To assess the impact of the removal of the target lymph node (TLN) on therapy after the completion of primary systemic therapy (PST) in initially node-positive breast cancer patients.

Pooled data analysis of participants of the prospective CLIP- and TATTOO-study at the University of Rostock was performed.

A total of 75 patients were included; 63 of them (84.0%) converted to clinically node-negative after PST. Both TLN and sentinel lymph node (SLN) were identified in 41 patients (51.2%). In five out of 63 patients (7.9%), the TLN was metastatic after PST and the SLN was either tumor-free or not detected. Axillary lymph node dissection (ALND) was conducted in all five patients. In one patient, systemic therapy recommendation was influenced by the TLN; adjuvant radiotherapy was influenced by the TLN in zero patients. For patients with fewer than three removed SLNs, the FNR was 28.6% for the SLN biopsy alone and 7.1% for targeted axillary dissection (TAD).

Moreover, non-orthogonal multiple-access (NOMA)-enabled schemes have recently emerged as a promising multiple-access scheme for these networks that would allow realization of the target spectral efficiency and user fairness requirements. The integration of NOMA in the widely adopted orthogonal frequency-division multiplexing (OFDM)-based VLC networks would require an optimal resource allocation for the pair or the cluster of users sharing the same subcarrier(s). In this paper, the max-min rate of a multi-cell indoor centralized VLC network is maximized through optimizing user pairing, subcarrier allocation, and power allocation. The joint complex optimization problem is tackled using a low-complexity solution. At first, the user pairing is assumed to follow the divide-and-next-largest-difference user-pairing algorithm (D-NLUPA) that can ensure fairness among the different clusters. Then, subcarrier allocation and power allocation are solved iteratively through both the Simulated Annealing (SA) meta-heuristic algorithm and the bisection method. The obtained results quantify the achievable max-min user rates for the different relevant variants of NOMA-enabled schemes and shed new light on both the performance and design of multi-user multi-carrier NOMA-enabled centralized VLC networks.(1) Introduction and Aim The aim of this study is to investigate the prognostic value, in terms of response and survival, of CT-based radiomics features for patients with HCC undergoing drug-eluting beads transarterial chemoembolization (DEB-TACE). The prognostic value of texture analysis was evaluated, investigating the correlation between radiomics features, response to therapy and overall survival. The three models built for survival prediction yielded an HR of 2.19 (95% CI 2.03-2.35) using texture features, of 1.7 (95% CI 1.54-1.9) using clinical data and of 4.61 (95% CI 4.24-5.01) combining both radiomics and clinical data (all p less then 0.0001). (4) Conclusion Texture analysis based on pre-treatment CT examination is associated with response to therapy and survival in patients with HCC undergoing DEB-TACE, especially if combined with clinical data.Mesothelioma is an aggressive disease arising from parietal pleura. Surgery is a valuable option in the frame of a multimodality treatment. Several surgical approaches have been standardized with the aim of a macroscopic complete resection; these often require homolateral diaphragm and pericardial resection and reconstruction. Extrapleural pneumonectomy (EPP) and extended pleurectomy decortication (EPD) have been recognized as radical surgical procedures. Nevertheless, both operations are technically challenging and associated with a significant rate of peri-operative morbidity and non-negligible mortality. The diaphragmatic and pericardial reconstruction technique is mandatory to avoid respiratory impairment and to reduce post-operative complications like gastric and cardiac herniation. Moreover, in the case of localized chest wall recurrence, surgery might be considered a valuable therapeutical option for highly selected and fit patients. All the technical aspects of the resection and reconstruction of the diaphragm, pericardium, and chest wall are described as well as the possible use of new minimally invasive techniques. In addition, the choice of different prosthetic materials, considering the most recent innovations in the field, are discussed.The antioxidant and antidiabetic properties and metabolite profiling of ethanol extracts of red cabbage (RC) and broccoli (BR) seeds and sprouts were investigated in this study. The total phenolic, flavonoid, and saponin contents were in the ranges of 385.4-480.4 mg FAE/100 g, 206.9-215.6 mg CE/100 g, and 17.8-27.0 mg soysaponin BE/100 g, respectively. BR seed had the highest total phenolic (480.4 mg FAE/100 g) and flavonoid (216.9 mg CE/100 g) contents, whereas BR sprout had the highest saponin content (27.0 soysaponin BE/100g). RC sprout demonstrated the highest antioxidant capacity, with DPPH and ABTS radical scavenging activity levels of 71.5% and 88.5%, respectively. Furthermore, BR and RC sprouts showed the most potent inhibition against α-glucosidase (91.32% and 93.11%, respectively) and pancreatic lipase (60.19% and 61.66%, respectively). BR seed (60.37%) demonstrated the lowest AGE inhibition. A total of 24 metabolites, predominantly amino acids and phenolic compounds, were characterized using UHPLC-QTOF-MS/MS.
