The aim of this retrospective radiographic cross-sectional study was to assess the prevalence and severity of ectopically erupting FPMs in children attending the Dental University Teaching Hospital (DUTH) at King Saud University, Riyadh, Saudi Arabia.

This retrospective study was conducted using panoramic radiographs (OPGs) of 2014 patients (973 females and 1041 males) aged from 5 to 8 years old who attended DUTH at King Saud University, Riyadh, Saudi Arabia. We assessed and recorded the age and gender of the subjects, the tooth number, arch, and location of the ectopic erupted FPMs, and the severity of resorption of the roots of the primary molars.

Of 2014 reviewed OPGs, 45 (2.2%) cases were diagnosed with an ectopic eruption of the permanent first molar. The distribution of ectopic eruption based on gender showed that there was a higher prevalence in males 28 (2.9%) than in females 17 (1.6%). Of the 45 ectopic FPMs cases, 47 (78.33%) were detected in the maxilla, while 13 (21.66%) were seen in the mandible. Severe and moderate degrees of the ectopic eruption were found to be more common in the maxilla than in the mandible (p < 0.001).

Clinicians should be aware of such conditions, as early diagnosis is essential for providing optimum dental care. To our knowledge, this study is the first to assess the prevalence of ectopically erupting FPMs in a Saudi population.

To our knowledge, this study is the first to assess the prevalence of ectopically erupting FPMs in a Saudi population.Thoracic epidural analgesia (TEA) is a standard procedure in multimodal analgesia applied in major thoracic and abdominal surgeries. Two cases are presented with serious complications related to TEA. In both cases, earlier reaction of the treating physicians to patient-reported sensory symptoms could have prevented the complicated course. The first case was a 73-year-old patient with bronchial carcinoma who underwent right lower lobe resection. In this case, dabigatran 150 mg/d (indication permanent atrial fibrillation) had been discontinued 72 hours before surgery, and enoxaparin 80 mg (every 12 hours) had been started 11 hours after surgery. An epidural hematoma developed postoperatively. Magnetic resonance imaging (MRI) was performed only after paraplegia had developed the next day. Unfortunately, delayed hematoma evacuation could not prevent persistent paraplegia in this case, which was complicated by hospital-acquired pneumonia with sepsis and acute renal failure. The second case was a 39-year-old patient with ulcerative colitis and an initially undetected malposition of the epidural catheter. Immediately after test bolus injection, the patient reported paresthesia and overall discomfort, which however could not be safely attributed to either the test dose or the already started general anesthesia. The patient could only be extubated after stopping the epidural infusion. Accidental re-start of epidural infusion led to coma, conjugate eye deviation, and respiratory arrest, necessitating re-intubation. Computed tomography (CT) ruled out intracerebral pathology and showed a catheter position centrally in the spinal canal. Fortunately, no neurological deficits were detected after catheter removal.
Fibromyalgia is a chronic condition characterized by widespread pain and interference with daily activities. The aim of this study is to assess the benefit of transcutaneous electrical nerve stimulation (TENS) for persons diagnosed with fibromyalgia.

Adults meeting diagnostic criteria for fibromyalgia were randomized in a double-blind trial to receive either an active (n=62) or sham (n=57) wearable TENS device for 3-months. Subjects were classified as having lower or higher pain sensitivity by Quantitative Sensory Testing (QST). Patient Global Impression of Change (PGIC, primary outcome) and secondary efficacy measures including Fibromyalgia Impact Questionnaire (FIQR), Brief Pain Inventory (BPI) and painDETECT questionnaire (PDQ) were assessed at baseline, 6-weeks and 3-months. Treatment effects were determined by a mixed model for repeated measures (MMRM) analysis of the intention-to-treat (ITT) population (N=119). A pre-specified subgroup analysis of pain sensitivity was conducted using an interaction on for people with fibromyalgia.

Opioid-induced constipation (OIC) remains an important clinical obstacle despite the availability of several guidelines and pharmacological options for its management. Here, we surveyed common practices and perceptions about OIC among physicians who prescribe opioids in Italy.

The online survey included 26 questions about OIC. Responses were analyzed descriptively and aggregated by physician specialty.

A total of 501 physicians completed the survey. Most respondents (67%) did not feel adequately educated about OIC despite general consensus regarding interest in the topic. Overall, 62-75% of physicians regularly evaluated intestinal function or OIC symptoms in patients receiving opioid therapy. The most common method for assessment was patient diary; few physicians used a validated instrument such as the Rome IV criteria. Psychiatrists and addiction specialists showed the lowest interest and poorest practices. Most respondents (78%) preferred macrogol prophylaxis followed by macrogol plus another laxative for first-line treatment of OIC symptoms. Peripheral-acting mu opioid receptor antagonists (PAMORAs) were not widely used among physicians; 61% had never prescribed a PAMORA for OIC.

Our findings reveal important differences in clinical practice for OIC across physician specialties. Additional formative efforts are necessary to improve awareness about best practices in OIC.

The research on the association between coronavirus disease 2019 (COVID-19) and cardiovascular disease (CVD) is still insufficient.

This study aimed to investigate the association between neutrophil/lymphocyte ratio (NLR) and risk of cardiovascular events in patients with COVID-19.

Our study included 159 patients with COVID-19 who were measured for NLR value within the first 24 hours of admission. They were followed up for 6 months after discharge and then the relationship between levels of NLR and risk of cardiovascular events was assessed.

In all included patients with COVID-19, NLR values in patients with cardiovascular events [16.28 (4.95-45.18)] were significantly higher than patients without cardiovascular events [4.75 (2.60-7.47)]. find more A multivariate logistic regression model revealed that elevated NLR value [increased per SD, 2.41 (1.43-4.29),
<0.001; increased 1 of NLR, 2.05 (1.33-4.01),
=0.010] was significantly and independently associated with increased risk of CVD history on admission after adjustment of related confounding factors. Then, Cox regression analysis revealed that elevated NLR value had a significant association with increased risk of cardiovascular events [increased per SD, 2.36 (1.42-4.36),

A scoping review was conducted using predefined search terms in three scientific databases, including Google Scholar, Science Direct, and PubMed. The search for studies was restricted to original research articles that reported risk assessment models published in the last thirteen years (between 2008 and 2021) to cover the most recently published evidence following the development of the principal risk assessment score in 2008. In this review, we present data from literature that supports the promising role of vitamin D in treatment and/or prevention of several infections including severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). This review summarizes vitamin D metabolism and its role in inflammation, thrombosis and immune regulation. It also reviews, in short, the role of vitamin D and the impact of its deficiency in several infections namely tuberculosis, influenza, human immunodeficiency virus (HIV), and SARS-CoV-2.
