The 2022 European LeukemiaNet system categorizes all acute myeloid leukemia (AML) cases with FLT3-ITD mutations as intermediate risk, irrespective of their FLT3-ITD allelic ratio or concurrent NPM1 mutation status. While the potential of other next-generation sequencing (NGS) computational genes to further complicate FLT3-ITD mutations, and the possibility of overcoming the poor prognoses linked to FLT3-ITD mutations via haploidentical hematopoietic stem cell transplantation (haplo-HSCT) are uncertain. This study sought to explore how NGS-based computations at diagnosis influence the clinical outcome of de novo AML patients harboring FLT3-ITD mutations, along with the impact of haplo-HSCT on these concomitant genetic alterations. Employing the NGS 99-gene platform, we investigated 95 de novo AML patients carrying FLT3-ITD mutations, documented between January 2018 and August 2021. The examination detected forty-one further types of molecular mutations. NPM1 (n = 43, 453%) and DNMT3A (n = 21, 221%) mutations stood out as the most frequently co-occurring mutations. The NPM1 mutation's presence did not correlate with any change in clinical outcomes. Patients with acute myeloid leukemia, harboring concurrent FLT3-ITD and DNMT3A mutations, experienced a considerably poorer 3-year disease-free survival (DFS) and overall survival (OS) compared to those without DNMT3A mutations. The DFS rate for the former group was 49.5% versus 69.3% (P=0.001), and OS rates were 61.1% versus 69.8% (P=0.054). Survival outcomes were demonstrably more positive following haplo-HSCT than after chemotherapy. Three-year DFS rates were 85.7% versus 30.8% (P=0.0006) and 3-year OS rates were 85.7% versus 43.1% (P=0.008) in the respective groups. In multivariate analyses, a mutation in the DNMT3A gene was a predictor of diminished disease-free survival, whereas haploidentical hematopoietic stem cell transplantation proved to be a protective measure. Overall, DNMT3A mutations could potentially predict a less favorable outcome in adult acute myeloid leukemia patients with co-occurring FLT3-ITD mutations, and haploidentical hematopoietic stem cell transplantation may successfully ameliorate the detrimental prognostic implications of DNMT3A mutations.

The CTS treatment demonstrably elevated the incorporation rate of FTD into the DNA of HGC-27 cells.
