Regioselective Reduction of 1H-1,Two,3-Triazole Diesters.
© 2020 Published by Elsevier Ltd.Paper/plastic sterilization pouches are commonly used packaging material for steam sterilization. Reuse of these pouches is a general practice in Thailand despite a single-use recommendation. This study aimed to determine microbial contamination after reusing paper/plastic sterilization pouches in a dental clinic and storage in a closed environment for 6 months. Three hundred and twenty pouches underwent 3 times of clinical use in terms of packaging, autoclave sterilization, handling, and unpacking. A mouth mirror was packed in each pouch to be used in a clinic. After each use, a pouch would be carefully inspected for reusability and undergone packaging, sterilization, handling again. In all steps, sterilization monitoring was rigorously applied. After 3 times of use, a piece of filter paper was placed inside each pouch (instead of a mouth mirror), the pouch was autoclaved and stored in a closed environment for 6 months. Then the filter paper was retrieved for microbial cultivation. A negative control group comprised new pouches containing filter paper without storage and a positive control group comprised pouches with impaired integrity. All samples in both the reuse and the negative control groups had no microbial contamination. All samples in the positive control group showed contamination. These results suggested that reusing paper/plastic sterilization pouches could be a safe practice provided careful monitoring and inspection were employed. © 2020 The Author(s).Purpose Although it is a well-known disease, the occurrence of Herpes simplex encephalitis (HSE) during a hospital stay may render the diagnosis particularly challenging. The objective of this report is to alert clinicians about the diagnostic pitfalls arising from hospital-developed HSE. Materials and methods Clinical observation of one patient. Case report An 87-year-old male was admitted to the Intensive Care Unit (ICU) because of respiratory failure due to an exacerbation of myasthenia gravis. After corticoids and azathioprine treatment, his clinical condition improved, allowing weaning from mechanical ventilation. One month after admission, while still hospitalized in the ICU, the patient developed fever and confusion. In the context of confounding factors, HSE was not suspected before a convulsive status epilepticus occurred, resulting in a significant delay in treatment. Diagnosis was confirmed by PCR-analysis in the cerebrospinal fluid. Serological status confirmed reactivation of prior herpes simplex infection. Afuresertib in vivo The patient died one week after the onset of confusion. Conclusions Hospital-"acquired" HSE must be suspected in case of new neurologic symptoms associated with fever, even in ICU-hospitalized patients. The diagnosis is made even more difficult by nonspecific symptoms due to previous diseases, leading to an even more severe prognosis in those vulnerable patients. © 2020 Published by Elsevier Ltd.Platelet-rich-plasma (PRP) is an autologous human platelet concentrate extracted from plasma. PRP has been investigated in order to be used in many fields, with emphasis on the musculoskeletal field applied to sports injuries, as well as on other medical fields such as cardiac surgery, gynecology, pediatric surgery, urology, ophthalmology and plastic surgery. Cancer treatment is another important field where PRP should be investigated; thus, it is important validating PRP preparation protocols to be used in clinical research. Many protocols should be revised since, overall, most studies do not provide necessary information to allow them to be multiplied or replicated. The current review focuses on several topics about cancer, mainly on innovative studies about PRP use as a feasible therapeutic alternative to treat bladder cancer - a field where it could play a key role. Relevant aspects such as platelets' contribution to immune regulation and the supportive role they play in innate and adaptive immune functions are also addressed. Another important topic reviewed in the current study refers to inflammatory process regulation associated with cancer and thrombosis sites, which indicated that tumor-induced platelet activation could be used as an important therapeutic target in the future. New aspects concerning nitric oxide's ability to restrain platelet adhesion and aggregation in order to slow metastasis progress in cancer patients provide an important advantage in cancer treatment. Finally, the current review has pointed out perspectives and the main concerns about, and possibilities of, PRP use in cancer treatment. © 2020 The Authors.Malaria and typhoid fever are revered for their ability to individually or jointly cause high mortality rate. Both malaria and typhoid fever have similar symptoms and are famous for their co-existence in the human body, hence, causes problem of under-diagnosis when doctors tries to determine the exact disease out of the two diseases. This paper proposes a Bioinformatics Based Decision Support System (BBDSS) for malaria, typhoid and malaria typhoid diagnosis. The system is a hybrid of expert system and global alignment with constant penalty. The architecture of the proposed system takes input diagnosis sequence and benchmark diagnosis sequences through the browser, store these diagnosis sequences in the Knowledge base and set up the IF-THEN rules guiding the diagnosis decisions for malaria, typhoid and malaria typhoid respectively. The matching engine component of the system receives as input the input sequence and applies global alignment technique with constant penalty for the matching between the input sequtions between the groups of malaria and malaria typhoid. Conversely, there were no differences between the groups of malaria and typhoid fever as well as between the groups of typhoid fever and malaria typhoid. In order to show mean difference in the diagnosis scores between the orthodox and the proposed diagnosis system, t-test statistics was used. The results of the t-test statistics indicates that the mean values of diagnosis from the orthodox system differ from those of the proposed system. Finally, the evaluation of the proposed diagnosis system is most efficient at providing diagnosis for malaria and malaria typhoid at 97% accuracy. © 2020 The Author(s).
