The pace of technology development with single-use endoscopy has led to a range of disposable ureteroscopes. We review the development of single-use scopes, deconstruct the basic design and functional characteristics of available devices, and discuss future directions for next-generation platforms.

Currently available devices are differentiated on the basis of several core features. The optical, deflection and irrigation characteristics are marginally different with no device clearly superior in every category. Studies comparing single-use ureteroscopes in patients linked to outcomes are limited. The incorporation of next-generation technologies into these platforms include sensors to monitor intrarenal pressure and temperature, suction of fluid and fragments, and computer vision for artificial intelligence.

Each ureteroscope has specific features that may be advantageous in different circumstances. Single-use devices could transform the ureteroscope from a visual conduit to a transformative surgical instrument that improves outcomes and reduces complications.

There is an ongoing explosion in the amount and quality, of research in the field of Endourology. From a solid basis of systematic reviews and small, single centre trials it has been possible to design large randomised controlled trials in the UK and in the USA. This review will describe some of the more recent trials (small and large) that are helping to provide a solid evidence base for our practice in Endourology.

Randomised controlled trial (RCTs) include The Therapeutic Interventions for Stones in the Ureter (TISU), the Percutaneous nephrolithotomy, flexible Ureterorenoscopy and extracorporeal Shockwave lithotripsy for lower pole kidney stones (PURE RCTs) and the Prevention of Urinary Stones with Hydration (PUSH). Quality of life (QoL) measures and studies include the Wisconsin Stone QoL Questionnaire, the Cambridge Renal Stone PROM, the Cambridge Ureteral Stone PROM, the Urinary stone and Intervention QoL questionnaire and the Study to Enhance Understanding of sTent-associated Symptoms (STENTS). The Core Outcome Set in Trials on treatments for Renal and UreteriC sTones (COSTRUCT) study aims to define a core outcome set to be used in future trials.

On-going studies will provide higher quality evidence on the treatment of ureteric and renal stones to inform treatment decision making and guideline recommendations. They will also guide decisions relating to prevention and recurrence and give insight into the true impact of urinary stones and endourological interventions on patients' quality of life. Future studies will incorporate big data and artificial intelligence.

We reviewed the latest guidelines on urolithiasis to highlight the commonalities and differences in the most important recommendations.

Most guidelines utilize systematic review of literature and grade evidence to generate the appropriate recommendations and statements. Only the latest versions of guidelines were included in this review.

A prospective study.

The aim of this study was to investigate the relationship between diffusion tensor imaging (DTI) derived parameters of compressed nerve roots at subregions and the corresponding clinical symptoms to evaluate the patients with intraspinal lumbar disc herniation (LDH)-related lumbosacral radiculopathy pre- and postoperatively.

It is crucial to explore whether magnetic resonanve imaging (MRI) can quantitatively evaluate intraspinal LDH-related lumbosacral radiculopathy before and after surgery.

In all, 66 patients underwent MRI scans and Clinical assessment before and after percutaneous transforaminal endoscopic discectomy (PTED). Pre- and postoperative findings of the related lumbar disk and nerve tractography were compared with two-way contingency table analysis. The embedded paired t test toolbox was applied to respectively compare the fractional anisotropy (FA) and apparent diffusion coefficient (ADC) values of nerves at the symptomatic and asymptomatic sides in three subregions nce 3.
The FA values at subarticular zone can reflect the microstructural changes of the corresponding compressed nerves and well associate with clinical symptoms. Therefore, the DTI parameter FA can be considered an effective tool in clinic to quantitatively evaluate intraspinal LDH-related lumbosacral radiculopathy before and after PTED surgery.Level of Evidence 3.
The authors developed a mathematical model to the sagittal vertical axis (SVA) change in ankylosing spondylitis whom PSO is planned.
