To evaluate the psychological characteristics of Chinese erectile dysfunction (ED) patients, we conducted a cross-sectional study of 153 ED outpatients. Patients were interviewed with the Structured Interview on Erectile Dysfunction (SIEDY) for pathogenic quantification. ED was measured by International Index of Erectile Function (IIEF). Depression and anxiety were evaluated with 9-item Patient Health Questionnaire (PHQ-9) and 7-item Generalised Anxiety Disorder Scale (GAD-7) respectively. Most patients (74.5%) were less then 40 years old. IIEF-5 were significantly correlated with SIEDY scale 3 (r = .16, p = .040) and GAD-7 (p = .15, p = .033). The SIEDY scale 1 increased with age, but the IIEF-5, SIEDY scale 3, PHQ-9 and GAD-7 decreased with age. A negative correlation was observed between ED and psychological stress, which conflicts with many Western-country studies. Younger patients were characterised by milder ED but more psychological stress, while older patients were characterised by worse ED but less psychological stress. Which may be responsible for the conflicting result. Meanwhile, the much younger age distribution among Chinese ED outpatients may indicate that quite a few older ED patients (≥40 years) in China do not seek outpatient service which should merit more attention. © 2020 Blackwell Verlag GmbH.OBJECTIVES To evaluate the agreement between grading of obstetric anal sphincter injuries (OASI) on translabial ultrasound (TLUS), using a newly developed algorithm and on examination, using the current clinical classification. A secondary aim was to assess correlation of tear severity, as defined on ultrasound, with symptoms of anal incontinence and/or fecal urgency. METHODS A retrospective study on 260 patients seen at a perineal clinic between 2012 and 2018 after recent primary repair of OASI. All patients underwent a standardized interview including the St. Mark's anal incontinence score (SMIS) and four-dimensional (4D) TLUS. Post processing of ultrasound volume data was performed blinded against all other data. Using tomographic ultrasound imaging (TUI) a set of eight slices was obtained and the central six slices were evaluated for sphincter abnormalities. Slices with distortion, thinning or defects were rated abnormal. The following algorithm was used to grade OASI 3a tear was diagnosed if less then 4sagreement by two categories. Twenty-four women (11%) were found to have a normal sphincter on imaging. Overall, potential clinical over-diagnosis was noted in 72 women (33%) and potential under-diagnosis In 36 (17%). Seniority of the diagnosing obstetrician did not significantly alter agreement between clinical and sonographic OASI grading (kappa 0.44, 0.43, and 0.34, for specialists, senior and junior residents, respectively). The association between symptoms of anal incontinence and/or fecal urgency and TLUS grading did not quite reach significance (P= 0.052). CONCLUSIONS Clinical and TLUS-based grading of OASI showed fair agreement. Although much is known about the molecular characteristics of the developing hematopoietic cells, we lack a complete understanding of their origin and the three-dimensional organization of the niche. Here, we use advanced live imaging techniques of organotypic slice cultures, clonal analysis, and mathematical modeling to show the two-step process of intra-aortic hematopoietic cluster (IACH) formation. First, a hemogenic progenitor buds up from the endothelium and undergoes division forming the monoclonal core of the IAHC. Next, surrounding hemogenic cells are recruited into the IAHC, increasing their size and heterogeneity. We identified the Notch ligand Dll4 as a negative regulator of the recruitment phase of IAHC. Blocking of Dll4 promotes the entrance of new hemogenic Gfi1+ cells into the IAHC and increases the number of cells that acquire HSC activity. Mathematical modeling based on our data provides estimation of the cluster lifetime and the average recruitment time of hemogenic cells to the cluster under physiologic and Dll4-inhibited conditions. © 2020 The Authors.The realistic prediction of protein-protein complex structures is import to ultimately model the interaction of all proteins in a cell and for the design of new protein-protein interactions. In principle, molecular dynamics (MD) simulations allow one to follow the association process under realistic conditions including full partner flexibility and surrounding solvent. However, due to the many local binding energy minima at the surface of protein partners, MD simulations are frequently trapped for long times in transient association states. We have designed a replica-exchange based scheme employing different levels of a repulsive biasing between partners in each replica simulation. The bias acts only on intermolecular interactions based on an increase in effective pairwise van der Waals radii (repulsive scaling (RS)-REMD) without affecting interactions within each protein or with the solvent. For a set of five protein test cases (out of six) the RS-REMD technique allowed the sampling of near-native complex structures even when starting from the opposide site with respect to the native binding site for one partner.
