Unfortunately, in clinical experience, the bipolar clamps have not been as effective, and often require multiple ablations to create conduction block. This study created a new experimental model using fresh, cardioplegically-arrested human hearts turned down for transplantation to evaluate the performance of a non-irrigated bipolar RF clamp. Methods Nine human hearts turned down for transplantation were harvested and the Cox-Maze IV lesion set was performed with a non-irrigated bipolar RF clamp. In the first seven hearts, a single ablation was performed for each lesion. In the last two hearts, a set of two successive ablations without unclamping were performed. The heart tissue was stained with 2,3,5-triphenyl-tetrazolium chloride. Each ablation lesion was cross-sectioned to assess lesion depth and transmurality. Results A single ablation with the bipolar RF clamp resulted in 89% (469/529) of the histologic sections and 65% (42/65) of the lesions being transmural. Of the non-transmural sections, 92% occurred in areas with epicardial fat. Performing two successive ablations without unclamping resulted in 100% of the cross-sections (201/201) and lesions (25/25) being transmural. Conclusions A single ablation failed to create a transmural lesion 35% of the time, and this was associated with the presence of epicardial fat. Niemann-Pick type C disease (NPCD), a LSD characterized by liver damage and progressive neurodegeneration that leads to early death, is caused by mutations in the genes encoding the NPC1 or NPC2 proteins. Both proteins are involved in the transport of cholesterol from the late endosomal compartment to the rest of the cell. We will particularly focus on the activation of signaling pathways that may be common to these three pathologies with emphasis on how the intra-lysosomal accumulation of lipids leads to pathology, specifically to neurological impairments. We will show that although the primary lipid storage defect is different in these three LSDs, there is a similar secondary accumulation of metabolites and activation of signaling pathways that can lead to common pathogenic mechanisms. This analysis might help to delineate common pathological mechanisms and therapeutic targets for lysosomal storage diseases.Seizure is a common acute and severe disease in infants and children. Recurrent seizures or persistent seizures may cause irreversible brain damage. Mitogen activated protein kinase (MAPK) signaling pathway is associated with an inflammatory response, however it's involvement in the pathological process of seizures is not clear. Histone deacetylase inhibitors (HDACi) have promising neuroprotective effects through epigenetic regulation. Therefore, this study aimed to investigate the mechanism of HDACi MS275 on p38 MAPK signaling pathway and p38 histone modifications in developing rats post-seizure. Intraperitoneal administration of Pentylenetetrazole (PTZ) was used to induce developing rat seizures, and MS275 (5 or 10 mg/kg) was injected intraperitoneally 2 h before PTZ injection. Hippocampal tissues were sampled at 24 h post-seizures for protein and mRNA levels of p38、MK2、CREB and IL-6. Neuronal apoptosis and microglia activation significantly increased after PTZ treatment. However, pretreatment with MS275 attenuated these effects as well as increased seizure latency and decreased seizure scores. Furthermore, MS275 was found to inhibit the expression of p38 by increasing histone H3 and H4 acetylation and decreasing histone H3 and H4 methylation. This study thereby demonstrates that HDACi MS275 can reduce the inflammatory response associated with seizure-induced brain injury through inhibiting the p38 MAPK signaling pathway and p38 gene expression.
