Decline in skills and knowledge among patients and/or caregivers contributes to peritoneal-dialysis (PD)-related peritonitis. Re-training is important, but no guidelines exist. We describe the implementation of a structured re-training program to decrease peritonitis rates.

This is a prospective quality improvement study involving pediatric patients on long-term home automated PD at National University Hospital, Singapore, between 2012 and 2018. With increasing peritonitis rates, systematic root cause analysis was performed, and based on the contributory factors identified, a structured re-training program was implemented from 2015. This was conducted in 5 cycles, each consisting of 4modules (hand hygiene, exit site care, peritonitis, and PD troubleshooting).

Peritonitis rates were analyzed in 2 phases Phase 1 (2012-2014) when no re-training was performed and Phase 2 (2016-2018) after re-training was instituted. Fifty-nine patients were included. Of these, 45 patients were in Phase 1, 32 in Phase 2, and 18 in both phases. Peritonitis rates decreased from 0.37 ± 0.67 episodes per patient-year in Phase 1 to 0.13 ± 0.32 episodes per patient-year in Phase 2. After adjusting for age at kidney failure onset, PD vintage, years of nursing experience, and the average patient-to-nurse ratio over the study period for each patient, the adjusted peritonitis rates decreased by 0.38 episodes per patient-year (95% CI, 0.09 to 0.67, p = 0.011) from Phase 1 to Phase 2.

Various methods have been introduced to eliminate the epicanthus; however, there are no clear guidelines to determine the optimal technique for epicanthus. We aimed to investigate the clinical effect of modified Z-plasty and modified Y-V flap on epicanthus and report our experience.

Medical records, including photographic information and scar score, of 81 eyes of 43 patients with congenital epicanthus were collected. All patients underwent epicanthus correction surgery by the modified Z-plasty or modified Y-V flap methods with a four-point design from January 2018 to December 2019 in Shanghai Ninth People's hospital and completed a follow-up of at least 6 months. The cosmetic outcome was evaluated in terms of by photographic evaluation and scar score, and the structural outcome was assessed by intercanthal distance (ICD) and palpebral fissure length, inner canthal angle, the canthal tilt angle. The results of two surgical techniques were compared and analyzed.

Indeed, the increase in remote video interaction seems to have led to an increase in the number of requests for teleconsultations with plastic surgeons in private practice (70% in the UK and 64% in the USA), the so-called Zoom Boom. The reasons connected to this increase are still under investigation, but it is likely that patients' greater exposure to seeing their own image in online video communication for prolonged periods has highlighted what patients perceive as flaws and/or their self-image does not correspond to the same image that they see in the mirror. However, the motivations for seeking cosmetic surgery cannot be solely attributed to seeing ourselves online more frequently; the psychological impact of the pandemic (lower moods, isolation and greater concerns) may also be to blame. Therefore, the reasons patients hope to touch-up, rejuvenate, re-do or revise their "lockdown faces" need careful attention with a view to understand the extent of which the online distortion of one's own body image may be the underlying cause.
