Chronic spontaneous urticaria (CSU) is characterized by the presence of wheals, angioedema, or both for at least 6 weeks. It may persist for a long time-up to 50% of the patients have been reported to be symptomatic 5 years after the onset. Some patients can suffer more than one episode of CSU during their lifetime. Considering the recurrences, disabling symptoms, and significant impact on quality of life, proper and effective treatment of CSU is critical. However, the partial response and lack of response to omalizumab in a subgroup of patients, as high as 70% in some studies, make the development of alternative treatments desirable. Ever-increasing knowledge on the pathogenesis is making new target molecules available and enabling drug development for CSU. In addition to drug repurposing as in anti-IL-4/13, IL-5, and IL-17 antibodies, novel targeted therapy options such as ligelizumab and Bruton's tyrosine kinase inhibitors are currently undergoing clinical trials and will be available in the near future. This article reviews the current challenges in the treatment of CSU, the pathogenesis and potential target molecules, and the rationale for novel treatments and their rapidly developing status.Violence and aggression are common problems encountered in prison, which frequently require clinical intervention. This increased prevalence is partially attributable to the high morbidity of psychiatric and personality disorders in prison inmates. As prisons are non-therapeutic environments, the provision of clinical care becomes more complex. This article examines the general principles of management of violence and aggression in prison settings, with a particular focus on the clinical and ethical considerations that guide pharmacological approaches. Use of psychotropic medication to address these problems is reserved for situations where there is (i) a diagnosable psychiatric disorder, or (ii) a significant risk of harm to an individual without urgent intervention. Initial focus should be on environmental and behavioural de-escalation strategies. Clear assessment for the presence of major mental illness is crucial, with appropriate pharmacological interventions being targeted and time-limited. Optimising management of any underlying psychiatric conditions is an important preventative measure. In the acute setting, rapid tranquilisation should be performed according to local guidelines with a focus on oral prior to parenteral administration. Clinicians must be mindful of capacity and consent issues amongst prisoners to protect patient rights and guide setting of care.Photodynamic therapy employs nontoxic dyes called photosensitizers (PS) that are excited by visible light of the correct wavelength to produce a variety of reactive oxygen species (ROS) by an interaction between the long-lived PS triplet states with ambient oxygen. The most important type of ROS in photodynamic therapy (PDT) is singlet oxygen, which is produced by a Type II energy transfer process. On the other hand, superoxide, hydrogen peroxide, and hydroxyl radicals can be produced by a Type I electron transfer process. This chapter describes a set of fluorescent probes that can be used to tease apart these different ROS produced when various PS are illuminated in solution. Singlet oxygen sensor green (SOSG) is used for singlet oxygen, 4-hydroxyphenyl-fluorescein (HPF) for hydroxyl radicals, Amplex Red for hydrogen peroxide, and nitroblue-tetrazolium or XTT for superoxide.The assessment of reactive oxygen species has increasing importance in biomedical sciences, due to their biological role in signaling pathways and induction of cell damage at low and high concentrations, respectively. Detection of lipid peroxidation with sensing probes such as some BODIPY dyes has now wide application in studies using fluorescent microplate readers, flow cytometry, and fluorescence microscopy. Two phenylbutadiene derivatives of BODIPY are commonly used as peroxidation probes, non-oxidized probes and oxidized products giving red and green fluorescence, respectively. Peculiar features of lipoperoxidation and BODIPY dye properties make this assessment a rather complex process, not exempt of doubts and troubles. Color changes and fluorescence fading that are not due to lipid peroxidation must be taken into account to avoid misleading results. As a characteristic feature of lipoperoxidation is the propagation of peroxyl radicals, pitfalls and advantages of a delayed detection by BODIPY probes should be considered.The most common way to demonstrate the reactive oxygen species (ROS)-mediated pathways in photodynamic therapy (PDT) and in sonodynamic therapy (SDT) is the use of specific ROS inhibitors. We present a general method to establish the relative efficiency of different sonosensitizers which produce the same ROS. To demonstrate it, we use peroxides as sonosensitizers which produce singlet molecular oxygen. The method is easily generalized by all types of ROS.Singlet oxygen (1O2) is the first electronic excited state of molecular oxygen. Due to its non-radical and non-ionic character as well as its mild reactivity, 1O2 has a pivotal role in cell signaling processes at low concentration, yet it is cytotoxic at high concentrations. Quantifying the production of 1O2, particularly in biological systems, is therefore essential for understanding and controlling its effects. 1O2 can be produced by chemical and biological reactions, yet its most common method of production is by photosensitization, whereby an initially photoexcited molecule transfers its acquired electronic energy to the dioxygen molecule. The efficiency of this process is characterized by the 1O2 production quantum yield, ΦΔ, which can be determined by directly monitoring its intrinsic weak near-infrared phosphorescence or indirectly by trapping it with a suitable acceptor, a process that can be monitored by common analytical techniques. selleck chemicals Indirect methods are thus very popular, yet they may lead to severe errors if used incorrectly.
