While regional brain alterations and functional connectivity in depressed suicidal patients have previously been reported, knowledge about gray matter (GM) structural networks is limited. The aim of this study was to explore the GM of depressed suicidal brains from the single-subject structural network level. This was a cross-sectional study, in which 50 healthy controls (HC, 31 ± 9 years), 50 major depressed patients without suicidality (NSD, 29 ± 10 years), and 50 major depressed patients with suicidality (SU, 29 ± 12 years) were enrolled. T1 -weighted images (T1 WI) were acquired with three-dimensional-magnetization prepared rapid gradient echo sequence in 3.0 T magnetic resonance. The analysis was performed using the automated Computational Anatomy Toolbox (CAT12) within Statistical Parametric Mapping while running MATLAB. The T1 images were segmented into GM, white matter, and cerebrospinal fluid. Then single-subject structural networks were constructed based on the morphological similarity of GM regionser small-worldness, compared to the NSD and HC groups. Abnormal nodal efficiency was found in the fronto-striatum-limbic-thalamic circuit in suicidal brains. This study provides new evidence for therapeutic targets for patients with depression and suicidality. LEVEL OF EVIDENCE 3 TECHNICAL EFFICACY STAGE 3.Freshwater unionid bivalves currently face severe anthropogenic challenges. Over 70% of species in the United States are threatened, endangered or extinct due to pollution, damming of waterways and overfishing. These species are notable for their unusual life history strategy, parasite-host co-evolution and biparental mitochondrial inheritance. Among this clade, the washboard mussel Megalonaias nervosa is one species that remains prevalent across the Southeastern United States, with robust population sizes. We have created a reference genome for M. nervosa to determine how genome content has evolved in the face of these widespread environmental challenges. We observe dynamic changes in genome content, with a burst of recent transposable element proliferation causing a 382 Mb expansion in genome content. Birth-death models suggest rapid expansions among gene families, with a mutation rate of 1.16 × 10-8 duplications per gene per generation. Cytochrome P450 gene families have experienced exceptional recent amplently facing widespread environmental challenges. This work has clear implications for conservation genomics on freshwater bivalves as well as evolutionary theory. This genome represents a first step to facilitate reverse ecological genomics in Unionidae and identify the genetic underpinnings of phenotypic diversity.General medical practitioners (GPs) and veterinarians have different but complementary knowledge and skills, with potential to enhance clinical management of zoonoses in human and animal patients through taking a One Health approach that promotes cross-professional collaboration. Ability and willingness to engage within this framework is contingent on knowledge of endemic zoonoses and an understanding of the diversity of professional roles; however, previous research suggests that this is lacking. A unique parallel survey of Australian GPs and veterinarians was implemented to ascertain clinician experience, concern, confidence and current practices regarding zoonoses management as well as willingness to engage in cross-professional collaboration where it is beneficial to overall health outcomes. Responses from 528 GPs and 605 veterinarians were analysed. Veterinarians in clinical practice were found to more frequently diagnose zoonoses; have greater concern about zoonoses; be more confident in diagnosing, managing and giving advice about the prevention of zoonoses; more likely to give advice about managing the risk of zoonoses; and more likely to initiate cross-professional referral compared to GPs (p less then .001 in all areas, adjusted for other factors). The findings of this study indicate a need for change in both clinical and continuing professional education, especially for GPs, in order to better equip them in the area of zoonoses management. Exploration of pathways to encourage and facilitate cross-professional referral and collaboration will further improve clinical outcomes for both human and animal patients.
Emergency department (ED) patients with unexplained syncope are at risk of experiencing an adverse event within 30days. Our objective was to systematically review the accuracy of multivariate risk stratification scores for identifying adult syncope patients at high and low risk of an adverse event over the next 30days.
We conducted a systematic review of electronic databases (MEDLINE, Cochrane, Embase, and CINAHL) from database creation until May 2020. We sought studies evaluating prediction scores of adults presenting to an ED with syncope. We included studies that followed patients for up to 30days to identify adverse events such as death, myocardial infarction, stroke, or cardiac surgery. We only included studies with a blinded comparison between baseline clinical features and adverse events. We calculated likelihood ratios and confidence intervals (CIs).
We screened 13,788 abstracts. We included 17 studies evaluating nine risk stratification scores on 24,234 patient visits, where 7.5% (95% CI= 5.3% admission rates, cost, or outcomes of care is not known.
