The COVID-19 pandemic has dramatically changed health care, forcing providers to adopt and implement telehealth technology to provide continuous care for their patients. Amid this rapid transition from in-person to remote visits, differences in telehealth utilization have arisen among neurosurgical subspecialties. In this study, we analyze the impact of telehealth on neurosurgical healthcare delivery during the COVID-19 pandemic at our institution and highlight differences in telehealth utilization across different neurosurgical subspecialties.

To quantify differences in telehealth utilization, we analyzed all outpatient neurosurgery visits at a single academic institution. Internal surveys were administered to neurosurgeons and to patients to determine both physician and patient satisfaction with telehealth visits. Patient Likelihood-to-Recommend Press-Ganey scores were also evaluated.

There was a decrease in outpatient visits during the COVID-19 pandemic in all neurosurgical subspecialties. TelehealthTelehealth visits will likely continue after the pandemic as they can improve clinical efficiency; overall, both patients and physicians are satisfied with health care delivery over video.
Present guidelines on reducing aerosol generation during neurosurgical procedures are futile. The aim of this article was to describe a novel device to contain aerosol within a small localized environment around the operative field-the negative pressure assisted microenvironment surgical hood (NEPA-MESH).

This device can be assembled using easily available materials-steel wires, image intensifier cover, surgical drape, and three-dimensional-printed self-locking copolyester double hoops. Large-bore pipes in continuity with a high-volume suction apparatus create a constant negative pressure microenvironment around the operative field. The CEM DT-9880 particle counter was used to estimate particle concentration inside the NEPA-MESH during various stages of a neurosurgical procedure as well as outside. The NEPA-MESH was tested in different craniotomies and endoscopic procedures.

Mean particle concentration inside the NEPA-MESH and outside during drilling in various procedures was calculated and compared using unpaired t test. Significant reduction in particle concentrations was recorded for particles sized 0.3 μm (t= 17.55, P < 0.0001), 0.5 μm (t= 11.39, P < 0.0001), 1 μm (t= 6.36, P= 0.0002), 2.5 μm (t= 2.04, P= 0.074), 5.0 μm (t= 7.026, P= 0.0008), and 10 μm (t= 4.39, P= 0.0023).

Computer-assisted navigation (CAN) has been shown to improve accuracy of screw placement in procedures involving the posterior cervical spine, but whether the addition of CAN affects complication rates, neurologic or otherwise, is presently unknown. The objective of this study is to determine the effect of spinal CAN on short-term clinical outcomes following posterior cervical fusion.

The American College of Surgeons National Surgical Quality Improvement Program database was queried from 2011 to 2018. Patients receiving posterior cervical fusion were identified and separated into CAN and non-CAN cohorts on the basis of a propensity score matching algorithm to select similar patients for comparison. Rates of 30-day unplanned readmission, reoperation, and other complications were evaluated. A separate matching algorithm was used to generate a subgroup of patients undergoing C1-C2 or occiput-C2 fusion for comparison of the same outcomes.

A total of 12,578 patients met inclusion criteria, of which 689 received CAN and 11,889 did not. After adjusting for baseline differences, patients receiving CAN experienced longer operations and had higher total relative value units associated with care. selleck inhibitor There were no significant differences in 30-day complication, readmission, or revision rates.
