METHODS Systematic search of MEDLINE, Web of Science and Cochrane Library was conducted for all comparative studies from 2000 to 2019 with the limitations of the English language. RESULTS For a notable period of time, researchers have mainly focused on the therapeutic mechanisms of mir146a involved in the modulation of inflammatory and anti-inflammatory genes. We found that levels of mir146a expression were associated with cancer cell metastasis as a dual role (Inhibitory and stimulatory roles). The results of various studies also showed that this microRNA has a therapeutic role through its effects on other gene expressions such as NF-kB, SIRT1, TNF- α and IL-1β and leads to disease control. CONCLUSION Knowledge about alterations in mir146a regulation will give a better understanding of the molecular basis for various chronic inflammatory diseases and cancers. Calycosin is reportedly evidenced with pharmacologically treating bone cells. However, the comprehensive anti-osteosarcoma (OS) mechanisms of calycosin have not been uncovered. By using a systemic method of network pharmacology, the present study aimed to reveal potential anti-OS biotargets and molecular mechanisms played by calycosin. Moreover, human and animal experiments were conducted to verify the core biotargets of calycosin against OS. As results, all primary and core biotargets, biological processes, molecular pathways of calycosin against OS were revealed. Additionally, top 20 biological processes and pathways of calycosin against OS were identified. Telotristat Etiprate In human study, the OS sections resulted in reduced expressions of tumor protein p53 (TP53), Caspase-3 (CASP3), and elevated X-linked inhibitor of apoptosis protein (XIAP) expression in comparison with OS-free controls. As shown in cell culture study, calycosin-treated OS cells showed reduced cell proliferation, and promoted cell apoptosis. In TUNEL stains, calycosin resulted in elevated apoptotic cells. As showed in immunostaining, calycosin-treated OS cells exhibited intracellular up-regulation of TP53, CASP3 expressions, and decreased XIAP expressions. Taken together, the biological informational findings manifest the candidate and core biotargets, molecular functions and pathways of calycosin against OS. Attractively, these core biotargets may be used for effectively detecting and treating human OS. OBJECTIVE To investigate the relation of trochlea morphology and patellofemoral alignment to prevalent radiographic patellofemoral osteoarthritis (PFOA). DESIGN A within-person knee-matched case-control study was performed. Participants with unilateral radiographic PFOA were selected from the Multicenter Population-based Osteoarthritis Study (MPOA) in three towns in northern China that was approved by Peking University Health Science Center Ethics Committee (2018PHB166-01). Radiographic PFOA, radiographic tibiofemoral OA, sulcus angle, patellofemoral index and patella displacement were assessed using posterio-anterior and skyline views of the knee. We classified sulcus angle, patellofemoral index and patella displacement into quarters and investigated the relation of each of these measures to prevalent radiographic PFOA. RESULTS Among 451 participants (mean age 65.2 years, women 66.8%) with unilateral radiographic PFOA, a reverse J-shaped relationship was observed between sulcus angle and prevalent radiographic PFOA (P = 0.039 for quadratic term). Both higher patellofemoral index and patella displacement were associated with higher prevalent radiographic PFOA. Adjusted odds ratios (ORs) of prevalent radiographic PFOA in the highest quarter of patellofemoral index and patella displacement were 4.69 and 3.60 (P for trend less then 0.001), respectively, compared with the lowest quarter of each measurement. A similar relationship was observed between sulcus angle with either prevalent radiographic lateral or medial PFOA. Higher values of patellofemoral index and patella displacement were statistically significantly associated with higher prevalent radiographic lateral (ORs = 5.07 and 4.59, respectively), but not medial PFOA (ORs = 0.58 and 0.76, respectively). CONCLUSION Extreme sulcus angles as well as higher values of patellofemoral index and patella displacement were associated with higher prevalent radiographic PFOA. OBJECTIVES To investigate the effects of lower limb strength training in addition to neuromuscular exercise and education (ST + NEMEX-EDU) compared to neuromuscular exercise and education alone (NEMEX-EDU) on self-reported physical function in patients with knee osteoarthritis (KOA). DESIGN Patient-blinded, parallel-group randomized controlled trial (RCT). METHODS The trial included 90 patients in secondary care with radiographic and symptomatic KOA, ineligible for knee replacement. Both groups exercised twice weekly for 12 weeks. Additional strength training consisted of a single, fatiguing knee extension set (30-60RM) before four sets of leg-press (8-12RM). Primary outcome was the between-group difference on the subscale activities of daily living from the Knee Injury and Osteoarthritis Outcome Score (KOOSADL) at 12 weeks. Secondary outcomes included KOOS symptoms, pain, function during sport and recreation, and quality of life, 40 m walk, stair climb, leg extension power, EuroQol-5D-5L, pain medication usage, and adverse events. RESULTS There was no statistically significant between-group difference in KOOSADL at 12-weeks; adjusted mean difference -1.15 (-6.78 to 4.48). Except for the stair climb test, which demonstrated an adjusted mean difference of 1.15 (0.09-2.21) in favor of ST + NEMEX-EDU, all other outcomes showed no statistically significant between-group differences. Neither group improved leg extension power. CONCLUSION The addition of lower-limb strength training, using a low-dose approach, to neuromuscular exercise and education carried no additional benefits on self-reported physical function or on most secondary outcomes. Both groups displayed similar improvements at 12-week follow-up. Hence, the current low-dose strength training approach provided no additional clinical value in this group of KOA patients.
