A quantitative structure-activity relationship model was developed using 40 compounds and two selected descriptors. These descriptors indicated the importance of pKa and molar refractivity for the inhibitory activity against SaDHFR. The values of R2train, CVLOO and R2test generated by the model were 0.808, 0.766, and 0.785, respectively. The integration between QSAR, molecular docking, ADMETox analysis and molecular dynamics simulations with binding free energies calculation, yielded the compounds PC-124127620, PC-124127795 and PC-124127805 as promising candidates to SaDHFR inhibitors. These compounds presented high potency, good pharmacokinetics and toxicological profile. Thus, these molecules are good potential antimicrobial agent to treatment of infect disease caused by S. aureus.Communicated by Ramaswamy H. Sarma.
Sweden updated its legislation on universal free school meals in 2011 and nutrition was explicitly mentioned. This study i) describes cross-sectional changes in school lunch nutritional quality during the following eight years, and ii) examines if repeated self-auditing, using a fully automated, online tool (School Food Sweden), based on the implementation strategy of audit and feedback, was associated with improvements.

Primary schools who self-selected to audit meal quality between March 2012 and July 2019.

Almost half of all (ca 4800) primary schools signed up to use the tool, and 1500 audited nutritional quality at least once. Repeated cross-sectional analyses showed the proportion meeting the nutritional criteria increased significantly between 2012/13 (11%) and 2018/19 (34%). Longitudinally, each additional audit completed increased the odds of meeting the nutritional criteria by 1.30 (CI 1.20-1.41), controlling for region and time elapsed since the legislative change. In 774 schools with repeat audits, both number of audits and frequency of accessing feedback predicted meeting the nutritional criteria (OR 2.02, CI 1.23-3.31), even after adjusting for time since the legislative change and days elapsed since previous audit.

Both legislation and self-audit with automatic feedback appear effective in helping schools to improve school meal quality. Self-audit with feedback may be an effective complement to legislation, or a promising alternative in settings where regulation is not an option.

The COVID-19 pandemic led to the cancellation of healthcare appointments and to lower demand, which generated unmet healthcare needs. The aim of this study was to evaluate their prevalence and distribution in Portugal.

Data came from the "Survey of Health, Ageing and Retirement in Europe". Between June and August 2020, 1118 Portuguese individuals aged 50 or over were inquired about unmet healthcare needs due to i) fear of getting infected; ii) cancellation by the doctor/healthcare services; iii) unsuccessful consultation request. The analysis of the prevalence of unmet needs was complemented by the calculation of the concentration indices as a function of the variables income, education and health status.

About 60% of respondents reported at least one unmet need, which was almost twice the European average. Motive ii) cancellation by the doctor/healthcare services was the most frequent. The prevalence of unmet needs differed depending on income level and health status. The indices evidence the concentration of unmet needs in individuals with the worst health status, although for the reason fear of infection the concentration occurred in those with higher levels of income and education.

Our study showed a high prevalence of unmet needs and their concentration in individuals with worse health status. Given the association between unmet needs and the subsequent deterioration of health, these results should raise concerns about the near future.

Fibromyalgia is a contested illness with unknown aetiology and poorly understood development. The present aim is to explore the pre-diagnostic illness trajectory narrated by individuals recently diagnosed with fibromyalgia (FM).

Individual interviews about the course of the illness were conducted with seven women and three men (age from early 20s to 50s) who had recently been diagnosed with FM. A narrative analysis of what the interviewees told and how the stories were narrated was conducted.

The findings are expressed by three storylines. "Strenuous life and alerted body preluding illness" displays a difficult, unsupported life and bodily sensitivity to stimuli. "Recurrent pains unfolding to become a lasting and complex illness" describes individuals pushing themselves to meet social obligations until they come to a full stop. "Diagnosed but still uncertain presence and future" portrays satisfaction with finally being diagnosed with FM and being supported by others, but still there are no solutions as to do about it.

