Demyelination is the most common cause of optic neuritis. Typical optic neuritis needs intravenous steroids followed by tapering dose oral steroids. Atypical optic neuritis entails clinical manifestations that deviate from the classic pattern of features. Atypical ON can have devastating visual results if not treated in a timely fashion. Thus, it is critical that cases of atypical optic neuritis are recognized early to initiate proper treatment and preserve vision. Our case presented as an atypical case of optic neuritis. The patient had painless onset and was not responding to steroids, but presentation in the other eye and systemic administration of antivirals helped in management of this case. Methods The patient was a 27-year-old male with left eye grade 3 keratoconus with superior cone. He was diagnosed by Sirius Scheimpflug corneal topography (CSO, Florence, Italy). selleck inhibitor The right eye had previously been subjected to penetrating keratoplasty. The uncorrected visual acuity (UCVA) in the right eye (RE) was 1.2 LogMAR, best corrected visual acuity (BCVA) was 0.8 by a refraction of -7.50Ds -3.00Dc @12. The UCVA in the left eye (LE) was 1.2 LogMAR, BCVA was 0.9 by a refraction of -8.50 Ds -5.50Dc @169. Follow-up was done for a period of 12 months after surgery. Results The patient was followed up for 12 months with improvement of visual acuity as regards UCVA and BCVA and improvement of corneal topographic parameters including keratometry and front and back elevations. Conclusion Combined accelerated corneal collagen crosslinking and intrastromal Kerarings implantation by femtosecond laser is an effective method in the treatment of this uncommon type of keratoconus. Methods A 27-year-old female presented with itching in her right eye and head after she had been traveling in southern Italy five days earlier. Visual acuity (VA) was 20/20 in both eyes. Slit-lamp examination showed bilateral blepharitis. Moreover, mobile insects and eggs attached to the eyelashes were observed. A microbiological study was performed with a Phthirus pubis result. The patient was treated with mechanical dislodging. Results The resolution of the infection was carried out removing every insect and egg. A vaseline application twice daily for 7 days was necessary to stifle any nits that could remain. Conclusions The eyelashes pediculosis is frequently caused by Phthirus pubis. Only a minor percentage of the cases are due to Phthirus capitis, but the differential diagnosis is essential there are Phthirus pubis pediculosis cases due to sexual abuse. A sexual history and screening for other sexually transmitted diseases is warranted. We report a case of painless, progressive and bilateral blurring of vision in an HIV-positive 54-year-old lady within months of treatment with anti-retro viral therapy including Efavirenz. On presentation, her visual acuity was 6/18; N24 and 6/9; N10 in both eyes with mottling of the retinal pigment epithelial at the macula with corresponding scotomas on HVF 30-2 and loss of ellipsoid layer on spectral domain optical coherence topography (OCT). Though full field ERG was normal, multifocal ERG revealed reduced foveal and parafoveal amplitudes. Our case emphasizes the need of periodic ocular examination of these patients on long-term EFV. There are multiple causes of folds and tears in Descemet's membrane. In our case, the most likely origin is the mother's prolonged labor, although a severe car accident of the mother at the gestational age of 27 weeks as the cause of these folds cannot be entirely excluded. Methods Single case report. Results The large optic fragment was removed successfully in the operating room. Conclusions While eyes with a posteriorly-displaced lens or IOL remnants may occasionally be observed without intervention, these patients should be monitored closely. Copyright © 2020 Schechet et al.Introduction The migration of a dexamethasone implant to the anterior chamber is a vision-threatening complication which can happen in non-compartmentalized eyes treated with this device. Previous literature suggests that the solution to this complication is almost always surgical and in most cases cannot be delayed. Case description We present the case of a 78-year-old woman with a scleral-fixated IOL and macular edema treated with Ozurdex®. She came to us complaining of blurred vision and was subsequently diagnosed with an anterior-chamber migration of her dexamethasone implant. Postural manoeuvres were performed until the dexamethasone implant returned to the vitreous cavity through the pupil. Pilocarpine drops were prescribed with a positive outcome and no further migrations were described. Discussion This case shows a practical and efficient way of managing a potentially vision-threatening complication without placing the patient onto an operating table. It is interesting to see how it is possible to relocate a dexamethasone implant despite the presence of a scleral-fixated IOL. Conclusion Postural manoeuvres are an interesting option in patients with a dexamethasone implant migrated to the anterior chamber.
