Chelonians are increasingly challenged by anthropogenic threats and disease. This article summarizes recent literature and clinical experiences regarding 4 emerging infectious diseases in turtles and tortoises ranaviruses, cryptosporidiosis, intranuclear coccodiosis of Testudines, and Emydomyces testavorans. Recently, multiple infectious organisms have been identified as the cause of emerging diseases in lagomorphs. The most important of these emerging diseases is rabbit hemorrhagic disease virus (RHDV) type 2, a new variant with differences in pathogenicity to classical RHDV. This article provides a recent review of the published literature on emerging infectious diseases in rabbits, including farmed, laboratory, and pet rabbits, some of which have zoonotic potential. BACKGROUND The conservative treatment of peripheral arterial disease (PAD), as recommended in current guidelines, encompasses measures such as lifestyle modification and risk-factor management. In addition, in patients with vasogenic intermittent claudication (IC), it is recommended that patients first be given drugs to improve perfusion and undergo supervised gait training. Revascularization is not recommended for asymptomatic persons, but it is considered mandatory for patients with critical ischemia. METHODS In 2014, the German Society for Angiology carried out a comprehensive literature search in order to prepare a new version of the S3 guideline on PAD. CONCLUSION In the treatment of vasogenic IC, SET and drugs to increase perfusion are now giving way to revascularization, which is more effective. As far as can be determined, SET is not currently implemented at all in the German health care system. It would be desirable for SET to be more available and more widely used, both to sustain the benefit of revascularization over the long term and to lower the general cardiovascular risk.BACKGROUND The percentage of organ donors among the German population is very low, and the supply of organs does not meet the demand. Relatively few organ explantations are performed in Germany, compared to other countries. The central element leading up to organ explantation is the donor's consent. This can be expressed either orally or in writing, in the form of an advance directive or organ donor card. The goal of this study was to find out how many patients in hospital resuscitation rooms had organ donor cards on their person. METHODS The study population encompassed all 2044 patients treated in the trauma surgery resuscitation room at the Essen University Hospital from 1 February 2017 to 31 March 2019. The data were retrieved from protocols of patient possessions made on admission. The measures taken to enhance data quality were sensitization of the documenting nursing staff and documentation by the first author himself. The literature led us to expect a card-carrier rate of 36%. RESULTS Over the entire period of the study, 17 organ donor cards were found (0.8%; 95% confidence interval, [0.5; 1.3]). The figures in all patient subgroups were markedly below the expected 36%. The low rate of carrying organ donor cards could not be explained by a poor quality of documentation. CONCLUSION The organ donor card system as it currently exists in Germany is inadequate. These cards are not carried in the emergency situations for which they were developed.BACKGROUND The use of e-cigarettes is on the rise around the world. Many case reports of acute lung injury due to e-cigarette use have been published in recent months in the USA, but no comparable cases have emerged in Germany up to the present report. The use of e-cigarettes has risen very rapidly in the USA in recent years, simultaneously with the legalization of marijuana sale in many American states. Most of the cases described there involved the use, not only of nicotine, but of tetrahydrocannabinol (THC, the psychoactive ingredient in marijuana) as well, though some of the patients had indeed not used additives (e.g. THC). METHODS We report three cases in Germany of acute pulmonary illness that we consider to have been caused by the use of e-cigarettes. RESULTS All three patients were hospitalized for acute shortness of breath. Two displayed partial respiratory insufficiency and bilateral pulmonary infiltrates. All three stated that they had used ordinary, commercially available e-cigarettes every day fypical clinical picture; rather, their symptoms represent different components of the wide spectrum of interstitial lung disease. A uniform national registry should be established to improve our understanding of the adverse effects of e-cigarettes and the resulting acute and chronic changes in the lungs.General practice is arguably the ideal setting to initiate advance care planning (ACP), but there are many barriers. This pilot study was designed to assess the feasibility, acceptability and perceived utility of a nurse-facilitated screening interview to initiate ACP with older patients in general practice. Patients were recruited from four general practices in Sydney, Australia. General practice nurses administered the ACP screening interview during routine health assessments. Patients and nurses completed a follow-up questionnaire consisting of questions with Likert responses, as well as open-ended questions. Descriptive statistics and content analysis were used to analyse the data. Twenty-four patients participated; 17 completed the follow-up questionnaire. All patients found the ACP screening interview useful and most felt it would encourage them to discuss their wishes further with their family and general practitioner. Several patients were prompted to consider legally appointing their preferred substitute decision-maker.
