The hypothalamic neuropeptide oxytocin (OT) is critically involved in the modulation of socio-emotional behavior, sexual competence, and pain perception and anticipation. While intracellular signaling of OT and its receptor (OTR), as well as the functional connectivity of hypothalamic and extra-hypothalamic OT projections, have been recently explored, it remains elusive how one single molecule has pleotropic effects from cell proliferation all the way to modulation of complex cognitive processes. Moreover, there are astonishing species-dependent differences in the way OT regulates various sensory modalities such as touch, olfaction, and vision, which can be explained by differences in OTR expression in brain regions processing sensory information. Recent research highlights a small subpopulation of OT-synthesizing cells, namely, parvocellular cells, which merely constitute 1% of the total number of OT cells but act as "master cells' that regulate the activity of the entire OT system. In this chapter, we summarize the latest advances in the field of OT research with a particular focus on differences between rodents, monkeys and humans and highlight the main differences between OT and its "sister" peptide arginine-vasopressin, which often exerts opposite effects on physiology and behavior.The arcuate nucleus (ARC) is located in the mediobasal hypothalamus and forms a morphological and functional entity with the median eminence (ME), the ARC-ME. The ARC comprises several distinct types of neurons controlling prolactin release, food intake, and metabolism as well as reproduction and onset of puberty. The ME lacks a blood-brain barrier and provides an entry for peripheral signals (nutrients, leptin, ghrelin). ARC neurons are adjacent to the wall of the third ventricle. This facilitates the exchange of signals from and to the cerebrospinal fluid. The ventricular wall is composed of tanycytes that serve different functions. Axons of ARC neurons contribute to the tuberoinfundibular tract terminating in the ME on the hypophysial portal vessels (HPV) and establish one of the neurohumoral links between the hypothalamus and the pituitary. ARC neurons are reciprocally connected with several other hypothalamic nuclei, the brainstem, and reward pathways. The hypophysial pars tuberalis (PT) is attached to the ME and the HPV. The PT, an important interface of the neuroendocrine system, is mandatory for the control of seasonal functions. This contribution provides an update of our knowledge about the ARC-ME complex and the PT which, inter alia, is needed to understand the pathophysiology of metabolic diseases and reproduction.Cerebrospinal fluid (CSF) disorders are challenging conditions in neurosurgical practice. The majority of CSF is contained in the basal cisterns of the brain, which are subarachnoid compartments that communicate with each other, and contribute to the circulation of CSF. Yaşargil et al. (1976) was the first to provide the systematic classification and naming of the basal cisterns. The lamina terminalis (LT) starts from the gyrus rectus and descends to the lateral aspect of the optic chiasm. It is a thick arachnoidal membrane delineating the anterior wall of the third ventricle that borders the LT cistern. With the introduction of the operating microscope and the progressive development of modern neurosurgery, the arachnoid and basal cisterns have been used as surgical corridors in order to reach deep areas of the brain and to release CSF for brain relaxation. see more In this way, the LT is used as a surgical corridor for the treatment of several conditions such as obstructive hydrocephalus and diencephalic tumors. In this chapter, we will describe the anatomy of the LT, possible conditions treated by opening the LT, the different surgical approaches to opening the LT, along with their advantages and disadvantages.In this chapter, we review the extensive literature describing the roles of the subfornical organ (SFO), the organum vasculosum of the terminalis (OVLT), and the median preoptic nucleus (MnPO), comprising the lamina terminalis, in cardiovascular regulation and the control of fluid balance. We present this information in the context of both historical and technological developments which can effectively be overlaid upon each other. We describe intrinsic anatomy and connectivity and then discuss early work which described how circulating angiotensin II acts at the SFO to stimulate drinking and increase blood pressure. Extensive studies using direct administration and lesion approaches to highlight the roles of all regions of the lamina terminalis are then discussed. At the cellular level we describe c-Fos and electrophysiological work, which has highlighted an extensive group of circulating hormones which appear to influence the activity of specific neurons in the SFO, OVLT, and MnPO. We highlight optogenetic studies that have begun to unravel the complexities of circuitries underlying physiological outcomes, especially those related to different components of drinking. Finally, we describe the somewhat limited human literature supporting conclusions that these structures play similar and potentially important roles in human physiology.Nearly a century ago it was reported that stimulation of the hypothalamus could evoke profound behavioral state changes coupled with altered autonomic function. Since these initial observations, further studies in animals have revealed that two hypothalamic regions-the dorsomedial and ventromedial hypothalamic nuclei-are critical for numerous behaviors, including those in response to psychological stressors. These behaviors are coupled with changes in autonomic functions, such as altered blood pressure, heart rate, sympathetic nerve activity, resetting of the baroreflex and changes in pituitary function. There is also growing evidence that these two hypothalamic regions play a critical role in thermogenesis, and suggestions they could also be responsible for the hypertension associated with obesity. The aim of this chapter is to review the anatomy, projection patterns, and function of the dorsomedial and ventromedial hypothalamus with a particular focus on their role in autonomic regulation. While most of what is known about these two hypothalamic regions is derived from laboratory animal experiments, recent human studies will also be explored.
