Published by BMJ.BACKGROUND Endometrial cancer is the most common gynecologic cancer, and lymph node metastasis is one of the most important prognostic factors. Increasing evidence shows that sentinel lymph node (SLN) mapping is an effective alternative to comprehensive lymphadenectomy. Single photon emission CT with computed tomography (SPECT/CT) is associated with a high SLN detection rate. OBJECTIVE To compare the clinical efficacy of SPECT/CT with that of lymphoscintigraphy in detecting SLNs in patients with endometrial cancer. METHODS Between May 2014 and October 2018, 151 patients with endometrial cancer were enrolled in this study at the Department of Obstetrics and Gynecology of the Kagoshima University Hospital. Inclusion criteria were patients with endometrial cancer, older than 18 years, and with pre-operative International Federation of Gynecology and Obstetrics (FIGO) staging of I and II. All patients underwent pre-operative CT, and patients with suspected peritoneal dissemination and lymph node metastasis were eymphoscintigraphy (68% (102/151)) (p less then 0.0001). The distribution of SLN locations detected using SPECT/CT was as follows external iliac, 91 (45%); obturator, 61 (30%); common iliac, 28 (14%); internal iliac, 19 (9%); para-aortic, 2 (1%); parametrium, 2 (1%), and presacral, 1 (1%). The SLN detection rate using SPECT/CT was significantly worse in patients of advanced age (p less then 0.0001). DISCUSSION Overall and bilateral SLN detection rate was better with SPECT/CT than with lymphoscintigraphy in patients with stage I/II endometrial cancer. © IGCS and ESGO 2020. No commercial re-use. We used modestly elevated expression of human MYC, driven from the Rosa26 locus, to investigate the pancreatic phenotypes arising in mice from an approximation of MYC trisomy. We show that this level of MYC alone suffices to drive pancreatic neuroendocrine tumours, and to accelerate progression of KRAS-initiated precursor lesions to metastatic pancreatic ductal adenocarcinoma. Our phenotype exposed suppression of the Type I Interferon pathway by the combined actions of MYC and KRAS and we present evidence of repressive MYC/MIZ1 complexes binding directly to the promoters of type I Interferon regulators IRF5, IRF7, STAT1 and STAT2. This boost in glycolytic flux supports proliferation, but also generates acid in the form of hydrogen ions that must be eliminated from the cytoplasm to maintain the alkaline intracellular pH (pHi) associated with transformation. To cope with acid production, tumor cells employ ion transport systems, including the family of sodium-hydrogen exchangers (NHEs). Here, we identify NHE7 as a novel regulator of pHi in pancreatic ductal adenocarcinoma (PDAC). We determine that NHE7 suppression causes alkalinization of the Golgi, leading to a build-up of cytosolic acid that diminishes tumor cell fitness mainly through the dysregulation of actin. Importantly, NHE7 knockdown in vivo leads to the abrogation of tumor growth. These results identify Golgi acidification as a mechanism to control pHi and point to the regulation of pHi as a possible therapeutic vulnerability in PDAC. Copyright ©2020, American Association for Cancer Research.BACKGROUND Chronic kidney disease (CKD) stage 5 presents a high risk for dialysis initiation and for complications such as uremic encephalopathy, uremic symptoms, gastrointestinal bleeding, and infection. Among the most common barriers to health care during CKD stage 5 is poor continuity of care due to unresolved communication gaps. OBJECTIVE Our aim was to determine the potential value of a care model that uses a social networking service (SNS) to improve the quality of CKD health care and delay dialysis initiation. METHODS We used a retrospective cohort of CKD patients aged 20-85 years who received care between 2007 and 2017 to evaluate the efficacy of incorporating an SNS into health care. In 2014, Dr H started to use an SNS app to connect with CKD stage 5 patients and their families. In cases of emergency, patients and families could quickly report any condition to Dr H. Using this app, Dr H helped facilitate "productive interactions" between patients with CKD stage 5 and the health care system. The inten394.8 [318.8] days, P=.039; G3 369.1 [330.8] days, P=.049). After adjusting for baseline eGFR, G4 had a longer duration for each eGFR drop (84.8 [65.1] days) than the other groups (G1 43.5 [45.4] days, P=.005; G2 42.5 [26.5] days, P=.032; G3 38.7 [33.5] days, P=.002). CONCLUSIONS Use of an SNS app between patients with CKD stage 5 and their physicians can reduce the communication gap between them and create benefits such as prolonging time to dialysis initiation. The role of SNSs and associated care models should be further investigated in a larger population.We describe the case of a 6-year-old boy who developed myelin oligodendrocyte glycoprotein antibody (MOG-Ab) associated demyelinating syndrome, after initially presenting with aseptic meningitis. Magnetic resonance imaging showed cerebral and spinal lesions consistent with acute disseminating encephalomyelitis. Rapid clinical improvement occurred after intravenous high dose methylprednisolone. A small number of cases with MOG-Ab associated demyelinating syndrome presenting as aseptic meningitis have previously been reported in adults, but to our knowledge, this is the first pediatric case of this new clinical phenotype.