The three storylines portray a long, winding trajectory of suffering starting before the onset of illness, and unfolding illness gradually becoming persistent and overwhelming. Finally, a diagnosis of FM is arrived at, but how the situation will evolve is uncertain.

Finally, a diagnosis of FM is arrived at, but how the situation will evolve is uncertain.The backward double integration method uses one force plate and could calculate jump height for countermovement jumping, squat jumping and drop jumping by analysing the landing phase instead of the push-off phase. This study compared the accuracy and variability of the forward double integration (FDI), backwards double integration (BDI) and Flight Time + Constant (FT+C) methods, against the marker-based rigid-body modelling method. It was hypothesised that the jump height calculated using the BDI method would be equivalent to the FDI method, while the FT+C method would have reduced accuracy and increased variability during sub-maximal jumping compared to maximal jumping. Twenty-four volunteers performed five maximal and five sub-maximal countermovement jumps, while force plate and motion capture data were collected. The BDI method calculated equivalent mean jump heights compared to the FDI method, with only slightly higher variability (2-3 mm), and therefore can be used in situations where FDI cannot be employed. The FT+C method was able to account for reduced heel-lift distance, despite employing an anthropometrically scaled heel-lift constant. However, across both sub-maximal and maximal jumping, it had increased variability (1.1 cm) compared to FDI and BDI and should not be used when alternate methods are available.
Myringotomy with trans-tympanic T-tube (TTT) placement is the most common surgical procedure performed in children. Finding a technique that provides longer aeration periods, lower complications rates, and less need for subsequent interventions will help the healthcare system from preventable events.

To compare the efficiency and complications rates of the traditional TTT to those of the novel postero-superior tubes in a grooved bone (TGB) for long-term middle ear ventilation in children suffering from recurrent acute otitis media (RAOM), or chronic otitis media with effusion (COME).

A total of 200 pretreated ears (96 by TGB and 104 by TTT), were examined after at least 3 years. Data concerning tube condition, and ear outcomes were collected. Pre- and post-operative audiograms were also performed.

TGB remained in place for longer periods, and it resulted in significantly lower rates of persistent tympanic membrane (TM) perforation (0% vs. 30.8%), TM retraction (14.6% vs. 32.7%), myringosclerosis (12.5% vs. 38.4%), otorrhea (29.2% vs. 52.9%), and need for topical antibiotics (31.3% vs. 51%). It has also resulted in more significant reduction in the air-bone gap on audiograms.

Pathologically, these disorders are characterised by disease-specific protein aggregates in neurons and/or glia and referred to as proteinopathies. Many neurodegenerative diseases show pathological overlap with the same abnormally deposited protein occurring in anatomically distinct regions, which give rise to specific patterns of cognitive and motor clinical phenotypes. Sequential distribution patterns of protein inclusions throughout the brain have been described. Rather than occurring in isolation, it is increasingly recognised that combinations of one or more proteinopathies with or without cerebrovascular disease frequently occur in individuals with neurodegenerative diseases. In addition, complex constellations of ageing-related and incidental pathologies associated with tau, TDP-43, Aβ, α-synuclein deposition have been commonly reported in longitudinal ageing studies. This review provides an overview of current classification of neurodegenerative and age-related pathologies and presents the spectrum and complexity of mixed pathologies in community-based, longitudinal ageing studies, in major proteinopathies, and genetic conditions. Mixed pathologies are commonly reported in individuals >65 years with and without cognitive impairment; however, they are increasingly recognised in younger individuals ( less then 65 years). Mixed pathologies are thought to lower the threshold for developing cognitive impairment and dementia. Hereditary neurodegenerative diseases also show a diverse range of mixed pathologies beyond the proteinopathy primarily linked to the genetic abnormality. Cases with mixed pathologies might show a different clinical course, which has prognostic relevance and obvious implications for biomarker and therapy development, and stratifying patients for clinical trials.
