Head-to-head comparison of treatment failure and costs among chronic obstruct pulmonary disease (COPD) patients who used noninvasive ventilation (NIV) in the ward versus in the ICU is lacking.
This retrospective study was performed in a department of respiratory and critical care medicine in a teaching hospital. COPD patients who used NIV in the respiratory ward or respiratory ICU were screened. We enrolled patients with PaCO
more than 45 mmHg and pH less than 7.35 before the use of NIV.
We enrolled 83 patients who initiated NIV in the ward and 319 patients in the ICU. Only 5 (6%) patients in the ward were required to transfer to ICU for intensive care. The vital signs were worse but improved faster within 24 h of NIV among patients in the ICU than those in the ward. The NIV failure, hospital mortality, and the length of stay in hospital did not differ between the two groups. However, the duration of NIV was shorter (median 4.0 vs. 6.1 days,
< 0.01) and hospital costs were higher (median 4638 vs. 3093 $USD,
< 0.01) among patients in the ICU than those in the ward. After propensity matching, 42 patients were left in each group, and the baseline data were comparable between the two groups. The findings in the overall cohort were confirmed again in the propensity-matched cohort.

The aim of our study was to evaluate the diagnostic characteristics of selected inflammatory markers and the results of multiplication of their concentrations in the diagnosis and assessment of Crohn's disease (CD) activity.
We studied 49 patients with CD and 31 healthy controls. The CD patients were assigned to subgroups with active and inactive disease based on the Crohn's Disease Activity Index score. Serum interleukins and C-reactive protein (CRP) were measured using immunoassays.
Serum CRP and interleukins IL-6, IL-17A, IL-23 were significantly higher in the CD group than in controls, with the best diagnostic performance for IL-23. Only serum IL-6 and CRP were significantly higher in active than in inactive disease, with the better performance of CRP. Multiplication results did not perform better than individual multipliers.
Serum CRP may be useful in the assessment of CD activity and there is a need for introduction of IL-23 for the CD diagnosis.

Annual health expense of hospital admissions, due to venous thromboembolism including pulmonary embolism, exceeds 10 billion dollars in the United States. Most of these patients still get admitted to the hospital despite the advent of novel oral anticoagulants. Our aim is to show that low-risk pulmonary embolism patients can safely be discharged from the emergency department with similar patient satisfaction and lower length of stay.
A comprehensive search in Medline indexed and non-indexed, Embase, and Cochrane Central was performed to search for all the randomized controlled trials that compared inpatient treatment of low-risk pulmonary embolism to outpatient treatment.
Of 68 potentially relevant studies, a total of 2 studies (453 participants) met our inclusion criteria and had data available on patient satisfaction, length of stay, efficacy, and patient safety. The pooled estimate of the included studies showed that at 3-month follow-up, there was no statistically significant difference between inpatient and outpatient treatment of these low-risk patients.
In conclusion, our meta-analysis of 2 randomized controlled trials shows that low-risk pulmonary embolism patients can safely be discharged from the emergency departments in the limited studies available. We need more randomized controlled trials to confirm these findings.
