A total of 476 adult patients who had undergone a primary elective unilateral TKA (N=467).
An intervention group received a standardized postoperative rehabilitative exercise protocol taught and supervised by peer volunteers in additional to standard physiotherapy (n=309) compared with a control group receiving standard physiotherapy alone (n=167).
Discharge outcomes were the pain score using the Numeric Rating Scale pain score, passive knee flexion and extension range of motion (ROM), length of hospitalization, ability to perform an unassisted straight leg raise of the operated leg, ambulation distance, ability in independent walking, walking aids required, discharge destination, and adverse events.
On multivariate analysis, patients in the intervention group had an increased discharge passive knee flexion ROM of 7.89 degrees (95% confidence interval, 5.47-10.33; P<.001). There were no significant differences for the other outcome measures between the intervention and control group.

Various fibula cutting guides have been designed to allow for accurate and predictable head and neck reconstruction. In this paper, we describe the design, manufacture, and operative technique for use of a novel fibula cutting guide with detachable connecting arms. This straightforward design produces a guide that is less bulky, provides better access for safe osteotomies, and is versatile and adjustable to minimize bone removal between segments.
Pediatric mandibular fractures are immensely challenging compared to adult fractures. The current update management spectrum ranges from the conservative one in the form of soft diet and regular follow-up, or less invasive surgical intervention by closed reduction and nonrigid fixation, to open reduction and internal fixation with plates and screws. In this study, we investigated the use of a straightforward fabricated mold for the parasymphyseal mandibular fracture in infants with an unerupted dentition.
This prospective study was conducted on 8 infants presenting with parasymphyseal fractures with unerupted dentitions in our specialized trauma center. In the operating room before the induction of anesthesia, the authors used a straightforward plastic airway to create a splint. The curved part was split into 2 transverse halves making 2 U-shaped curved pieces that were utilized as a mold. see more Intraoperatively, the U-shaped piece was placed over the mandible and stabilized with circummandibular wires. This molded airway is used to stabilize the fracture site for 2-3weeks. The average period of follow-up was around 6months.
The average time of mandibular fixation was 17.6±2.4 SD (14 to 20) days. The mean of the total operative time was 38.7±3.5 SD minutes, ranging from 35 to 45minutes. Our infants were observed in the outpatient clinic for 6months postoperatively during the follow-up period. There were no noticeable complications nor any interference with tooth eruption or mandibular growth.
The results of this study suggest that this technique is straightforward to use and affordable. It does not require a long learning period. It also exhibits the advantage of reducing the cost in many developing countries.

To develop a novel classification making full use of radiographic three-dimensional (3D) information to describe the position, geometric configuration of a mandibular third molar (M3M) and its spatial relation to neighboring structures such as the mandibular canal (MC). The classification presented in this study helps to establish a common language and serves as a framework for further studies, for example, on 3D parameters influencing prognosis of the M3M and level of difficulty associated with its removal.
A total of 8 parameters were included in the classification including position, the cranio-caudal height, the number of roots of the M3M as well as its spatial relationship to the MC, its course, its deformation, and distance to the M3M. The classification was retrospectively applied to a sample of 483 patients and these data were statistically evaluated.
The most commonly observed categories were the (+) vertical position (39.8%); the cranio-caudal height A (38.7%); 2 roots (83.2%); and the spatial relationship no contact caudal (37.5%). Contact between the M3M and inferior alveolar nerve was observed in 58% of all cases. A deformation of the MC was found in 81% of cases.
Our novel classification aims to provide a more holistic description of parameters defining 3D M3M position, as well as its spatial relation to relevant neighboring structures. It creates a uniform terminology that can be applied in practice by surgeons and radiologists, and also serves as a foundation for further research projects, such that results from similarly designed studies could be compared.
