The mediation analysis suggested that NAFLD accounted for ~36% (95% CI 27, 44) of the effect (direct effect HR=3.2, 95% CI 2.3, 4.6; indirect effect through NAFLD, HR=1.4, 95% CI 1.3, 1.5).

Hemiarthroplasties cause damage to the cartilage that they articulate against, which is a major limitation to their use. This study investigates the use of lower stiffness materials to determine if they improve hemiarthroplasty contact mechanics and thus reduce the risk of cartilage damage.

Eleven fresh-frozen cadaveric upper extremities were disarticulated and fixed in a custom-built jig that applied a static load of 50 N to the radiocapitellar joint. Flexion angles of 0°, 45°, 90° and 135° were tested with radial head implants made of cobalt-chrome (CoCr) and ultra-high weight molecular-polyethylene (UHMWPE), compared to the native radial head. A Tekscan thin-film sensor was used to measure the contact area and contact pressure between the radius and capitellum.

UHMWPE and CoCr are too stiff in the application of hemiarthroplasty resulting in lower contact areas and higher contact pressures relative to the native joint. The native contact area was on average 42±20 mm
larger than UHMWPE (p<0.001), and 55±24 mm
larger than CoCr (p<0.001). UHMWPE had a contact area 13±10 mm
greater than CoCr (p=0.014).

This study shows that even though UHMWPE has a stiffness several times lower than CoCr, the use of this material in hemiarthroplasty lead to only a minor improvement in contact mechanics. Neither implant restored contact similar to the native articulation.

We aimed to summarize the current literature for rotator cuff intact osteoarthritis treated with primary reverse shoulder arthroplasty and to determine whether morphological changes in the glenoid led to inferior outcomes.

A literature search was performed which utilized an inclusion criteria of primary reverse shoulder arthroplasty for osteoarthritis with intact rotator cuff. Glenoid bone loss can be effectively managed with reverse's robust glenoid fixation, with and without the use of bone graft. The capability to lateralize the joint center of rotation may be valuable when faced with a medialized glenoid wear pattern. The current findings suggest that reverse arthroplasty can achieve highly favorable outcomes for glenohumeral osteoarthritis with intact rotator cuff.
