Five new oleanane-type triterpenoid saponins (1-5), together with 24 known saponins (6-29) were isolated from the fruit of Acanthopanax senticosus. Their structures were determined by extensive spectroscopic analysis, including 1D, 2D nuclear magnetic resonance (NMR), and high-resolution electrospray ionization mass spectrometry (HR-ESI-MS), in combination with chemical methods (acid hydrolysis). The neuroinflammation model was established by lipopolysaccharide (LPS)-induced BV2 microglia, and the neuroprotective effects of all compounds (1-29) were evaluated.
Although the importance of educational programs in the prevention and control of diabetes mellitus (DM) and its complication is well-recognized, there are concerns about whether these programs are achieving the desired goal of increasing knowledge of DM and its complication in developing countries. Therefore, this study assessed knowledge of DM complications and associated factors among type-2 diabetic patients in public hospitals of Addis Ababa.

Simple random sampling technique was used to select 422 participants. Data were entered to EpiData Version 4.6.0.1 and analyzed using SPSS Version 25 software. Multicollinearity and model goodness-of-fit was checked. A multivariate logistic regression model at 95% CI was used to identify the predictors.

The overall knowledge of diabetes complications among diabetic patients in the Hospitals of Addis Ababa was 54.9%. In the fitted model, being a profession of governmental workers [adjusted odds ratio (AOR) = 3.12, 95% CI (1.33, 7.34)] and merchants [AOR = 2.54, t diabetes complications for patients who are more than 5 years since diagnosis and attention is needed about knowledge diabetic farmers.Low- and middle-income countries (LMICs) endure an asymmetrically high burden of worldwide disease and death caused by chronic respiratory diseases (CRDs), i.e., asthma, emphysema, bronchiectasis, and post-tuberculosis lung disease (PTLD). CRDs are firmly related with indigence, infectious diseases, and other non-communicable diseases (NCDs) and add to complex multi-disease with great impact on the lives and livelihood of those affected. The pertinence of CRDs to health and demographic wellbeing is relied upon to increment in the long time ahead, as expectations of life rise and the contending dangers of right on time youth mortality and irresistible infections level. The WHO has distinguished the counteraction and control of NCDs as an earnest improvement issue and crucial for the sustainable development goals (SDSs) by 2030. In this review, we center on CRDs in LMICs. We examine the early life roots of CRDs, challenges in their avoidance, identification and administration in LMICs, and the pathways to resolve for accomplish valid widespread wellbeing inclusion. The association between hunger and adherence to antiretroviral therapy (ART) is less known especially in vulnerable populations receiving HIV care and treatment services. Caregivers of orphans and vulnerable children (OVC) are vulnerable and likely to experience hunger due to additional economic pressure in caring for OVC. Using data from the community-based, USAID-funded Kizazi Kipya project, this study assesses the association between hunger and ART adherence among caregivers of OVC in Tanzania. HIV positive caregivers enrolled in the project from January to July 2017 were analyzed. The outcome variable was adherence to ART, defined as "not having missed any ART dose in the last 30 days," and household hunger, measured using the Household Hunger Scale (HHS), was the main independent variable. Data analysis included multivariable logistic regression. The study analyzed 11,713 HIV positive caregivers who were on ART at the time of enrollment in the USAID Kizazi Kipya project in 2017. Aged 48.2 years on average, 72.9% of the caregivers were female. While 34.6% were in households with little to no hunger, 59.4 and 6.0% were in moderate hunger and severe hunger households, respectively. Overall, 90.0% of the caregivers did not miss any ART dose in the last 30 days. ART adherence rates declined as household hunger increased (p less then 0.001). Multivariable analysis showed that the odds of adhering to ART was significantly lower by 42% among caregivers in moderate hunger households than those in little to no hunger households (OR = 0.58, 95% CI 0.50-0.68). The decline increased to 47% among those in severe hunger households (OR = 0.53, 95% CI 0.41-0.69). Hunger is an independent and a significant barrier to ART adherence among caregivers LHIV in Tanzania. Improving access to adequate food as part of HIV care and treatment services is likely to improve ART adherence in this population.Bike share engages riders in physical activity, beneficial to health. In addition, it promotes green mobility and contributes to carbon neutrality. An understanding of the association between bike share usage and the built environment is essential for system operation/management and urban/transportation planning. Limited reviews of the existing literature exist now. Therefore, we review previous studies to decipher the complex relationship between bike share usage and the built environment. We focus on a few built environment dimensions (e.g., land use, transportation system, and urban design) and find that many attributes affect bike share usage differently across mobility cultures, between docked and dockless bike share, and between arrival and departure usage patterns. The effects of several attributes (e.g., proximity to a park or university and metro station density) on bike share usage also vary between weekdays and weekends and across trip purposes. The findings of this paper advance knowledge on the connection between bike share usage and the built environment.Large-scale public buildings (e.g., stadiums and comprehensive hospitals) in modern cities provide places for various social activities. However, all of these public places encounter the scenario of large passenger flow and crowd gathering, which is highly likely to induce serious safety problems, such as stampedes. Previous studies have shown that efficient evacuation is an important way to ensure the safety of dense crowds in public places. This study aims to explore the optimization methods to improve the evacuation efficiency of public buildings. Two strategies considering plane partition and multi-floor layout are proposed for plane evacuation and vertical evacuation, respectively. Simulation scenarios and models of large stadiums and high-rise hospitals are established to verify the strategies. The results show that plane partition could effectively shorten the total evacuation time, which is due to the optimization of the initial exit choice of individuals and the avoidance of regional congestion in some evacuation channels or exits. Multi-floor layout optimization is an effective management method to arrange the different features of different floors, which could improve the evacuation efficiency for the whole multi-floor building. This study is helpful for building designers and managers to improve the building space layout design and the daily safety management mode.
Approximately 20~30% of all traffic accidents are caused by fatigue driving. However, limited practicability remains a barrier for the real application of available techniques to detect driving fatigue. Use of pupillary light reflex (PLR) may be potentially effective for driving fatigue detection.

