To assess whether vessel morphology depicted by three-dimensional (3D) power Doppler ultrasound improves discrimination between benignity and malignancy if used as a second-stage test in adnexal masses that are difficult to classify.

This was a prospective observational international multicenter diagnostic accuracy study. Consecutive patients with an adnexal mass underwent standardized transvaginal two-dimensional (2D) grayscale and color or power Doppler and 3D power Doppler ultrasound examination by an experienced examiner, and those with a 'difficult' tumor were included in the current analysis. A difficult tumor was defined as one in which the International Ovarian Tumor Analysis (IOTA) logistic regression model-1 (LR-1) yielded an ambiguous result (risk of malignancy, 8.3% to 25.5%), or as one in which the ultrasound examiner was uncertain regarding classification as benign or malignant when using subjective assessment. Even when the ultrasound examiner was uncertain, he/she was obliged to classify to classify by subjective ultrasound assessment. #link# For tumors in which the IOTA LR-1 model yields an ambiguous result, subjective assessment is superior to vessel morphology as a second-stage test. © 2020 The Authors. The harmonious interplay between bacteria and host requires fine-tuned regulations by environmental and genetic factors. Thus, the disturbed immune response to microbial components or metabolites and dysbiosis related to immunodeficiency are absolute risk factors to intestinal inflammation and cancer. In this review, we discuss the crosstalk between bacteria and host immunity in the gut and highlight the critical roles of bidirectional regulation between bacteria and the mammalian immune system involved in intestinal inflammation.
To analyse the effect of nurse staffing (number of nurses and staff structure) on patient-safety outcomes in hospitals in China.

The number of nurses has been shown to have a significant effect on patient-safety outcomes, but the structure of the nursing staff has received little attention.

A cross-sectional survey was conducted in 211 units of 13 hospitals. Data on four patient-safety outcomes and numerical and structural indices of nurse staffing were collected from seven types of hospital units.

Nurse staffing had inconsistent effects on units' rates of fall and hospital-acquired pressure ulcers. The nurse-to-patient ratio and rate of nurses with work experience ≤5years predicted fall rates, and rate of hospital-acquired pressure ulcers was positively correlated to the level of nurse staffing (low nurse-to-patient ratio and high bed-to-nurse number).

The number and structure of nurses are both important to patient safety, and the rate of nurses with work experience ≤5years might be a protective factor against adverse events.

Genetic differentiation quickly becomes zero as soon as a few dozen migrants are exchanged per generation. This provides little information to determine whether migration can ensure demographic coupling. The resulting difficulties in delineating conservation units for the management of commercially exploited marine fish species are well illustrated in the case of the European sea bass (Dicentrarchus labrax). Previous attempts to assess connectivity patterns in the northeast Atlantic have been hampered by a lack of spatial genetic structure. In contrast, mark-recapture data suggested low migration rates between regional spawning areas. Here, we show how a spatial gradient of introgressed Mediterranean ancestry across the northeast Atlantic reflects cryptic patterns of genetic and demographic connectivity. Using a 1K SNP chip data set in 827 individuals sampled from Portugal to the North Sea, we found null overall genetic differentiation across the northeast Atlantic. We however detected a subtle latitudinal admixture gradient originating at the edge of the contact zone with the Mediterranean sea bass lineage. Two significant breaks in the ancestry gradient at the tip of Galicia and northern Brittany indicated barriers to effective dispersal between demographically distinct units. Moreover, a northward expansion signal in Irish and North Seas was revealed by the surfing of rare Mediterranean alleles at the edge of the species range. Our results show that introgressed ancestry gradients offer a powerful alternative to assess genetic and demographic connectivity when the neutral migration-drift balance is not informative.
