The frontal aslant tract (FAT) is a white-matter tract connecting the inferior frontal gyrus (IFG) and the supplementary motor complex (SMC). Damage to either component of the network causes spontaneous speech dysfluency, indicating its critical role in language production. However, spontaneous speech dysfluency may stem from various lower-level linguistic deficits, precluding inferences about the nature of linguistic processing subserved by the IFG-SMC network. Since the IFG and the SMC are attributed a role in conceptual and lexical selection during language production, we hypothesized that these processes rely on the IFG-SMC connectivity via the FAT. We analysed the effects of FAT volume on conceptual and lexical selection measures following frontal lobe stroke. The measures were obtained from the sentence completion (SC) task, tapping into conceptual and lexical selection, and the picture-word interference (PWI) task, providing a more specific measure of lexical selection. Lower FAT volume was not associated with lower conceptual or lexical selection abilities in our patient cohort. Current findings stand in marked discrepancy with previous lesion and neuroimaging evidence for the joint contribution of the IFG and the SMC to lexical and conceptual selection. A plausible explanation reconciling this discrepancy is that the IFG-SMC connectivity via the FAT does contribute to conceptual and/or lexical selection but its disrupted function undergoes reorganisation over the course of post-stroke recovery. Thus, our negative findings stress the importance of testing the causal role of the FAT in lexical and conceptual lexical selection in patients with more acute frontal lobe lesions. The accumulating evidence suggests that prior usage of a second language (L2) leads to processing costs on the subsequent production of a native language (L1). However, it is unclear what mechanism underlies this effect. It has been proposed that the L1 cost reflects inhibition of L1 representation acting during L1 production; however, previous studies exploring this issue were inconclusive. It is also unsettled whether the mechanism operates on the whole-language level or is restricted to translation equivalents in the two languages. We report a study that allowed us to address both issues behaviorally with the use of ERPs while focusing on the consequences of using L2 on the production of L1. In our experiment, native speakers of Polish (L1) and learners of English (L2) named a set of pictures in L1 following a set of pictures in either L1 or L2. Half of the pictures were repeated from the preceding block and half were new; this enabled dissociation of the effects on the level of the whole language from those specific to individual lexical items. Our results are consistent with the notion that language after-effects operate at a whole-language level. Behaviorally, we observed a clear processing cost on the whole-language level and a small facilitation on the item-specific level. The whole-language effect was accompanied by an enhanced, fronto-centrally distributed negativity in the 250-350 ms time-window which we identified as the N300 (in contrast to previous research, which probably misidentified the effect as the N2), a component that presumably reflects retrieval difficulty of relevant language representations during picture naming. As such, unlike previous studies that reported N2 for naming pictures in L1 after L2 use, we propose that the reported ERPs (N300) indicate that prior usage of L2 hampers lexical access to names in L1. Based on the literature, the after-effects could be caused by L1 inhibition and/or L2 interference, but the ERPs so far have not been informative about the causal mechanism. During comprehension, language users not only immediately integrate information from local sentence context, but also information from global discourse context for full understanding. In the present study, we examined whether the time course of these integration processes is constrained by language users' working memory capacity. Sentence and discourse stimuli were constructed. For the sentence stimuli, each sentence contained a critical word that was either congruent or incongruent with its preceding sentence context. For the discourse stimuli, each discourse contained four sentences with a target word embedded at the final sentence and the target word was either congruent or incongruent with the information provided at the first sentence of the discourse. Participants of high and low working memory span were instructed to read for comprehension. PHA-665752 nmr Our results showed that while the high span readers showed the N400 and P600 effects to semantically incongruent words, the low span readers only showed the P600 effect. This pattern was found regardless of whether the incongruent words were placed at sentence or discourse context. These results suggest that the low span readers are relatively delayed than the high span ones at both sentence- and discourse-level semantic integration and indicate that working memory functions have greater influence than context scope on the time course of semantic integration. BACKGROUND Shared decision making (SDM) is the process through which patients and their medical provider mutually explore therapy goals, risks/benefits, and treatment options regarding medical care. Decision-aids are tools to help with the values clarification process and assess decisional needs and potential decisional conflicts. OBJECTIVE To develop and assess acceptability of a decision-aid for commercial peanut allergy therapies. METHODS The creation of this decision-aid occurred in three stages, including a qualitative study to assess decisional needs, development of a draft decision-aid through multiple iterations in accordance with international guidelines and decision-aid experts, and assessment of decisional acceptability, decisional conflict, and decisional self-efficacy related to using the decision-aid. RESULTS The decision-aid went through 9 iterations, resulting in a 4-page aid with 7 parts, explaining the therapies, the key risks and benefits of the therapy choices, the relative importance of key attributes of the therapies, and a self-check assessment regarding informational adequacy and how to take the next steps.