A 90 min monotonous simulated driving task was utilized to induce driving fatigue. During the task, PLR measurements were performed at baseline and at an interval of 30 min. Subjective rating scales, heart rate variability (HRV) were monitored simultaneously.

Thirty-two healthy volunteers in China participated in our study. Based on the results of subjective evaluation and behavioral performances, driving fatigue was verified to be successfully induced by a simulated driving task. Significant variations of PLR and HRV parameters were observed, which also showed significant relevance with the change in Karolinska Sleepiness Scale at several timepoints (|
| = 0.55 ~ 0.72,
< 0.001). Furthermore, PLR variations had excellent ability to detect driving fatigue with high sensitivity and specificity, of which maximum constriction velocity variations achieved a sensitivity of 85.00% and specificity of 72.34% for driving fatigue detection, vs. 82.50 and 78.72% with a combination of HRV variations, a nonsignificant difference (AUC = 0.835, 0.872,
> 0.05).

Pupillary light reflex variation may be a potential indicator in the detection of driving fatigue, achieving a comparative performance compared with the combination with heart rate variability. Further work may be involved in developing a commercialized driving fatigue detection system based on pupillary parameters.

Health interventions may differently impact adolescents from diverse backgrounds. This study examined whether a smoking preventive intervention was equally effective in preventing cigarette smoking and use of alternative tobacco products (ATPs, i.e., snus, e-cigarettes, and waterpipe) among students from different socioeconomic backgrounds, i.e., occupational social classes (OSC).

Data was from the school-based intervention XIT II targeting 13- to 15-year-olds Danes. The intervention focused on three main components smoke-free school time, smoke-free curriculum, and parental involvement. In total, 46 schools were included at baseline (
= 2,307, response rate = 86.3%). Using a difference-in-differences approach, changes in current smoking and ever use of ATPs were estimated among students in high versus low OSC at second follow-up. Analyses were based on available cases (
= 826) and multiple imputations of missing data at the second follow-up (
= 1,965).

At baseline (age 13), 1.0% of students from eased. Therefore, besides focusing on narrowing the social disparities in cigarette smoking, future efforts may, to a larger extent, focus on adolescents' use of ATPs.The rapid spread of COVID-19 worldwide makes an uncertain impact on the development of digital finance in China. In this background, the measurement of digital financial risk and analysis of influence factor become the focus of the financial field. Therefore, this article builds the indicator system of digital financial risk and uses the Lagrange multiplier method to obtain the optimal comprehensive weight of AHP and entropy weight. Then, this article measures the digital financial risk indexes of China's major regions with high-level economic development from 2013 to 2020. Furthermore, the maximum likelihood estimates of the unknown parameters of skew-normal panel data model are obtained based on the EM algorithm, and the comparative study of the normal and skew-normal panel data models is conducted under AIC and BIC. Finally, based on the result of the model, the influence factors of digital financial risk of China's economically developed regions under COVID-19 are analyzed to provide data support for the prevention and governance of digital financial risk.
