Stressful experiences evoke, among others, a rapid increase in brain (nor)epinephrine (NE) levels and a slower increase in glucocorticoid hormones (GCs) in the brain. Microglia are key regulators of neuronal function and contain receptors for NE and GCs. These brain cells may therefore potentially be involved in modulating stress effects on neuronal function and learning and memory. In this review, we discuss that stress induces (1) an increase in microglial numbers as well as (2) a shift toward a pro-inflammatory profile. These microglia have (3) impaired crosstalk with neurons and (4) disrupted glutamate signaling. Moreover, microglial immune responses after stress (5) alter the kynurenine pathway through metabolites that impair glutamatergic transmission. All these effects could be involved in the impairments in memory and in synaptic plasticity caused by (prolonged) stress, implicating microglia as a potential novel target in stress-related memory impairments.
The aim of the study was to explore the risk of levator ani avulsion and levator hiatus enlargement following vaginal birth after cesarean section (VBAC) in comparison to primiparous women after their first vaginal delivery.
In this multicenter observational cohort study we identified all women who had a term VBAC for their second delivery at the Departments of Gynecology and Obstetrics, Faculty of Medicine in Pilsen and the 1st Faculty of Medicine , Prague, Charles University, Czech Republic between 2012 and 2016. Women having a repeat VBAC, preterm birth or stillbirth were excluded from the study. As a control group, we enrolled a cohort of primiparous women who had their first vaginal birth during the studied period. To increase our control sample we also invited all primiparous women who delivered vaginally between May and June 2019 . All participants were invited for a 4D pelvic floor ultrasound to assess levator trauma. Levator avulsion and the levator hiatus area at rest and on maximal Valsalva were VBAC compared to the control group (32.6 vs 18.6 %, p=0.01). The difference was observed dominantly in unilateral avulsions and remained significant after controlling for age and BMI (adjusted OR 2.061; 95% CI1.103 - 3.852). There were no statistically significant differences in the size of levator hiatus at rest (12.0 vs. 12.6 cm
, p=0.28) or Valsalva (18.6 vs. 18.7 cm
, p=0.55). The incidence of levator ballooning was comparable in VBAC and Controls (17.7 % and 18.6 %, p=0.9).

Using an in vivo model of intratracheal lipopolysaccharide (IT LPS)-induced acute lung injury, we show that skeletal muscle rapidly responds with expression of proinflammatory genes, which may be explained by migration of LPS into the circulation. Treatment of mature C2C12 myotubes with LPS at a level achieved in the circulation following IT LPS elicited a proinflammatory cytokine expression profile similar to that of in vivo murine muscle following IT LPS. Stimulation with toll-like receptor (TLR) 2 and 3 agonists provoked comparable responses in C2C12 myotubes. Additionally, co-cultures of C2C12 myotubes and bone marrow-derived macrophages (BMDM) identified the capacity of macrophages to increase myotube proinflammatory gene expression, with tumor necrosis factor-α (TNFα) gene and protein expression largely attributable to BMDM. To investigate the contribution of TNFα in the synergy of the co-culture environment, C2C12 myotubes were treated with recombinant TNFα, co-cultures were established using TNF-deficient BMDM, and co-cultures were also depleted of TNFα using antibodies. To determine whether the in vitro observations were relevant in vivo, mice received intramuscular administration of LPS ± TNFα or TNFα-neutralizing antibodies and showed that TNFα is both sufficient and necessary to induce synergistic cytokine release from muscle. Taken together, these data demonstrate how skeletal muscle tissue may contribute proinflammatory cytokines following acute endotoxin injury and the potential of leukocytes to augment this response via TNFα secretion.
