Echocardiography in the setting of resuscitation can provide information as to the cause of the cardiac arrest, as well as indicators of futility. This systematic review and meta-analysis were performed to determine the value of point-of-care ultrasonography (PoCUS) in the assessment of survival for adult patients with cardiac arrest.

This meta-analysis was performed in adherence to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. PubMed, EMBASE, Web of Science, Cochrane have been searched from databases inception until March 2nd 2021. The search was limited to adult patients with cardiac arrest and without publication dates or country restrictions. Papers were chosen if they met the required criteria relating to the sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of this diagnostic technique concerning resuscitation outcomes.

This systematic review identified 20 studies. Overall, for survival to hospital discharge, PoCUS was 6.2% sensitivity (95% confidence interval [CI] 4.7% to 8.0%) and 2.1% specific (95% CI 0.8-4.2%). PoCUS sensitivity and specificity for return of spontaneous circulation were 23.8% (95% CI 21.4-26.4%) and 50.7% (95% CI 45.8-55.7%) respectively, and for survival to admission 13.8% (95% CI 12.2-15.5%) and 20.1% (95% CI 16.2-24.3%), respectively.

The results do not allow unambiguous recommendation of PoCUS as a predictor of resuscitation outcomes and further studies based on a large number of patients with full standardization of operators, their training and procedures performed were necessary.

Eighty-eight wild boars were managed by authorized controllers between 2017 and 2019 in the state of Paraná in southern Brazil. Management georeferencing, sex, and weight were recorded for each animal. The presence of Mhyo antibodies in wild boar serum samples was evaluated using a commercial indirect ELISA kit. The presence of enzootic pneumonia-like gross lesions was evaluated, and the observed macroscopic lesions were subjected to immunohistochemistry (IHC). The Chi-square test and the intensity of the association with the odds ratio and 95% confidence interval were used to evaluate the differences in the qualitative variables between groups (sex and municipality). selleck products Juvenile wild boars exhibited a higher seroprevalence than older ones (p = 0.005). The Teixeira Soares municipality differed in Mhyo seroprevalence in comparison with Castro (p  less then  0.001), Ponta Grossa (p = 0.004), and Carambeí (p  less then  0.001). Females were 6.79 times more likely to present consolidation lesions than males (p = 0.004). Among the evaluated lung samples with injuries, 57.1% (8/14) and 53.8% (7/13) were Mhyo positive by IHC in Castro and Ponta Grossa, respectively, confirming that the identified macroscopic lesions were caused by Mhyo. This study demonstrates the circulation of Mhyo in free-living wild boars, which raises concerns regarding the epidemiological role of this animal species for the spread of the pathogen. The race of an individual is a salient physical feature that is rapidly processed by the brain and can bias our perceptions of others. How the race of others explicitly impacts our actions toward them during intergroup contexts is not well understood. In the current study, we examined how task-irrelevant race information influences cognitive control in a go/no-go task in a community sample of Black (n = 54) and White (n = 51) participants. We examined the neural correlates of behavioral effects using functional magnetic resonance imaging and explored the influence of implicit racial attitudes on brain-behavior associations. Both Black and White participants showed more cognitive control failures, as indexed by dprime, to Black versus White faces, despite the irrelevance of race to the task demands. This behavioral pattern was paralleled by greater activity to Black faces in the fusiform face area, implicated in processing face and in-group information, and lateral orbitofrontal cortex, associated with resolving stimulus-response conflict. Exploratory brain-behavior associations suggest different patterns in Black and White individuals. Black participants exhibited a negative association between fusiform activity and response time during impulsive errors to Black faces, whereas White participants showed a positive association between lateral OFC activity and cognitive control performance to Black faces when accounting for implicit racial associations. Together our findings propose that attention to race information is associated with diminished cognitive control that may be driven by different mechanisms for Black and White individuals. The gene encoding hepatocyte nuclear factor 1β (HNF1B), a transcription factor involved in the development of the kidney and other organs, is located on chromosome 17q12. Heterozygous deletions of chromosome 17q12, which involve 15 genes including HNF1B, are known as 17q12 deletion syndrome and are a common cause of congenital anomalies of the kidneys and urinary tract (CAKUT) and may also present as a multisystem disorder. Autosomal recessive polycystic kidney disease (ARPKD), on the other hand, is a severe form of polycystic kidney disease caused by mutations in PKHD1 (polycystic kidney and hepatic disease 1). It is important to differentiate between these two diseases because they differ significantly in inheritance patterns, renal prognosis, and extrarenal manifestations. Here we report a case of 17q12 deletion syndrome that clinically mimicked ARPKD in which genetic testing was essential for appropriate genetic counseling and monitoring of possible extrarenal manifestations. The patient presented antenatally with markedly enlarged kidneys and showed bilaterally hyperechoic kidneys with poor corticomedullary differentiation and multiple cysts on ultrasonography.
