These mostly negative findings indicate absent or subtle association between salivary cortisol and cognitive decline.Objective To investigate fluid flow through enamel in intact human premolars in vivo and in vitro by using the replica technique. Design The experiments were done on 17 premolars of 10 subjects (aged 10-25 yrs) to be extracted during orthodontic treatment. Teeth were divided into 2 groups. In the first group (n = 11), the fluid accumulated on the enamel surface was recorded with the impression material in the mid-buccal enamel both in vivo and in vitro. In vitro replicas were obtained when the pressure in the pulp cavity held at 0, +20, +100, and +200 mmHg. They were examined by a scanning electron microscope (SEM). For the second group (n = 10), each tooth was prepared for fluid flow measurement during which the pulpal pressures of +20, +100, +200, and +300 mmHg were applied. Results The mean number of droplets presented after the pressure had been raised to 200 mmHg is significantly greater than those observed at lower pressures, including those obtained from the same tooth in vivo. A significant linear relationship between these numbers and the pulpal pressures was found (p less then 0.001). In vitro fluid conductance through the crown also increased significantly with increased pressure (p less then 0.001). Conclusion SEM evaluation of droplets distribution on enamel surface revealed that the presence of fluid transudation forced by pulpal pressure may influence the accumulation of enamel fluid on the tooth surface in vitro, similar to those observed in vivo.Breast cancer diagnosis and staging is based on mammography, ultrasound, and magnetic resonance imaging (MRI). Contrast enhanced spectral mammography (CESM) has gained momentum as an innovative and clinically useful method for breast assessment. CESM is based on abnormal enhancement of neoplastic tissue compared to surrounding breast tissue. We performed a systematic review of prospective trial to evaluate its diagnostic performance, following standard PRISMA-DTA. We used a bivariate random-effects regression approach to obtain summary estimates of both sensitivity and specificity of CESM. 8 studies published between 2003 and 2019 were included in the meta-analysis for a total of 945 lesions. The summary area under the curve obtained from all the study was 89% [95% CI 86%-91%], with a sensitivity of 85% [95% CI 73%-93%], and a specificity of 77% [95% CI 60%-88%]. With a pre-test probability of malignancy of 57% a positive finding at CESM gives a post-test probability of 83% while a negative finding a post-test probability of 20%. CESM shows a suboptimal sensitivity and specificity in the diagnosis of breast cancer in a selected population, and at present time, it could be considered only as a possible alternative test for breast lesions assessment when mammography and ultrasound are not conclusive or MRI is contraindicated or not available.Background The combination of chemotherapy plus anti HER-2 agents is the mainstay of HER-2 positive advanced breast cancer (ABC) therapy. We conducted a phase II trial testing activity and safety of trastuzumab and metronomic capecitabine/cyclophosphamide (HEX) as first-line therapy in HER-2 positive ABC. Methods. Patients at first relapse or with synchronous metastasis were treated with trastuzumab (4 mg/kg, biweekly) plus oral capecitabine (1500 mg/daily) and cyclophosphamide (50 mg/daily). Primary endpoint was objective response rate (ORR), secondary endpoints progression-free survival (PFS), clinical benefit rate (CBR; PR + CR + SD for ≥ 24 weeks) and tolerability. Optimal two-stage design was applied. Results Sixty patients with measurable ABC, tumors scored as +3 for HER-2 or FISH +, untreated for advanced disease were enrolled. Median age was 62.5 years, visceral metastases were present in most patients (57.9%). Median number of cycles was 16 (range 1-98). ORR was 56.7% (95% CI, 44.1-68.4%), with 5 CR (8.3%) and 29 PR (48.3%). Fifteen patients had SD (25%). The CBR was 78.2%. Nine progressions were observed (15%). Median PFS was 11 months. One year PFS was 47.7%. Median OS was 45.9 months. Worst toxicities were grade 3 hand-foot syndrome in 2 pts (3.3%), grade 3 anaemia in 2 pts (3.3%), grade 2 nausea in 2 pts (3.3%) and grade 3-4 diarrhea in 2 pts (3.3%). Cardiac toxicity grade 1 was reported in 1 pt. Conclusions Combination of trastuzumab and metronomic oral chemotherapy has clinical activity. The tolerability was excellent and allowed the prolonged delivery of treatment.Emulsion polymerization provides a sustainable way to produce latex polymers for coatings and adhesives thanks to the use of water as a dispersion medium. This synthesis approach can be even more attractive if synthetic surfactant can be replaced by biobased solid particles as a stabilizer, through what is known as a "Pickering effect". Herein, latex dispersions with solid content up to 35 wt% were successfully produced by emulsion polymerization using starch nanocrystals (SNCs) as a sole stabilizer and H2O2/citric acid as a redox-initiator. The effect of the SNC modification with vinyltriethoxysilane (VTES) on the colloidal properties of the polymer dispersion and performance of the resulting nanocomposite film were investigated. As an application of this approach, pressure-sensitive adhesive (PSA) dispersions have been prepared via Pickering emulsion polymerization in the presence of 8 wt% SNCs. The use of VTES-SNCs has a beneficial impact on the performance of PSAs with improved peel strength and wettability. The possibility to use SNCs as a stabilizer to replace synthetic surfactants in emulsion polymerization opens new avenues for the application of SNCs as biobased Pickering stabilizers to produce latex for coatings, adhesives, inks, and textiles.Recombinantly engineered bacterial outer membrane vesicles (OMVs) are promising vaccine delivery vehicles. The diversity of exogenous antigens delivered by OMVs can be enhanced by induced fusion of OMV populations. To date there are no reports of induced fusion of bacterial OMVs. Here we measure the pH and salt-induced aggregation and fusion of OMVs and analyze the processes against the Derjaguin-Landau-Verwey-Overbeek (DLVO) colloidal stability model. Vesicle aggregation and fusion kinetics were investigated for OMVs isolated from native E. coli (Nissle 1917) and lipopolysaccharide (LPS) modified E. coli (ClearColi) strains to evaluate the effect of lipid type on vesicle aggregation and fusion. Electrolytes and low pHs induced OMV aggregation for both native and modified LPS constructs, approaching a calculated fusion efficiency of ~25% (i.e. ~1/4 of collision events lead to fusion). AICAR mw However, high fusion efficiency was achieved for Nissle OMVs solely with decreased pH as opposed to a combination of low pH and increased divalent counterion concentration for ClearColi OMVs.
