Retention of cotton-based gauze or sponge in surgical field, variously called as gauzoma, gossypiboma, textiloma, and muslinoma, is an unexpected and untoward human error that carries sinister consequences for patients and surgeons. Although reported at various surgical sites, no such case has been reported so far in upper limb after wrist surgery.

Here, we present a case of gauzoma in a 24-year-old male who was operated for his right side distal radius fracture at a private hospital. He noticed gradually increasing swelling in his forearm 6 months after operation which persisted after the removal of implant. Initial cytological evaluation suspected soft-tissue tumor and he was referred to our hospital for further management. Plain radiographic, sonographic, and magnetic resonance imaging findings could not ascertain the pathology conclusively. selleck compound Surgical exploration revealed retained gauze piece surrounded by granulation tissues and membrane. Necrotizing fasciitis (NF) is known to be a life-threatening condition of the subcutaneous tissue and superficial fascia with rapid progression.

This case report will present a relevant case and summarize the literature to discuss the dilemma of amputation in patients with NF. We present the case of a 32-year-old male with a longstanding history of intravenous drug abuse, who developed NF from a benign cellulitis after injection drug use in his right groin region, for which he underwent surgical debridement followed by above-the-knee amputation because of septic shock. He was discharged after 6 months from admission with all blood biochemical parameters recovered to normal.

Absorbable materials have been used as fixation devices in orthopaedic surgery. However, their use for treating isolated ulnar diaphyseal fractures in adults has not yet been studied. The aim of this study was to assess whether Inion OTPS™ absorbable implants consisting of L-lactide, D, L-lactide, and trimethylene carbonate provide adequate fixation for the healing of isolated ulnar diaphyseal fractures, their complication rate, and the patients' clinical functional outcome.

Three consecutive patients (all women; mean age, 45 years, and range 38-55 years) with isolated, unstable ulnar fractures were enrolled and treated operatively using Inion OTPS™ absorbable plates and screws. Discontinuation of the study was decided because of the early failure of all implants. The patients were assessed clinically (DASH Score and grip strength) and radiographically at 6 weeks, 3 months, 6 months, and 9 months. The incidence of late foreign body reactions was evaluated for 10 years follow-up period.

Implant failure was noticed radiographically in the early post-operative period in all three patients. Subsequently, one patient was treated operatively using metallic devices, and the other two with prolonged splinting. All fractures healed uneventfully in variable time frames. No foreign body reactions were noticed during and beyond the degradation period, other than a small painless mass in one case.

The results of the current study suggest that the Inion OTPS™ plating system is not appropriate for the fixation of isolated unstable ulnar diaphyseal fractures. It seems that these specific implants cannot withstand the internal mechanical forces of this anatomical area despite the protective splinting.

Giant cell tumors (GCTs) of distal ulna are extremely rare accounting for 0.45%-3.2% of all the cases of GCTs. These are locally aggressive and have a higher rate of recurrence of up to 40% with conservative modality of treatment. Proximity to carpus and diminished range of motion makes their treatment a challenge.

A 27-year-old male presented to us with swelling right distal ulna. X-ray and MRI were suggestive of GCT. The diagnosis was confirmed by core needle biopsy. The patient was managed by wide resection ulna with extensor carpi ulnaris tenodesis.

GCT ulna although very rare in presentation is a possible diagnosis. Wide resection of ulna is a viable treatment option to achieve disease free status. Extensor carpi ulnaris tenodesis helps stabilization of ulnar stump.
