The research investigates dosimetric alterations during the first month following GammaTile surgically targeted radiation therapy (STaRT) for gliomas, attributing these alterations to Cesium-131 (Cs-131) seed displacement related to cavity shrinkage during brain brachytherapy. After maximal safe resection during craniotomy procedures, 10 glioma patients in this study received GammaTiles, with 4 to 11 tiles placed along the resection bed. A biodegradable collagen sponge, strategically housing four Cs-131 seeds, forms the foundation of each GammaTile, reducing seed movement and upholding the proper seed-to-cavity distance. Utilizing VariSeed on the first day, the Cs-131 seed positions were determined. Calculations of dosimetry parameters and post-implant computed tomography (CT) images were performed on the 30th day. Employing an iterative closest point (ICP) algorithm, the rigid transformation between the seed clouds from day one and day thirty was computed. After the registration procedure was completed, the seed displacement was calculated. Both CT images were used to assess dosimetric variations due to seed displacement. These calculations comprised the volume absorbing 100% of the prescription dose (V100), the dose received by 90% of the planning target volume (D90 PTV), the planning target volume absorbing 100% of the prescribed dose (V100 PTV), and the dose delivered to vulnerable organs (OARs). The study revealed an average seed displacement of 18.10 mm in all patients, measured over the course of 30 days, starting from the first day. The maximum seed displacement, across all patients, was observed to fluctuate between 23 mm and 73 mm. Comparing V100 levels on day one and day 30, the average change was 25 cubic centimeters, with a variation spanning from 05 to 65 cubic centimeters. On the initial day, the average D90 PTVs were 955% (690%–1310%), increasing to 981% (199%–1490%) by day 30. Observing the mean V100 PTV values, day one measurements averaged 884%, fluctuating from 813% to 991%. Day thirty measurements showed an average of 879% with a range of 470% to 997%. On the first day of treatment, the brainstem dose for the first patient was 635 Gy, whereas the second patient received 281 Gy; the following month, the brainstem doses had decreased to 558 Gy and 206 Gy, respectively, translating to brainstem dose reductions of 77 Gy (128%) for the first patient and 75 Gy (125%) for the second patient. The optic nerves of only two patients received a dose, one receiving 341 Gy and the other 52 Gy. The optic nerves experienced dose discrepancies of 18 Gy and 5 Gy, respectively, as determined by comparing dose calculations from day one and day thirty CT images. Identical to the other, two patients simultaneously had radiation treatment to their chiasm; one receiving 304 Gy, and the other 68 Gy. A difference in the dosage (11 Gy) given to the chiasm was recorded between day one and day thirty. Within the first month post-GammaTile STaRT glioma treatment, cavity shrinkage caused a maximum seed displacement of 73 mm and an average seed displacement of 18 mm. The dosimetry parameters demonstrated notable differences. Dose modifications to organs at risk, particularly the brainstem, displayed substantial changes, with some instances amounting to a 128% increase from the prescribed dose. The evaluation of treatment results and the preparation for subsequent GammaTile treatments hinge on the understanding of these changes in dosimetry.
