conception (RPOC) refer to the presence of placental and/or fetal tissue in the uterus following delivery, miscarriage, or termination of pregnancy. The presence of such tissue might lead to complications, which might be the culprit of secondary infertility. Although some studies have considered the management of symptomatic RPOC, there are no data regarding the management of asymptomatic, incidentally diagnosed RPOC, nor the optimal time for surgical intervention required to prevent adverse reproductive outcomes.

This study aimed to examine whether the time interval between the pregnancy termination to surgical evacuation of RPOC influences the reproductive outcome in asymptomatic women.

This is a retrospective cohort study, which includes women who were admitted for an elective procedure in the gynecology day-care clinic due to suspected RPOC. The diagnosis was made during patients' routine examination following either delivery or miscarriage between the years 2010 and 2018.

Adenomyosis is a benign uterine disease resulting from the myometrial invasion of the endometrial gland and stroma. In the current study, angiogenesis, apoptosis and energy metabolism were investigated in adenomyosis.

A retrospective study was performed using paraffin archival tissues. Three groups were included in the study Group I and Group II; ectopic and eutopic endometrial tissues of patients with adenomyosis, respectively, and Control Group; endometrial tissue of individuals without adenomyosis. Vascular endothelial growth factor (VEGF), epidermal growth factor (EGF), intercellular adhesion molecule 1 (ICAM-1) and hypoxia-inducible factor 1 alpha (HIF-1A) levels were evaluated as angiogenic markers. Bcl-2, caspase-9 and caspase-3 levels were investigated as apoptotic indicators, and isocitrate dehydrogenase 1 (IDH1), succinate dehydrogenase complex subunit C (SDHC) and fumarate hydratase (FH) levels were also examined as energy metabolism markers. Gene expression levels of all parameters were determined by RT-PCR.

VEGF expression levels were found to be increased in Group I according to the control group and Group II. Bcl-2 expression levels were found to be increased in the Group I compared to the Group II. It was determined that expression levels of IDH1 were decreased in the Group I and Group II compared to the Control Group. There was no significant difference in the other examined parameters. Although we did not find a significant difference in HIF-1A levels between the groups, we found a positive correlation between VEGF and HIF-1A in the Group I.

Historically, general anaesthesia (GA) has been used to facilitate incision and drainage, with or without exploration for a fistula-in-ano (FIA). This review aims to assess outcomes following management of perianal abscesses without GA in infants less than 24 months old. We aim to identify the success of management without GA. Using PRISMA guidelines, studies were identified from MEDLINE, EMBASE and PubMed. Studies including infants less than 24 months with perianal abscesses managed without GA were reviewed. The primary outcome was the number of patients requiring GA following initial management without GA. Secondary outcomes included rates of recurrent perianal abscesses, rates of progression to FIA and time to cure. Nine studies, involving 1049 infants less than 24 months old met inclusion criteria. A total of 1039 (99.0%) were males. Study design consisted of one prospective case series of 18 patients, and eight retrospective cohort studies. There were no randomized control trials. A total of 1037 (98.9%) patients were initially managed without GA. Of these, 59 (5.6%) were documented to subsequently require a general anaesthetic. Treatment modalities included antibiotics, regular baths, needle aspiration, incision and drainage under local anaesthesia, hainosankyuto and fibroblast growth factor. A total of 790 (75.3%) healed primarily without further intervention. A total of 243 (23.2%) were documented to have a recurrence or progression to FIA. We evaluated the CD3 + , CD8 + , and Foxp3 + T-lymphocyte densities in tumor centers and invasive margin regions of 389 patients with surgically resected stage II/III GC who received 5-FU-based adjuvant chemotherapy and investigated the impact of IS on survival. In univariate analysis, high CD3 + , CD8 + , and Foxp3 + T-lymphocyte densities in the invasive margin were correlated with better prognosis (all P  less then  0.05). Patients with high IS had significantly longer disease-free survival (DFS; P  less then  0.001) and overall survival (OS; P  less then  0.001). In multivariate analysis, IS demonstrated a powerful prognostic impact on patient outcome [DFS, hazard ratio (HR) = 0.465; 95% confidence interval (CI), 0.306-0.707, P  less then  0.001; OS, HR = 0.478; 95% CI, 0.308-0.743, P = 0.001]. Additionally, although all EBV-positive cases had high IS, IS was similar in both microsatellite instability (MSI)-high and microsatellite stable (MSS)/MSI-low groups (83.3% and 80.5%, respectively). Subgroup analysis according to MSI status revealed that high IS patients had significant DFS and OS benefits in both MSS/MSI-low (DFS, HR = 0.527, 95% CI, 0.341-0.816, P = 0.004; OS, HR = 0.528, 95% CI, 0.334-0.837, P = 0.007) and MSI-high (DFS, HR = 0.166, 95% CI, 0.033-0.826, P = 0.028; OS, HR = 0.177, 95% CI, 0.036-0.883, P = 0.035) groups. Thus, the assessment of immune cell infiltration based on IS may provide a strong indicator of survival in stage II/III GC patients with curative resection following 5-FU-based adjuvant chemotherapy.
